The histopathological classification, diagnosis and differential diagnosis of mucinous appendiceal neoplasms, appendiceal adenocarcinomas and pseudomyxoma peritonei.

The vermiform appendix is the primary site of several distinctive benign and malignant neoplasms. Some can produce the clinical syndrome of pseudomyxoma peritonei (PMP). A consensus on their terminology was reached by an international panel of pathologists and clinicians working under the auspices of the Peritoneal Surface Oncology Group International (PSOGI), and this review discusses the application of the PSOGI classification to routine reporting. We discuss diagnosis and differential diagnosis together with implications for patient management, covering low-grade appendiceal mucinous neopla

Grading and staging mucinous neoplasms of the appendix: a case series and review of the literature.

The grading and staging of appendiceal mucinous neoplasms is challenging and fraught with terminology problems, but has critical prognostic and therapeutic implications. We utilized a small case series to examine the grading and staging systems of appendiceal mucinous neoplasms and outline the evidence for the new systems proposed in the upcoming 8th edition of the American Joint Committee on Cancer (AJCC) Staging Manual. We reviewed 33 cases of appendiceal mucinous neoplasms with available clinical follow-up data, 6 of which were widely disseminated in the peritoneum. An additional 4 cases wi

Metastatic pineoblastoma via a ventriculoperitoneal shunt: CT demonstration.

We report the CT findings in a patient with pineoblastoma metastatic to the peritoneum via a VP shunt. A large, soft tissue tumor mass was revealed in the pelvis with associated peritoneal seeding and ascites. The initial intracranial tumor biopsy and later biopsy of metastatic peritoneal tumor demonstrated identical tissue diagnostic for pineoblastoma. Patients with intracranial malignant neoplasms and VP shunts will be followed during or after treatment with MRI brain scans. Periodic CT abdomen scans should be obtained to detect potential peritoneal metastases early to allow more effective t

Pathogenesis of histologic variations of appendiceal mucinous neoplasms.

BACKGROUND: In order for peritoneal metastases from a primary appendiceal mucinous neoplasm to occur, the wall of the appendix must perforate to allow mucus with tumor cells access to the peritoneal spaces. With progression the peritoneal metastases show a broad spectrum of tumor biology varying from indolent to aggressive activity. METHODS: The histopathology of peritoneal tumor masses was determined from the clinical material resected at the time of cytoreductive surgery (CRS). All groups of patients were treated by a uniform strategy that involved complete CRS and perioperative intraperito

High-volume mucinous ascites secondary to low-grade mucinous neoplasm arising in a mature cystic teratoma: a case report.

BACKGROUND: Low-grade mucinous neoplasms arising within mature ovarian cystic teratomas are rare and represent an uncommon ovarian source of pseudomyxoma peritonei (PMP). Because mucinous ascites and peritoneal disease are most often associated with appendiceal or other gastrointestinal primaries, ovarian-origin mucinous neoplasms may be underrecognized, contributing to diagnostic uncertainty and potential misclassification. CASE DESCRIPTION: We report the case of a 30-year-old nulliparous woman who presented with progressively worsening abdominal pain, distension, and respiratory discomfort.

Prophylactic hyperthermic intraperitoneal chemotherapy in patients with epithelial appendiceal neoplasms.

Background Prophylactic hyperthermic intraperitoneal chemotherapy (HIPEC) is a promising approach for preventing peritoneal carcinomatosis in high-risk patients. We report our initial experience with prophylactic HIPEC in a series of patients with appendiceal neoplasms. Methods We retrospectively reviewed our prospectively maintained database to identify patients who underwent HIPEC in the absence of peritoneal disease. Patients with previously documented peritoneal surface disease were excluded. Data regarding clinical, operative and pathological features were analysed. Results Out of 322 HIP

Mucinous Appendiceal Neoplasms and Pseudomyxoma Peritonei: Imaging Features and Current Therapies.

Pseudomyxoma peritonei involves the gradual accumulation of gelatinous peritoneal metastases, typically resulting from the rupture of a mucinous appendiceal neoplasm. Standard treatment often includes cytoreductive surgery and hyperthermic intraperitoneal chemotherapy. However, recurrence is common despite optimal cytoreduction, prompting ongoing investigation into novel therapies. This comprehensive review aims to enhance radiologists' understanding of mucinous appendiceal neoplasms and pseudomyxoma peritonei by focusing on nomenclature, pathophysiology, optimal imaging strategies, and the ty

Cytohistologic immunohistochemical correlation of epithelial tubo-ovarian neoplasms: Can cell blocks substitute for tissue?

BACKGROUND: Cytologic specimens often represent the initial diagnostic material for tubo-ovarian neoplasms resulting from therapeutic paracentesis for patients presenting with high-volume ascites. However, subtyping and immunohistochemical (IHC) characterization, which have implications in preoperative management and downstream ancillary testing, are not routinely performed in many institutions. This study aims to perform cytohistologic correlation of commonly used IHC stains to establish their reliability in peritoneal fluids/washing specimens. METHODS: A retrospective search of the laborato

Extensive peritoneal and omental lymphomatosis with raised CA 125 mimicking carcinomatosis: CT and intraoperative findings.

Diffuse peritoneal and omental seeding are well-known forms of dissemination of metastatic carcinoma. A wide variety of primary neoplasms may cause peritoneal and omental carcinomatosis, most commonly carcinomas of the ovary, gastrointestinal tract and breast. Extensive involvement of the peritoneal cavity with lymphoma is, however, rare. The association of peritoneal lymphoma with a raised CA 125, a tumour marker which is commonly raised in ovarian carcinoma, is a highly challenging clinical situation, which to our knowledge has not been published before in the medical literature. Not being a

[Is the localization of the tumor in intra-peritoneal part of the rectum contraindication to transanal endoscopic microsurgery].

Transanal endoscopic microsurgery (TEM) is the method used in cases of benign tumors at the early stages of rectal cancer. The tumor localization in peritonized part of the rectum indicates a limiting level for removal of the neoplasm. TEM was performed on 137 patients. The mean age was 63.8 ± 9.8 years and the number of women consisted of 65.7%. Neoplasms were located in the upper ampullar rectum and a potential possibility of connection with the peritoneal cavity was noted in 12 (8.7%) patients, but during TEM it was only in 5 cases. There wasn't any conversion to a peritoneal surgery. The w

Peritoneal Carcinomatosis and Its Mimics: Review of CT Findings for Differential Diagnosis.

Peritoneal carcinomatosis (PC) indicates the metastasis of a malignant neoplasm to the peritoneal surface. PC can be incidentally detected before discovery of the primary malignancy during an imaging study. There are other conditions that can mimic PC, such as pseudomyxoma peritonei, peritoneal lymphomatosis, peritoneal malignant mesothelioma, leiomyomatosis peritonealis disseminata, and tuberculous peritonitis. These diseases may appear similar on computed tomography (CT), but there are some clues for the differential diagnosis. This article will describe the CT findings of PC and its mimics

Trends in peritoneal surface malignancies: evidence from a Czech nationwide population-based study.

BACKGROUND: The aim of this study is to identify the incidence trends of primary and secondary peritoneal surface malignancies in a representative Czech population. METHODS: Data were obtained from patients registered in the Czech National Cancer Registry between 1979 and 2016. The incidence rates were analyzed between 2012 and 2016. To observe the incidence trends, we analyzed the data from two time periods, 1979-2005 and 2006-2016. The analyzed data included age, sex, and the histological types and primary origins of the malignancies. The Cochrane-Armitage test for linear trends was used fo

Staging of appendiceal mucinous neoplasms: challenges and recent updates.

Low-grade appendiceal mucinous neoplasms are unique tumors of the appendix, characterized by low-grade mucinous epithelium with villiform, undulating, or flat architecture. These tumors lack infiltrative growth or destructive invasion, but can extend into the appendiceal wall by a "pushing" pattern of invasion, with a broad front that can mimic a diverticulum. These neoplasms have a propensity for peritoneal dissemination, resulting in the clinical presentation of pseudomyxoma peritonei. The pathologic staging of these neoplasms is challenging and fraught with confusing terminology and numerou

Laparoscopy and mesothelioma.

Malignant mesothelioma is a well-recognized long-term sequela of chronic asbestos exposure. Asbestos use in the United States began in the 1950s and was widespread until the mid-1970s. Although currently only 2.2 cases per million population per year are diagnosed, disease incidence is increasing because of the long latency of this neoplasm. A latency of 15-50 years means that a higher incidence of this neoplasm can be anticipated in the future. The authors report a patient with peritoneal mesothelioma and no known prior exposure to asbestos. The diagnosis was confirmed by exploratory laparosc

Mucinous appendiceal neoplasms: pathologic classification, clinical implications, imaging spectrum and mimics.

Mucinous appendiceal neoplasms are uncommon, but important to recognize with imaging due to malignant potential. Peritoneal seeding and pseudomyxoma peritonei can occur with both frankly malignant as well as low-grade appendiceal neoplasms. Prospective imaging identification of potential appendiceal neoplasm is paramount to clinical/surgical management. When a mucinous appendiceal neoplasm is suspected, a right hemicolectomy with lymph node dissection is the preferred surgical management. Unfortunately, accurate preoperative diagnosis can be challenging due to a wide range of clinical presenta

[Plasma-cell granuloma of the sigmoid colon concomitant with adenocarcinoma of the cecum. Viewpoint for debate, literature review on pseudotumors, idiopathic colitis and cancer].

A case of colonic pseudotumor, causing intestinal occlusion, concomitant with a caecum neoplasia is reported. A male 69 years old was referred to our Institution for colic abdominal pain: colonoscopy aborted because of an insuperable sigma stenosis; diagnostic enema confirmed the sigmoid stenosis, originally advised as neoplastic. As operation, the sigmoid tumor appeared accompanied with enlarged draining lymph nodes and peritoneal sac was disseminated of miliary-like whity granules. Hystological examination showed a plasma-cell granuloma with nodular and peritoneal chronic inflammatory reacti

Delivery of an Incidental Appendiceal Mucinous Neoplasm.

Low-grade appendiceal mucinous neoplasms (LAMNs) are rare and non-invasive tumors of the appendix, and their unexpected discovery during surgery can pose challenges to management. To date, only two cases pertaining to LAMNs without peritoneal spread in pregnancy exist in the literature. Here, we present a literature review of appendiceal mucinous neoplasms and discuss our management and operative approach to a large, incidental appendiceal mucinous neoplasm discovered during an emergency cesarean section of a 38-year-old female.

Peritoneal Malignancy in Children: A Pictorial Review.

Peritoneal malignancies are a rare occurrence in children, often metastatic and rarely originating primarily in the peritoneum. The imaging findings of these entities in the pediatric age have not been recently reviewed or they have been previously described or depicted mostly as part of articles discussing each entity separately. This is a review of the most relevant peritoneal malignancies in childhood emphasizing imaging features. Knowledge of these appearances may facilitate diagnosis and staging of these neoplasms.

Selected topics in peritoneal pathology.

This essay considers selected peritoneal lesions many of which were the subject of studies coauthored by Dr Robert E. Scully. His article on multilocular peritoneal inclusion cysts has largely led to these lesions being considered non-neoplastic, eschewing the term cystic mesothelioma. These cysts are often associated with reactive mural mesothelial proliferations that can potentially lead to a misdiagnosis of mesothelioma. Clinical findings, such as a common association with endometriosis or prior operations, can prompt consideration of a reactive lesion. Mesothelial hyperplasia may be diffic
