A Unified Classification of Middle Ear and Petrous Bone Congenital Cholesteatomas.

BACKGROUND: The aim of this study was to classify congenital cholesteatoma along an entire spectrum of involvement ranging from the middle ear to petrous apex. METHODS: A total of 131 patients (85 adults and 46 children) underwent operations for congenital cholesteatoma over the duration of 27 years. RESULTS: For most cases, middle ear mucosa was normal, the first ossicle eroded by the mass was the stapes, and the mastoid air cell system was well-pneumatized on intraoperative and radiographic views. Totally 34% of patients presented with facial nerve weakness and 45% of these cholesteatomas

Solitary fibrous tumor of the petrous bone: a successful treatment option.

The patient was a 48-year-old man complaining about right hemicranial pain and deafness of 1 year's duration. MRI showed a solid well-vascularized mass destroying the mastoid and petrous bone, and with an epidural component that pulled the right cerebellar hemisphere. The patient underwent a right modified type A infratemporal approach, which allowed complete resection of the tumor, with a low morbidity. The pathological diagnosis was solitary fibrous tumor. The patient received complementary treatment with radiotherapy. At 24 months after the initial treatment the patient is free of disease.

[Computerized tomography in children and adolescents with suspected petrous bone abnormality].

PURPOSE: To demonstrate HRCT findings and their therapeutic relevance in suspected congenital hearing disorders. MATERIAL AND METHODS: It was checked in 96 young patients if HRCT findings of the temporal bone could explain functional findings. Furthermore, the therapeutic consequences were noted. RESULTS: Normal CT and normal functional findings were obtained in 49 temporal bones (TB). In conductive hearing loss (41 TB), dysplasias of the conducting apparatus (37 TB) and inflammatory changes (3 TB) were found. Combined hearing loss (18 TB) was clarified completely or partially in half the ca

Management of petrous bone fractures in children: analysis of 127 cases.

Petrous bone fractures (PBF) in children are relatively frequent. They are mostly diagnosed after collisions and falls. The complications typically associated with PBF were different types of hearing disorders in 69.1% of the patients who had audiometry, liquorrhea in 16.5%, palsy of cranial nerves in 10.8% (facial nerve palsy in 9.4%), bacterial meningitis, stenosis of the external ear canal, and posttraumatic cholesteatoma in 0.7% of the fractures each. Most complications were transient; 8.6% of the patients underwent surgery because of PBF-related complications and 9.4% suffered from severe

[Radiation burden in high-resolution computerized tomography of the petrous bone].

Exposure to irradiation and radiation stress of the eye lens, thyroid and skin in high resolution computed tomography of the petrous bone are demonstrated by means of measurements conducted by the authors, and compared with results published to date. The radiation stress is less than the doses measured during conventional polytomography of the petrous bone. The exposure of the crystalline lens, in particular, depends decisively on the angle of the gantry or on the position at which CT is performed. The coronary plane is the projection of choice in respect of minimum irradiation exposure. The s

Primary aneurysmal bone cyst of the petrous temporal bone: A case report and review of literature.

Aneurysmal bone cyst (ABC) arising in the petrous portion of the temporal bone is a rare entity with only five such reported cases in the literature. We report the case of a 28-year-old man who presented with a tender swelling in the right preauricular region with right ear discharge and conductive hearing loss of 4 years' duration. Computed tomography and Magnetic Resonance imaging showed a destructive lesion in the right petrous bone with cavitation consistent with the diagnosis of ABC. Gross total resection of the lesion was achieved and diagnosis was confirmed histologically. The patient h

[Bone drilling in microvascular decompression for trigeminal neuralgia: high morphological variety of the petrous bone].

Microvascular decompression is now a standard surgical technique for the treatment of trigeminal neuralgia. However, it is occasionally difficult to expose the trigeminal nerves because of the high anatomical variety of vascular or bony structures in the posterior fossa. We reported the case of a 59-year-old woman with trigeminal neuralgia whose site of neurovascular compression could not be observed in microvascular decompression. On approaching the trigeminal nerve, the suprameatal tubercle was so prominent that it prevented adequate visualization of the nerve tract. After drilling out the t

[Epidermoid tumor originating from the petrous bone: report of two cases].

In case 1, the tumor was incidentally found in the right petrous bone, middle cranial fossa and cerebello-pontine angle. T1 weighted MRI demonstrated a low intense mass and T2 weighted imaging demonstrated hyperintensity. Through the subtemporal extradural approach, an epidermoid in the middle cranial fossa was partially removed. Postoperative course was uneventfull but an episode of rhinorrhea occurred 15 months later. Bone-window CT scan disclosed air cells of the petrous bone were exposed to the previous surgical cavity. Using the same approach, an epidermoid was totally removed. With the s

Modern imaging of petrous bone malformations: improvement for clinical-embryological correlations.

The rapid perfecting of modern diagnostic imaging during the past decade has provided several new ways to explore ear malformations. Helical computed tomography, three-dimensional (3D) reformations, virtual endoscopy and thin-sliced 3D magnetic resonance imaging constitute the most important recent progress. In parallel, conventional morphologic studies and modern teratologic experimentation give rise to new concepts relative to ear development. The molecular and genetic control of ear morphogenesis is becoming better known, and several malformations can be considered as a consequence of a dis

Management of Intractable Petrous Bone Cholesteatoma With a Combined Translabyrinthine-Transsphenoidal Approach.

OBJECTIVE: To evaluate whether a combined translabyrinthine-transsphenoidal approach can be used to achieve adequate surgical resection of an extensive petrous bone cholesteatoma and create a debris drainage route for the residual cholesteatoma that is maintained long-term. PATIENT: A 71-year-old man with residual petrous temporal bone cholesteatoma that had spread extensively to the internal carotid artery and posterior cranial fossa. INTERVENTION: Surgical resection of the cholesteatoma via a translabyrinthine approach and creation of a debris drainage route into the nasopharynx via a tran

[A rare pathology of the petrous part of the temporal bone: adenoma of the middle ear].

Adenomas are very rare tumors of the middle ear. They are benign neoplasms originating from the glandular components of the mucous membrane of the middle ear. The middle ear adenoma was first described by Hyams and Michaels in 1976, which was named an adenomatous tumor. This article reports the case of a 50-year-old female patient, who presented with recurrent right-sided dull otalgia and pulse synchronous tinnitis, which began 1 year prior to presentation, with the suspected diagnosis of a glomus tympanicum tumor. Following the otorhinolaryngological examination and imaging an unclear mesotym

[Carcinoma of the nasopharynx, radiotherapy, otitis, labyrinthitis, inflammation of the petrous bone, meningitis].

Endocranial complications of otitis show new etiopathogenic possibilities. We have accomplished an analysis of this issue after one of these cases. A female patient with a nasopharyngeal carcinoma developed an otogenous meningitis. Radiotherapeutic management was followed by osteoradionecrosis of cranial basis, local hypoimmunity favorized the reported course: middle otitis, labyrinthitis, petrositis and meningitis. Comments on pathogenetic, clinic and therapeutical peculiarities of this case.

A petrous bone destructive acoustic neurinoma: a tumor of far-lateral origin?

We report a case of an acoustic neurinoma that recurred more than 11 years after "total" tumor removal through the posterior cranial fossa. Recurrent tumor had obliterated the petrous bone, filled the middle ear, and was visible grossly at the external auditory meatus. The unique behavior of this tumor implied that the site of origin was within the labyrinth, beyond the fundus of the internal auditory canal and far lateral to the usual site of acoustic neurinoma origin. We offer an hypothesis as to why these tumors occasionally recur, even after clinical "total" removal.

Pneumatization and thickness of the petrous bone in patients with Meniere's disease. A histopathological study.

Histological evaluations were made of the degree of pneumatization and thickness of the petrous bone surrounding the vestibular aqueduct, and of the relationships of these parameters to the condition of the vestibular aqueduct in 27 temporal bones with idiopathic endolymphatic hydrops from individuals with Meniere's disease. The results were compared with those from the same parameters in 79 control temporal bones without endolymphatic hydrops from individuals who had no premortem history of otologic disease. Pneumatization of the temporal bones of patients with Meniere's disease was in most c

Long-term facial nerve outcome in surgically treated petrous bone cholesteatoma patients.

INTRODUCTION: The term petrous bone cholesteatoma (PBC) represents a slow-growing epidermal lesion arising from the petrous part of the temporal bone. It is a rare incidence accounting for only 4-9% of all petrous bone lesions. PBC represents a real surgical challenge due to its complex relationship with critical neurovascular structures. OBJECTIVE: To demonstrate our experience in using various options of facial nerve (FN) management during surgical treatment of PBC and analyze the long-term facial function outcomes. STUDY DESIGN: Retrospective medical record review in a quaternary skull-ba
