The forensic and legal implications of water, wet, or fry.

Embalming fluid applied to marijuana cigars or cigarettes, with or without the addition of phencyclidine (PCP), has several names, such as water, wet, illy, and fry. Individuals who commit crimes under the influence of this substance are often violent and may appear psychotic, with symptoms resembling schizophrenia or delirium. Currently, there are no case reports or case law involving the use of this substance. Wet may impair one's competency to stand trial (CST). The authors present a composite case of a man who abused wet for three years and had been sober for three months when he presented

False-positive interferences of common urine drug screen immunoassays: a review.

Urine drug screen (UDS) immunoassays are a quick and inexpensive method for determining the presence of drugs of abuse. Many cross-reactivities exist with other analytes, potentially causing a false-positive result in an initial drug screen. Knowledge of these potential interferents is important in determining a course of action for patient care. We present an inclusive review of analytes causing false-positive interferences with drugs-of-abuse UDS immunoassays, which covers the literature from the year 2000 to present. English language articles were searched via the SciFinder platform with th

Substance abuse and movement disorders.

An array of movement disorders is associated with ethanol, illicit drugs, and tobacco. Heavy ethanol users experience withdrawal tremor and, less often, withdrawal parkinsonism, chorea, and myoclonus. Asterixis is a feature of hepatic failure. On the other hand, ethanol can ameliorate essential tremor and myoclonus-dystonia. Among opioid drugs, meperidine can precipitate myoclonus. Severe parkinsonism affected users of a synthetic meperidine analog contaminated with 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine. Spongiform leukoencephalopathy, sometimes with chorea and myoclonus, occurred in in

Effects of drugs of abuse on response accuracy and bias under a delayed matching-to-sample procedure in squirrel monkeys.

The effects on memory of drugs of abuse from several pharmacological classes were examined in four adult male squirrel monkeys responding under a delayed matching-to-sample schedule of food presentation. Subjects were required to emit 20 responses on a sample key transilluminated by either a constant white or a flashing blue light. The twentieth response initiated a 3-second delay followed by presentation of two comparison stimuli. If a response was made to the key that matched the sample stimulus (correct match), a single food pellet (97 mg) was delivered. Pentobarbital (0.32-10 mg/kg), diaze

Altered mental status and end organ damage associated with the use of gacyclidine: a case series.

INTRODUCTION: Over the past decade, there has been a sharp increase in the number of newly identified synthetic drugs. These new drugs are often derivatives of previously abused substances but have unpredictable toxicity. One of these drugs is gacyclidine, a derivative of phencyclidine (PCP). Gacyclidine has been studied as a neuroprotective agent in trauma and as a therapy of soman toxicity. There are no previous reports of its use as a drug of abuse. CASE REPORTS: During a two-month period in the summer of 2013, a series of patients with severe agitation and end-organ injury were identified

Late-stage borreliosis and substance abuse.

BACKGROUND: Infectious diseases can contribute to substance abuse. Here, a fatal case of borreliosis and substance abuse is reported. This patient had a history of multiple tick bites and increasing multisystem symptoms, yet diagnosis and treatment were delayed. He experimented with multiple substances including phencyclidine (PCP), an N-methyl-d-aspartate (NMDA) receptor antagonist that opposes NMDA agonism caused by Borrelia infection. During PCP withdrawal, he committed one homicide, two assaults, and suicide. METHODS: Brain tissue was obtained from autopsy and stained for microglial activ

Toxic agents causing cerebellar ataxias.

The cerebellum is particularly vulnerable to intoxication and poisoning, especially so the cerebellar cortex and Purkinje neurons. In humans, the most common cause of a toxic lesion to the cerebellar circuitry is alcohol related, but the cerebellum is also a main target of drug exposure (such as anticonvulsants, antineoplastics, lithium salts, calcineurin inhibitors), drug abuse and addiction (such as cocaine, heroin, phencyclidine), and environmental toxins (such as mercury, lead, manganese, toluene/benzene derivatives). Although data for the prevalence and incidence of cerebellar lesions rel

Issues of risk assessment: lessons from the use and abuse of drugs during pregnancy.

Therapeutic uses and risks associated with the opioids, cannabis, cocaine, and phencyclidine are discussed as well as limitations of the developmental animal and clinical pediatric literature, especially with respect to problems of quantitative risk assessment. Human and animal developmental findings for methadone and cannabis are compared with respect to long-term behavioral effects with special emphasis on pharmacological and interpretive issues. It is suggested that neonatal withdrawal is the most serious neurobehavioral sequela associated with maternal use of opioid and other sedative-hypn

Evaluation of the Syva ETS Plus urine drug and serum ethanol analyzer.

The Syva ETS is an automated system designed for analysis of drugs of abuse in urine. Recently, the Syva Company has upgraded this system to the ETS Plus, which is capable of analyzing ethanol in urine, serum, or plasma. We evaluated the new ETS Plus for urine screening of barbiturates, benzodiazepines, cocaine metabolite, opiates, and phencyclidine. Results of 505 patient sample assays obtained by ETS Plus were compared with those of ETS. There were only four discrepant results which had absorbance rates close to the low calibrator cutoff values. The within-run precision of the ETS Plus Ethyl

Specific drug use and violence in delinquent boys.

Delinquent boys who had been committed to a training school were interviewed by a psychiatrist to determine drug abuse status. Crimes that were officially recorded were classified as person offenses or property offenses, and the number of offenses was determined from the juvenile record. Drug scores were given subjects for each of 13 drug categories according to the following scale: 0, no use of drug; 1 to 4 v, varying use frequencies increasing in stepwise fashion but not reaching abuse proportions; 5, use greater than 8 days per month and abuse as defined in DSM III. Subject's number of crim

A multivariate analysis of the infrared spectra of drugs of abuse.

The infrared (IR) spectra of 15 drugs of abuse were analyzed for similarity by using techniques of numerical taxonomy. The study included six barbiturates (amobarbital, barbital, butabarbital, phentobarbital, phenobarbital, and secobarbital), four amphetamine-related compounds (amphetamine, ephedrine, methamphetamine, and phentermine), and five other drugs (cocaine, heroin, phencyclidine, phendimetrazine, and diazepam). Three character sets were based on increasing numbers (10, 24, and 36) of IR peaks. The cluster analysis, principal component analysis, and nonmetric multidimensional scaling e

Using molecular similarity to highlight the challenges of routine immunoassay-based drug of abuse/toxicology screening in emergency medicine.

BACKGROUND: Laboratory tests for routine drug of abuse and toxicology (DOA/Tox) screening, often used in emergency medicine, generally utilize antibody-based tests (immunoassays) to detect classes of drugs such as amphetamines, barbiturates, benzodiazepines, opiates, and tricyclic antidepressants, or individual drugs such as cocaine, methadone, and phencyclidine. A key factor in assay sensitivity and specificity is the drugs or drug metabolites that were used as antigenic targets to generate the assay antibodies. All DOA/Tox screening immunoassays can be limited by false positives caused by cr

Assessment of urine drug screen utility at autopsy to predict laboratory postmortem blood toxicology.

When faced with increasing drug-related deaths and decline in practicing forensic pathologists, the need to quickly identify toxicology-related deaths is evident in order to appropriately triage cases and expedite turnaround times. Lateral flow immunoassays conducted pre-autopsy offer quick urine drug screen (UDS) results in minutes and are used to inform the need for autopsy. Over 1000 medicolegal cases were reviewed to compare UDS results to laboratory enzyme-linked immunosorbent assay (ELISA) blood results to evaluate how well autopsy UDS predicted laboratory findings. Drugs of abuse that cause developing neurons to commit suicide.

When neuronal activity is abnormally suppressed during the developmental period of synaptogenesis, the timing and sequence of synaptic connections is disrupted, and this causes nerve cells to receive an internal signal to commit suicide, a form of cell death known as "apoptosis". By altering glutamate and GABA transmission alcohol suppresses neuronal activity, causing millions of nerve cells to commit suicide in the developing brain. This proapoptotic effect of alcohol provides a likely explanation for the diminished brain size and lifelong neurobehavioral disturbances associated with the huma

Qualitative screening for drugs of abuse in hair using GC-MS.

A previously described method for the analysis of hair has been modified to include analysis for amphetamines, benzodiazepines, cocaine and its metabolites, methadone and its metabolite, and phencyclidine in addition to opiates on a sample of hair. The samples of hair were washed twice with dichloromethane and cut into 1-mm segments prior to extraction with methanol at 45 degrees C for 18 h. The extracts were split into two parts; both were evaporated to dryness. One half of the extract was derivatized using MBTFA for analysis of amphetamines, and the other half was derivatized using MTBSTFA f

Interference of common household chemicals in immunoassay methods for drugs of abuse.

I report how some adulterants affect results for drugs of abuse in urine as measured by Roche RIA, Syva emit d.a.u., and Abbott TDx fpia (fluorescence polarization immunoassay) for the following drugs: amphetamines, barbiturates, benzodiazepines, cannabinoids, cocaine, opiates, and phencyclidine (PCP). Sodium chloride interfered negatively with all of these drugs when assayed by emit and caused a slight decrease in measured benzodiazepine concentration by fpia. Drug concentrations were also decreased by added H2O2 (emit: benzodiazepine), Joy detergent (emit: cannabinoid, benzodiazepines, PCP),

How often do false-positive phencyclidine urine screens occur with use of common medications?

BACKGROUND: Previous reports describe false-positive urine immunoassay screens for phencyclidine (PCP) associated with use of tramadol, dextromethorphan, or diphenhydramine. The likelihood of these false positives is unknown. OBJECTIVE: We sought to find the relative frequency of false-positive PCP screens associated with these medications and to look for any other medications with similar associations. METHODS: In an IRB-approved study, we retrospectively reviewed charts of all ED encounters with positive urine screens for PCP in our hospital from 2007 through 2011, inclusive. Urine samples

Phencyclidine analog use in Sweden--intoxication cases involving 3-MeO-PCP and 4-MeO-PCP from the STRIDA project.

BACKGROUND: 3-Methoxy-phencyclidine (3-MeO-PCP) and 4-methoxy-phencyclidine (4-MeO-PCP) are analogs of and drug substitutes for the dissociative substance PCP ("Angel dust"), a recreational drug that was most popular in the 1970s. In Sweden, use of methoxylated PCP analogs was noted starting in mid-2013, according to statistics from the Poisons Information Centre. The objective of this case series was to present clinical and bioanalytical data from analytically confirmed non-fatal intoxications associated with 3-MeO-PCP and/or 4-MeO-PCP within the STRIDA project.
