Patient participation: causing moral stress in psychiatric nursing?

AIM: The aim of this study was to explore psychiatric nurses' experiences and perspectives regarding patient participation. Patient participation is an ambiguous, complex and poorly defined concept with practical/clinical, organisational, legal and ethical aspects, some of which in psychiatric units may cause ethical predicaments and moral stress in nurses, for instance when moral caring acts are thwarted by constraints. METHODS: An explorative quantitative pilot study was conducted at a psychiatric subacute unit through three focus group interviews with a total of nine participants. A themat

First-Time Magnetic Resonance Imaging Experience: A Descriptive Phenomenological Study with Implications for Nursing Preparation and Communication.

Background/Objectives: Magnetic resonance imaging (MRI) is often supported by structured safety screening and preparation, yet first-time patients may still experience anxiety, claustrophobic distress, loss of control and non-completion. This study aimed to describe the experiences of adults undergoing MRI for the first time and to identify implications for nursing preparation, communication and diagnostic imaging service design. Methods: A descriptive phenomenological study informed by Husserlian philosophy was conducted in the radiology department of a university hospital in Thailand. Eleven

Making it work: successful collaborative practice.

There are three major examples of collaborative programs between certified nurse-midwives (CNMs) and obstetrician-gynecologists at Baystate Medical Center in Springfield, Massachusetts, within the Department of Obstetrics and Gynecology. One program is a midwifery practice that serves a diverse population in a hospital-based office, four neighborhood health centers, and a correctional facility. Another program provides a triage function for patients who present to the hospital with obstetric or gynecologic problems. The third program introduces a team approach to the education of residents wit

Living in a Limited World: Experience of Lipreaders When Society Is Masked.

The goal of the current interpretive phenomenological study grounded in Heidegger's philosophies was to explore the experience of lipreaders when society was masked during the coronavirus disease 2019 pandemic. Participants were prelingually deafened English-speaking adults who predominantly relied on lip-reading and speaking for communication. Twelve in-depth email interviews were conducted with respondents recruited via social media. Thematic techniques of Benner were employed, and six themes emerged: Limiting of World Resulting in Negative Emotions, Increased Prominence of Deafness, Balanci

Autonomy and autonomy competencies: a practical and relational approach.

This essay will address a general philosophical concern about autonomy, namely, that a conception of autonomy focused on freedom of the will alone is inadequate, once we consider the effects of oppressive forms of socialization on individuals' formation of choices. In response to this problem, I will present a brief overview of Diana Meyers's account of autonomy as relational and practical. On this view, autonomy consists in a set of socially acquired practical competencies in self-discovery, self-definition, self-knowledge, and self-direction. This account provides a distinction between choic

Harm Reduction as a Form of 'Wrap-Around' Care: The Nursing Role.

In addiction treatment, harm reduction is a philosophy that aims to reduce the harms from ongoing alcohol and other drug use. Although abstinence may be the 'gold standard' in reducing harm from ongoing alcohol and other drug use, harm reduction recognises that abstinence may not be achievable for certain individuals. Accordingly, harm reduction is used to enable medical or mental health treatment for individuals who continue to use alcohol and other drugs, providing a form of care which meets individuals where they present to healthcare facilities. Harm reduction accepts ongoing alcohol and o

A modified grounded theory study of how psychiatric nurses work with suicidal people.

BACKGROUND: People with mental health problems continue to present a disproportionately high risk of suicide. Despite the relevance of suicide to psychiatric/mental health (P/MH) nurses, there is a documented paucity of research in this substantive area undertaken by or referring specifically to P/MH nurses; there is currently no extant theory to guide P/MH nursing care of the suicidal person. OBJECTIVES: Accordingly, this paper reports on a study undertaken to determine if P/MH nurses provide meaningful caring response to suicidal people, and if so how. DESIGN: The study used a modified gro

Shifting moral values to enhance access to health care: harm reduction as a context for ethical nursing practice.

BACKGROUND: People who are street involved including those experiencing homelessness and substance use are at increased risk of morbidity and mortality. Such inequities are exacerbated when those facing the greatest inequities in health have the least access to health care. These concerns have rarely been addressed in bioethics and there has been a lack of explicit attention to the dominant societal and organizational values that structure such injustices. The purpose of this paper is to describe the underlying value tensions that impact ethical nursing practice and affect equity in access to

"It's intense, you know." Nurses' experiences in caring for patients requesting euthanasia.

The Belgian Act on Euthanasia came into force on 23 September 2002, making Belgium the second country--after the Netherlands--to decriminalize euthanasia under certain due-care conditions. Since then, Belgian nurses have been increasingly involved in euthanasia care. In this paper, we report a qualitative study based on in-depth interviews with 18 nurses from Flanders (the Dutch-speaking part of Belgium) who have had experience in caring for patients requesting euthanasia since May 2002 (the approval of the Act). We found that the care process for patients requesting euthanasia is a complex an

Workplace bullying in nursing: towards a more critical organisational perspective.

Workplace bullying is a significant issue confronting the nursing profession. Bullying in nursing is frequently described in terms of 'oppressed group' behaviour or 'horizontal violence'. It is proposed that the use of 'oppressed group' behaviour theory has fostered only a partial understanding of the phenomenon in nursing. It is suggested that the continued use of 'oppressed group' behaviour as the major means for understanding bullying in nursing places a flawed emphasis on bullying as a phenomenon that exists only among nurses, rather than considering it within the broader organisational co

Degrees of division: Examining implicit and explicit bias towards nursing doctoral degrees (DNP and PhD).

BACKGROUND: The coexistence of the research-focused Doctor of Philosophy (PhD) and the practice-focused Doctor of Nursing Practice (DNP) has created a complex intraprofessional dynamic. Despite calls for collaboration, relationships are often characterized by competition and role confusion. PURPOSE: This study examined implicit and explicit biases of nurses toward PhD and DNP degrees to establish a baseline for understanding intraprofessional attitudes. METHODS: A cross-sectional design was used (n = 635). Participants completed a demographic survey, explicit bias questions, and a web-based

The meaning of living with dementia and disturbing behaviour as narrated by three persons admitted to a residential home.

AIM: The aim of our study was to illuminate the meaning of living with dementia and disturbing behaviour, as narrated by three persons admitted to a residential home. BACKGROUND: Living with dementia and so-called disturbing behaviour in an institution involves interaction with care providers and fellow residents and poses a challenge to all who are involved. METHODS: We asked the head nurses at a residential home to select persons with dementia and disturbing behaviour who were willing to be interviewed and able to communicate verbally. We performed 10 informal conversational interviews wit

An investigation into competence in TCM of BSN graduates from technological universities in Taiwan.

The purpose of this study was to determine what competences in Traditional Chinese Medicine (TCM) should be possessed by Bachelor of Science in Nursing (BSN) graduates from technological universities in Taiwan. There were two phases in this study: focus groups and expert panel discussion. First, we held four focus groups, and interviewed 31 participants, including consumers, nursing staff and clinical nursing faculty. The interview tapes were transcribed verbatim, were proofed for accuracy and the content was analyzed. The results were then discussed by four academic experts in a panel discuss

Assessment of palliative care knowledge among hospital physicians and nurses in conflict-affected, resource-limited settings: a multicentre cross-sectional study.

OBJECTIVES: Most people who require palliative care reside in low- and middle-income countries, particularly those living in areas affected by geopolitical conflict such as Palestine. Given the potential impact of conflicts on the palliative care knowledge of healthcare providers, it is crucial to assess how it is affected by the recent political instability occurring in the area. This study aimed to assess the overall and domain-specific palliative knowledge among physicians and nurses and evaluate the factors associated with palliative care knowledge. DESIGN: Cross-sectional multicentre stu

Involvement of nurses in the euthanasia care process in Flanders (Belgium): an exploration of two perspectives.

This study explored nurses' involvement in the care process for mentally competent, terminally ill patients requesting euthanasia in general hospitals in Flanders, Belgium. In-depth interviews with 18 nurses who had experience in caring for patients requesting euthanasia since May 2002 were analyzed using grounded theory qualitative methods. We found that the involvement of nurses in the care process is influenced by their predominant perspective on it: the procedural, action-focused perspective, from which good practical organization of the care process is essential; or the existential-interp

The prevailing winds of oppression: understanding the new graduate experience in acute care.

TOPIC: The experience of new graduates in acute care. PURPOSE: The majority of newly graduated nurses make their initial professional role transition in acute care. Being socialized into the dynamic culture of today's hospitals creates significant challenges not only for the nurses themselves but also for institutions of higher education, healthcare administrators, and policy makers across this country. Demanding workloads for hospital nurses, an aging nursing workforce, and the high level of stress inherent in workplaces across North America are factors contributing to an exodus of both new

On donkeys, ingratiates, thinkers and...clinical development units.

Clinical Development Units celebrate nurses as thinkers and nursing as a highly skilled professional activity. They are typified by the exploration and development of therapeutic, consumer-focused and research-based practice and participatory governance. Establishing and maintaining them, therefore, can be problematic in health care facilities typified by centralism, autocracy and nurses' disempowerment. In this type of facility nurses are treated like donkeys and/or ingratiates. What CDUs are, the standards nurses in them strategically and systematically pursue and their achievements to date

Service-learning: an eye-opening experience that provokes emotion and challenges stereotypes.

Descriptive phenomenology was used to explore the lived experience of nursing students in service-learning clinical placement working with families who are homeless. Fourteen students from two different service-learning courses involving a family homeless shelter participated in the interviews. Six constituent descriptions were identified from thematic analysis: eye-opening to realize the effects of homelessness on families; feeling intense emotions that are sometimes hard to express; realizing families who are homeless are both different from and similar to families who have housing; challeng

Productive power and the 'practices of the self' in contraceptive counselling.

This paper explores an under-researched issue within the reproductive health of women - the discursive construction of self-examination instruction in sexual health clinics. The study utilises Foucault's work on 'productive power', the 'practices of the self' and discourse to map out how nurses instruct contraceptive-using women in self-care practices. Forty-nine consultations in sexual health clinics were tape recorded and analysed. The data reveal how nurses utilise the discourse of risk as a technique to reinforce and develop self-care practices - a concept congruent with the Greco-Roman co
