Metabolism, Not Mendacity: Rethinking Prolonged Cocaine Positivity.

BACKGROUND: Urine toxicology is a cornerstone of monitoring abstinence in substance use disorder treatment, yet commonly cited detection windows are based on studies in healthy volunteers and do not account for metabolic variability. Prolonged metabolite positivity is typically interpreted as continued use, which can jeopardize treatment engagement and erode the therapeutic alliance. CASE SUMMARY: We describe a 42-year-old man with severe stimulant use disorder whose urine toxicology remained positive for cocaine metabolites for 18 days, including 12 days after verified abstinence in a reside

Merging pharmacometabolomics with pharmacogenomics using '1000 Genomes' single-nucleotide polymorphism imputation: selective serotonin reuptake inhibitor response pharmacogenomics.

OBJECTIVE: We set out to test the hypothesis that pharmacometabolomic data could be efficiently merged with pharmacogenomic data by single-nucleotide polymorphism (SNP) imputation of metabolomic-derived pathway data on a 'scaffolding' of genome-wide association (GWAS) SNP data to broaden and accelerate 'pharmacometabolomics-informed pharmacogenomic' studies by eliminating the need for initial genotyping and by making broader SNP association testing possible. METHODS: We previously genotyped 131 tag SNPs for six genes encoding enzymes in the glycine synthesis and degradation pathway using DNA

Comparison of pharmacogenomic information for drug approvals provided by the national regulatory agencies in Korea, Europe, Japan, and the United States.

Pharmacogenomics, which is defined as the study of changes in the properties of DNA and RNA associated with drug response, enables the prediction of the efficacy and adverse effects of drugs based on patients' specific genetic mutations. For the safe and effective use of drugs, it is important that pharmacogenomic information is easily accessible to clinical experts and patients. Therefore, we examined the pharmacogenomic information provided on drug labels in Korea, Europe, Japan, and the United States (US). The selection of drugs that include pharmacogenomic information was based on the drug

Clinical implementation of preemptive pharmacogenomics in psychiatry: Τhe "PREPARE" study.

PREemptive Pharmacogenomic testing for Preventing Adverse drug REactions (PREPARE) is the first prospective, pre-emptive pharmacogenomic study conducted in Europe, within the frame of the Horizon 2020 program. It aims to determine whether implementing pre-emptive pharmacogenomics (PGx) testing of clinically relevant biomarkers, so as the dose and drug selection to be guided, will result in an overall reduction of both the occurrence and the severity of drug-genotype-associated adverse drug reactions (ADRs). To achieve that, two groups of patients will be recruited; one that will receive treatm

A review of the existing pharmacogenomic literature of naltrexone for use in alcohol use disorder.

Naltrexone is an opioid receptor antagonist used to treat alcohol use disorder; however, like other commonly used addiction treatment options, it demonstrates inconsistency in treatment response. Pharmacogenomics may be used to individualize addiction treatment and identify patients who would be better candidates for this drug. Recently, the Clinical Pharmacogenomics Implementation Consortium (CPIC) provided guidelines on pharmacogenomic testing for naltrexone. Pharmacogenomic testing targets for naltrexone include pharmacodynamic genes, like the mu-opioid receptor gene OPRM1, as well as pharm

The Role of Pharmacogenomics in Opioid Prescribing.

Pharmacogenomics is increasingly important to guide objective, safe, and effective individualised prescribing. Personalised prescribing has revolutionised treatments in the past decade, allowing clinicians to maximise drug efficacy and minimise adverse effects based on a person's genetic profile. Opioids, the gold standard for cancer pain relief, are among the commonest medications prescribed in palliative care practice. This narrative review examines the literature surrounding opioid pharmacogenomics and its applicability to the palliative care cancer population. There is currently limited in

[Pharmacogenomics in the treatment of mental disorders].

Pharmacogenomics lies at the intersection of pharmacology and genomics. It analyzes the impact of genetic variants on patient’s response to a specific drug. Pharmacogenomic tests involve the analysis of variants of genes responsible for drug metabolism, to determine the rate of drug metabolism and apply this information to clinical practice. Pharmacogenomics assists in selecting the appropriate drug and adjusting its dose to minimize side effects while maximizing the effectiveness of a drug, which accelerates the introduction of effective therapy. The prospects for its application in psychiatr

The Australian Public's Awareness, Knowledge and Perspectives Towards Pharmacogenomics: A Nationwide Survey, and Semi-Structured Interviews.

Pharmacogenomics has the potential to improve medication responses for the public; however, its adoption in clinical practice has been slow. Consumer representation and engagement are crucial for pharmacogenomics implementation, since it is their data that is central to testing. This study investigated the Australian public's awareness, knowledge, and perceptions of pharmacogenomics via a mixed-methods approach. Firstly, a cross-sectional survey with a nationally representative sample of 772 members of the Australian general public assessed the frequency of pharmacogenomics awareness, knowledg

Pharmacogenomic and drug interactions risk in cardio-oncology: A precision medicine perspective for India.

Cardio-oncology patients may face complex treatment regimens due to the concurrent existence of cancer and cardiovascular disease, leading to a considerable polypharmacy burden. This significantly increases the prospect of drug-drug interactions (DDIs) and gene-drug interactions. The majority of these interactions arise from comparable pharmacokinetic and pharmacological pathways associated with drug transporters and cytochrome P450 enzymes. The significance of pharmacogenomics in tailored treatment strategies are emphasised by the fact that genetic variability enhances individual differences

Pharmacogenomic Considerations for Anticoagulant Prescription in Patients with Hereditary Haemorrhagic Telangiectasia.

Hereditary haemorrhagic telangiectasia (HHT) is a vascular dysplasia that commonly results in bleeding but with frequent indications for therapeutic anticoagulation. Our aims were to advance the understanding of drug-specific intolerance and evaluate if there was an indication for pharmacogenomic testing. Genes encoding proteins involved in the absorption, distribution, metabolism, and excretion of warfarin, heparin, and direct oral anticoagulants (DOACs) apixaban, rivaroxaban, edoxaban, and dabigatran were identified and examined. Linkage disequilibrium with HHT genes was excluded, before var
