Diagnostic criteria for persistent postural-perceptual dizziness (PPPD): Consensus document of the committee for the Classification of Vestibular Disorders of the Bárány Society.

This paper presents diagnostic criteria for persistent postural-perceptual dizziness (PPPD) to be included in the International Classification of Vestibular Disorders (ICVD). The term PPPD is new, but the disorder is not. Its diagnostic criteria were derived by expert consensus from an exhaustive review of 30 years of research on phobic postural vertigo, space-motion discomfort, visual vertigo, and chronic subjective dizziness. PPPD manifests with one or more symptoms of dizziness, unsteadiness, or non-spinning vertigo that are present on most days for three months or more and are exacerbated

Functional brain activity in persistent postural-perceptual dizziness (PPPD) during galvanic vestibular stimulation reveals sensitization in the multisensory vestibular cortical network.

Persistent postural-perceptual dizziness (PPPD) is often preceded by vestibular disorders. We applied galvanic vestibular stimulation (GVS) and related stimulus-evoked activity to individual ratings of perceived motion for each stimulus and to perceived egomotion thresholds by GVS and behavioural parameters outside the scanner: levels of functional disability by standardized questionnaires, visual motion coherence, passive egomotion perception by chair rotation and quantitative postural stability. We hypothesized that the preceding vestibular disorder predisposes to abnormal brain excitability

Effectiveness of emotional freedom techniques in patients with persistent postural-perceptual dizziness: study protocol for a multicenter, pragmatic, pilot randomized controlled trial.

BACKGROUND: Persistent postural-perceptual dizziness (PPPD) is a chronic vestibular disorder with psychological components. Cognitive behavioral therapy (CBT) is commonly used to manage its emotional aspects; however, emotional freedom techniques (EFT)-which combine cognitive reframing with acupoint tapping-have shown efficacy in various psychiatric conditions. Despite its potential, EFT has not been evaluated for PPPD. This study aims to address this gap through a pragmatic clinical trial. METHODS: Thirty patients aged 19-65 years diagnosed with PPPD will be recruited from multiple centers a

It's All in Your Head: A Case Report of Persistent Postural-Perceptual Dizziness and Chronic Anxiety Treated Simultaneously With Osteopathic Cranial Manipulative Medicine.

Persistent postural-perceptual dizziness is a chronic functional vestibular disorder characterized by persistent dizziness, imbalance, motion sensitivity, and significant psychological distress, frequently including anxiety. Symptoms often persist despite normal imaging and multidisciplinary evaluation, contributing to diagnostic uncertainty and misattribution to psychiatric causes. Osteopathic cranial manipulative medicine is not routinely incorporated into the management of persistent postural-perceptual dizziness despite osteopathic principles, emphasizing the interrelationship between cran

[Chronic vestibular syndromes: Correct diagnosis and treatment].

Chronic dizziness is defined as dizziness or vertigo symptoms that are continuously present for more than 3 months. Typical complaints include persistent to-and-fro vertigo accompanied by postural imbalance and gait unsteadiness with an increased risk of falling. Patients often experience inadequate or inappropriate medical care despite being significantly impaired in their daily lives. Common vestibular causes include persistent peripheral vestibular disorders (such as bilateral vestibulopathy or incompletely compensated unilateral vestibulopathy), central vestibular syndromes (e. g., downbea

Etiologic distribution of dizziness and vertigo in a referral-based dizziness clinic in South Korea.

This study aimed to determine the etiologic distribution of dizziness and vertigo in a referral-based dizziness clinic in South Korea. We analyzed the diagnoses of 21,166 consecutive dizzy patients (12,691 women, mean age = 57.9 ± 15.7, age range = 3-97) seen from 2003 to 2019 using a registry and medical records. Overall, dizziness and vertigo were more common in women (60.0%, CI 0.59-0.61) than in men without a difference in age (57.7 ± 15.5 vs. 58.1 ± 16.1, p = 0.094). Benign paroxysmal positional vertigo (BPPV, 24.1%) was the most common cause of dizziness/vertigo, followed by psychiatric

Data-driven vestibular fMRI in PPPD and residual dizziness after BPPV: a narrative methodological review of AI-based phenotyping and longitudinal modeling.

Neuroimaging studies of vertigo and chronic dizziness have identified abnormalities across vestibular, visual, somatosensory, and cognitive-emotional networks, but conventional group-level analyses are limited in capturing individual heterogeneity and longitudinal change. This narrative methodological review integrates current evidence and proposes a framework combining functional magnetic resonance imaging (fMRI), artificial intelligence (AI), and advanced statistical modeling, anchored in two priority research scenarios: PPPD phenotyping and longitudinal characterization of residual dizzines

Management of Vestibular Symptoms in Sports-Related Concussion.

Vestibular symptoms are common after sports-related concussion and predict slower recovery and delayed return to play. This review provides a practical, clinical profile-guided approach for clinicians. Early screening focuses on Vestibular/Ocular Motor Screen, timed tandem gait, and positional testing for benign paroxysmal positional vertigo. A comprehensive neurovestibular examination and history help distinguish dizziness symptom profiles. Early vestibular rehabilitation, combined with cervical management and graded aerobic exercise, speeds recovery. Medications are adjuncts, particularly fo

Current and Emerging Medical Therapies for Dizziness.

Medical therapies for dizziness are aimed at vertigo reduction, secondary symptom management, or the root cause of the pathologic process. Acute peripheral vertigo pharmacotherapies include antihistamines, calcium channel blockers, and benzodiazepines. Prophylactic pharmacotherapies vary between causes. For Meniere disease, betahistine and diuretics remain initial first-line oral options, whereas intratympanic steroids and intratympanic gentamicin are reserved for uncontrolled symptoms. For cerebellar dizziness and oculomotor disorders, 4-aminopyridine may provide benefit. For vestibular migra
