Oligometastatic renal cell carcinoma in the palatine tonsil: successful resection using trans-oral robotic surgery.

A man in his mid 70s was referred to head and neck outpatients with bulky tissue in the left tonsillar fossa. He had previously been treated for oligometastatic renal clear cell carcinoma (diagnosed over 15 years prior to disease recurrence) by tonsillectomy and adjuvant radiotherapy (years from primary treatment), followed by trans-oral laser surgery to his oropharyngeal recurrence 3 years later. Examination under anaesthetic and biopsy confirmed further recurrence of disease in the left tonsillar fossa, with parapharyngeal extension, which has not been previously reported in the literature.

Invasive Mucormycosis Involving the Laryngopharynx After Bone Marrow Transplantation.

Invasive larynx or pharyngeal fungal infections are rare. We report an invasive fungal laryngopharyngitis patient who complained of a persistent sore throat after an allogenic bone marrow transplant and a haploidentical stem cell transplant. An antifungal gargle was used after finding necrotic changes extending from the right soft palate to the aryepiglottic fold. Biopsy and culture suggested a fungal infection with suspicious mucormycosis. imaging showed the right oropharynx, supraglottis, and the para- pharnygeal space were involved. After initiating liposomal amphotericin В for 4 days, wide

Management of oral feeding following total laryngectomy around the world: YO-IFOS international study.

BACKGROUND: To analyze worldwide practices regarding the initiation of oral feeding after total laryngectomy (TL). METHODS: Online survey. RESULTS: Among the 332 responses received, 278 from 59 countries were analyzed. Our results showed that 45.6% of respondents started water and 45.1% started liquid diet between postoperative days 7 and 10. Semi-solid feeds were initiated between days 10 and 14 for 44.9% of respondents and a free diet was allowed after day 15 for 60.8% of respondents. This timing was significantly delayed in cases of laryngo-pharyngectomy and after prior radiotherapy (p <

[Reconstruction after extended partial laryngeal and hypopharyngeal resections. A new method with rib cartilage, chondrosynthesis and free radial transplants].

UNLABELLED: Partial pharyngectomy combined with subtotal laryngectomy results in permanent tracheostomy and long lasting swallowing difficulties in many cases. Based on our prior experimental and clinical studies on laryngeal chondrosynthesis the objectives of this project were to develop a method for reconstruction of up to two thirds of the larynx and one pyriform sinus. METHOD: The resected laryngeal framework is reconstructed with autogenous rib cartilage and stabilized with plates and screws. In addition a piece of cartilage is used as a buttress for apposition by the remaining mobile vo

Evaluation of total pharyngolaryngectomy for hypopharyngeal cancer.

In 37 patients with hypopharyngeal cancer treated by total pharyngolaryngectomy and radical neck dissection, the 5 year cure rate was 30%. Among these patients, T-1 and T-2 tumors had a 57% and 47% 5 year cure rate respectively. Better results were obtained in those patients with tumor of the posterior pharyngeal wall and tumor of sinus pyriformis extending toward postcricoid region or toward the larynx. In 14 patients treated with various approaches of partial pharyngolaryngectomy, the 5 year cure was 53%. It is concluded that the conservation surgery is the operation of choice for the tumors

Transoral palate-sparing nasopharyngectomy with the Flex® System: preclinical study.

OBJECTIVES/HYPOTHESIS: The nasopharynx remains one of the most challenging areas to surgically access given its deep location, narrow confines, and surrounding critical neurovascular anatomy. Various open and endoscopic approaches have been developed, all of which have been limited to some degree in their ability to bimanually instrument and visualize in this area. STUDY DESIGN: Cadaveric dissection. METHODS: Study using one fresh human cadaver. A novel, highly articulated endoscope with flexible instrumentation, the Flex System, was placed in the oral cavity and advanced around the soft pal

[Enlarged supraglottic laryngectomy (author's transl)].

Malignant tumour of the upper hypopharynx is the most often the continuation of the supraglottic cancer of the larynx or it develops from the beginning between the lingual site of the epiglottis and the base of the tongue. The enlarged supraglottic laryngectomy has the best results. The large defect between the partial resected base of the tongue and the border of the resected laryngeal cartilage was covered by the fascia of the sternohyoid muscle. From 22 cases operated from 1979 on this way one local relapse was found.

Tracheoesophageal Puncture Outcomes at a Safety Net Hospital.

Tracheoesophageal puncture (TEP) is the gold standard for voice rehabilitation after total laryngectomy (TL). Retrospective analysis was performed of TEP outcomes in patients between 2013 and 2020 at a single tertiary hospital. TEP was performed primarily in 79%, secondarily in 6%, and not placed in 15% of 226 patients. Within the study population, 53.4% utilized their TEP, including 52% of primary and 71.4% of secondary TEPs. TEP-related complication occurred in 50.8%, including 50.8% of primary and 50% of secondary TEPs. Secondary TEP was less likely in Black versus White patients, and more

Risk factors for pharyngocutaneous fistula formation: a study focused on pharyngeal reconstruction technique.

OBJECTIVE: This study investigated the risk factors for developing pharyngocutaneous fistula, the most common complication following total laryngectomy. METHODS: We included all patients who underwent total laryngectomy and bilateral neck dissection from 2009 to 2021. Patients excluded were those with hypopharyngeal involvement, total or partial pharyngectomy, base of the tongue resection, large pharyngeal defects requiring free/pedicle flap reconstruction, or salvage laryngectomy. RESULTS: A total of 164 patients participated in the study. Multivariate regression analysis identified two ind

Primary mucosal melanoma of the palatine tonsil: Report of a case and review of the literature.

BACKGROUND: Mucosal melanoma of the palatine tonsil is extremely rare. Due to its poor prognosis, primary tonsillar melanoma requires prompt recognition and treatment. METHODS: A 62-year-old female presented with a deeply pigmented and exophytic lesion in the left tonsillar fossa. The patient underwent a partial pharyngectomy through a midline labio-mandibulotomy approach along with a left level I-V neck dissection. Reconstruction with a left radial forearm free flap and a pharyngeal constrictor advancement pharyngoplasty was performed. RESULTS: The patient remains free of disease at eight m

Digestive Reconstruction After Pharyngolaryngectomy with Total Esophagectomy.

BACKGROUND: Pharyngolaryngectomy with total esophagectomy (PLTE) is often indicated for patients with synchronous head and neck cancer and thoracic esophageal cancer or those with head and neck cancer extending to the upper mediastinum. A long conduit is required for the reconstruction, and the blood flow at the tip of the conduit is not always sufficient. Thus, reconstructive surgery after PLTE remains challenging, and optimal reconstruction methods have not been elucidated to date. METHODS: This analysis investigated 65 patients who underwent PLTE. The short-term outcomes among the procedur

[Partial surgery in cancer of the upper respiratory and digestive tracts].

Partial or functional surgery for cancers of the upper aerodigestive tract essentially concerns the larynx and the pharyngolaryngeal area. The aim of this surgery is to preserve voice and deglutition. Therefore fundamental anatomical structures such as the cricoid cartilage or at least one arytenoid cartilage must be spared, pointing the limitations of the techniques. Based on these principles, many authors have divided partial surgical procedures ranging from the simplest (cordectomy) to reconstruction on near-total laryngectomy. Cancers of the hypopharynx can benefit from similar techniques

Synovial sarcoma of the hypopharynx in an 11-year-old boy: treatment by partial supraglottic laryngopharyngectomy.

Synovial sarcoma in an extra-articular location is a very rare tumor, particularly in relation to childhood tumors. We report our experiences with an 11-year-old boy who suffered from swallowing difficulties caused by a polypoid tumor of the hypopharynx. Biopsy demonstrated a fusocellular carcinoma, while the epithelial components showed cytokeratin and neuron-specific enolase positivity. A correct final diagnosis could only be established by the histology of the surgical preparation following partial laryngopharyngectomy. The presence of spindle cells associated with glandular-like ones prove

Hypocalcaemia in pharyngolaryngectomy: Preservation or autotransplantation of parathyroid glands.

OBJECTIVE: To describe transient and permanent hypocalcaemia following partial and total pharyngolaryngectomy with parathyroid gland preservation or autotransplantation. METHODS: Thirty patients underwent partial or total pharyngolaryngectomy by a single surgeon during the period 2009-2020. Intraoperative parathyroid gland preservation or autotransplantation (where the gland appeared devascularized) was routinely performed. Calcium levels performed on day 1, 3 months, and at 12 months postoperatively were collected. Rates of transient and permanent hypocalcaemia were calculated. RESULTS: A t
