Apical horizontal incision with periosteum graft wall technique for periodontal regeneration: A case study.

BACKGROUND: The periosteum consists of an outer fibrous layer and an inner cellular layer, where bone cells reside. Hence, it has been suggested that applying periosteum to a periodontal defect may help new bone formation. The purpose of this case study is to present the clinical and radiographic outcomes of a vestibular regenerative approach and the application of a connective tissue graft (CTG) with periosteum to improve the periodontal prognosis of a pathologically migrated hopeless tooth with an endo-periodontal lesion (EPL). METHODS: A 35-year-old female presented with a hopeless tooth w

Using hydroxylapatite ceramic buttons covered with periosteum to prevent cerebrospinal fluid leakage: technical note.

BACKGROUND: Many attempts have been made to prevent cerebrospinal fluid (CSF) leakage, but the procedures proposed to date are troublesome and not promising. We describe a method of completely preventing CSF leakage using hydroxylapatite ceramics. METHODS: Exposed frontal sinus, mastoid air cells, and frontal base defects caused by fracture are covered with periosteum or fascia and plugged with a trimmed hydroxylapatite ceramic button. RESULTS: There was no CSF leakage, postoperative meningitis, or other complication related to the technique in a consecutive series of 25 patients treated usi

Dual immuno-mechanical programming of a biomimetic periosteum for load-bearing bone restoration.

Age-related cortical bone loss remains a major clinical challenge, as current interventions largely neglect the load-bearing cortex and its periosteal niche. Here, a dual-targeted biomimetic periosteum is engineered by mechanistically decoding how inflammaging and mechanical decline converge to impair periosteal stem/progenitor cell (PSC) function. Aging-associated macrophage polarization toward a pro-inflammatory M1 state suppresses Piezo1 expression in PSCs, whereas reduced mechanical input attenuates Piezo1 activation, jointly leading to defective mechanotransduction and osteogenesis. Collagen molecular organization preservation in human fascia lata and periosteum after tissue engineering.

Large bone defect regeneration remains a major challenge for orthopedic surgeons. Tissue engineering approaches are therefore emerging in order to overcome this limitation. However, these processes can alter some of essential native tissue properties such as intermolecular crosslinks of collagen triple helices, which are known for their essential role in tissue structure and function. We assessed the persistence of extracellular matrix (ECM) properties in human fascia lata (HFL) and periosteum (HP) after tissue engineering processes such as decellularization and sterilization. Construction of bilayer biomimetic periosteum based on SLA-3D printing for bone regeneration.

An ideal biomimetic periosteum should have excellent biocompatibility to promote osteoclast adhesion and improve osseointegration, which is significant in promoting bone regeneration. In this work, a bionic artificial periosteum printed by the SLA-3D printing was prepared, consisting of a poly (ethylene glycol) diacrylate (PEGDA)/chitosan/tricalcium phosphate (TCP) fibrous layer and a gelatin methacryloyl (GelMA)/ammonium molybdate (Mo) cambium layer. Distinct surface characteristics were achieved on both sides of the biomimetic periosteum. Among them, the fibrous layer has high mechanical pro

Necessity of infraorbital wall reconstruction in patients with residual periosteum after total maxillectomy.

BACKGROUND: To date, there have been no reports regarding the necessity or methods of reconstruction for cases where the orbital periosteum remains even though the orbital floor bone has been resected in primary reconstruction after total maxillectomy. This study investigated the reconstruction methods for infraorbital wall reconstruction in patients with residual orbital periosteum. METHODS: This was a retrospective cohort study at a single center from June 2010 to June 2019. The following two main cohorts were evaluated: non-reconstruction and reconstruction groups (reconstructed with fasci

Structural and cellular differences between metaphyseal and diaphyseal periosteum in different aged rats.

In both physiological and pathological processes, periosteum plays a determinant role in bone formation and fracture healing. However, no specific report is available so far focusing on the detailed structural and major cellular differences between the periostea covering different bone surface in relation to ageing. The aim of this study is to compare the structural and cellular differences in diaphyseal and metaphyseal periostea in different aged rats using histological and immunohistochemical methods. Four female Lewis rats from each group of juvenile (7 weeks old), mature (7 months old) and

A Vascularized Bone-on-Paper Platform To Study Periosteum-Vascular Crosstalk and Angiogenic Remodeling.

Osteoarthritis (OA) is a common degenerative joint disease in the elderly. It is characterized by articular cartilage degradation and periosteal bone spur formation. These changes often lead to pain, tissue compression, and disability. Periosteum-derived progenitor cells (PDPCs) play a crucial role in periosteal reactions, relying on vascular endothelial cells (VECs) for signal transduction through vascular networks. While bone-related studies often emphasize tissue regeneration, the intercellular signaling mechanisms driving OA progression remain underexplored. This study developed a vascular

A bioinspired bilayer periosteum-mimicking membrane for enhanced osteogenesis and angiogenesis in guided bone regeneration.

Guided bone regeneration (GBR) membranes with osteoinductive and angiogenic capabilities are essential for the effective repair of large bone defects. In this study, we design and fabricate a biomimetic bilayer GBR membrane composed of a nano-hydroxyapatite/type I collagen/ polycaprolactone (HC layer) and chitosan/ polycaprolactone (CP layer) composite via electrospinning strategy. The core-shell fibrous architecture replicates the natural periosteum's fibrous hierarchy and provides controlled collagen release, mechanical integrity, and enhanced interfacial functionality. The physical, chemica

Periosteum as a lateral pedicle graft for the treatment of single tooth root recession: A novel approach.

The treatment of gingival recession defects is indicated for esthetic and functional reasons to reduce root sensitivity, to remove muscle pull, to create or augment keratinized tissue, and to prevent disease progression. The presence of sufficient amount of periosteum adjacent to gingival recession defects makes it a suitable graft. The adult human periosteum is a highly vascular connective tissue with immense regenerative potential. It contains fibroblasts, osteogenic progenitor cells, and stem cells as a result of which it has the ability to differentiate into fibroblast, osteoblast, chondro

[Interposition of periosteum in joint fractures in adolescents; comparison of operative and conservative treatment (author's transl)].

Interposition of periosteum in joint fractures in adolescents may lead to delayed consolidation of the fracture or even malposition (varus-valgus deformities etc.) by early partial closure of the epiphyseal line. In severe cases, early arthrosis may be sequelar of inappropriate treatment of these patients. 14 cases of joint fractures with interposition of periosteum in adolescents are presented. The results of 5 operated cases are compared with 9 conservatively treated patients. The need for operative treatment is stressed.

Periosteum preservation facilitates osteogenesis after costochondral grafting in patients with hemifacial microsomia: a cohort study.

BACKGROUND: Costochondral graft (CCG) are the preferred treatment for Kaban-Pruzansky type IIB/III hemifacial microsomia (HFM) in children. However, debate persists regarding whether the periosteum and perichondrium should be preserved during surgery. This retrospective clinical study evaluated the role of periosteal and perichondral preservation in CCG transplantation. METHODS: Twenty-one consecutive patients with HFM who underwent CCG transplantation for mandibular ramus reconstruction were included. Based on whether the periosteum and perichondrium were preserved, patients were categorized

Entrapped periosteum in distal tibial physeal fracture: Is open reduction always necessary? - A case report.

INTRODUCTION: Distal tibial physeal fractures are common in pediatric trauma and are typically managed with closed reduction. However, in some cases, reduction failure is due to soft tissue interposition, particularly periosteum, which may necessitate open reduction. This case examines whether open reduction is indicated when confirming periosteal interposition. CASE PRESENTATION: We report the case of a 14-year-old male with a displaced Salter-Harris type II distal tibial fracture and concurrent fibular fracture sustained during sports activity. Closed reduction attempts failed, and MRI reve

Free gingival graft embossed over laterally flipped periosteum for root coverage: A novel case report.

Innovations in surgical techniques have improved the esthetic outcome and predictability of root coverage procedures in recent years. A free gingival graft (FGG) augments the attached gingiva, but the compromised blood supply precludes its use in root coverage. In the surgical technique described in this case report, the FGG kept over a laterally placed periosteal flap enhanced the outcome. A laterally flipped periosteal flap was adapted over the root surface using resorbable sutures. The free graft was secured at the recipient site with cyanoacrylate adhesive, and adaptation was ensured with

Coating cortical bone allografts with periosteum-mimetic scaffolds made of chitosan, trimethyl chitosan, and heparin.

Bone allografts have very limited healing leading to high rates of failure from non-union, fracture, and infection. The limited healing of bone allografts is due in large part to devitalization and removal of the periosteum, which removes osteogenic cells and osteoinductive signals. Here we report techniques for directly coating cortical bone with tissue scaffolds, and evaluate the scaffolds' capacity to support osteoprogenitor cells. Three types of coatings are investigated: N,N,N-trimethyl chitosan-heparin polyelectrolyte multilayers, freeze-dried porous chitosan foam coatings, and electrosp
