Study of the mechanism of vocal fold vibration during phonation.

The precise mechanism of vocal fold vibrations during phonation is not well known. At present it is impossible to say whether this movement is active or passive. Since the studies of MacLeod & Louis Sylvestre, we know that the vocal muscle is asynchronous and, so, can have a fast oscillatory movement induced by a continuous depolarization of the muscular fibre membranes. Thus, we believe at present that the glottic opening can be merely passive and that the closing can be both passive and active through a myogenic mechanism. The purpose of this work was to try to establish whether this concept

Revisiting Sustained Phonation Time of /s/, /z/, and /α/.

PURPOSE: Research studies of maximum phonation time, have questioned its validity and value as a tool to assess laryngeal competence. This study proposes a fresh look at phonation time, from a different perspective. The interrelationships of the durations between semiocclusion vs. open vocal tract (/s/-/z/ vs. /α/) and voiced vs voiceless (/z/-/α/ vs. /s/) sound are examined to determine whether this approach might provide additional clinical insight into the respiratory phonatory efficiency of patients. METHODS: Comfortable sustained phonation time of /s/, /z/ and /α/ was collected as part o

Measurement reliability of phonation threshold pressure in pediatric subjects.

OBJECTIVES/HYPOTHESIS: Phonation threshold pressure (PTP), the minimum subglottal pressure (Ps ) required for phonation, is sensitive to changes in laryngeal biomechanics and is often elevated with pathology. Little is reported on PTP in children; challenges with task performance and measurement reliability represent barriers to routine clinical assessment. STUDY DESIGN: Pilot study evaluating PTP and Ps measurement reliability in children using labial and mechanical interruption. METHODS: Twenty-two subjects aged 4 to 17 years (10.7 ± 3.9 years) participated. Ten trials were performed for e

Whisper and Phonation: Aerodynamic Comparisons Across Adduction and Loudness.

INTRODUCTION: Whisper is known to be produced by different speakers differently, especially with respect to glottal configuration that influences glottal aerodynamics. Differences in whisper production and phonation types imply important linguistic information in many languages, are identified in vocal pathologies, are used to communicate mood and emotion, and are used in vocal performance. OBJECTIVE: The present study focused on investigating the aerodynamic differences between whisper and phonation at different loudness and adduction levels. METHODS: Three men and five women between 20 and

Airway Hypersensitivity, Reflux, and Phonation Contribute to Chronic Cough.

BACKGROUND & AIMS: Although chronic cough is common, its etiology is often elusive, making patient management a challenge. Gastroesophageal reflux and airway hypersensitivity can cause chronic cough. We explored the relationship between reflux, phonation, and cough in patients with idiopathic chronic cough. METHODS: We performed a blinded, cross-sectional study of nonsmoking patients with chronic cough (duration, >8 weeks) refractory to reflux treatment referred to the Digestive Disease Center at Vanderbilt University. All underwent 24-hour acoustic recording concurrently and temporally synch

Acoustic analyses of thyroidectomy-related changes in vowel phonation.

OBJECTIVES: Changes in vocal function that can occur after thyroidectomy were tracked with acoustic analyses of sustained vowel productions. The purpose was to determine which time-based or spectral/cepstral-based measures of two vowels were able to detect voice changes over time in patients undergoing thyroidectomy. STUDY DESIGN: Prospective, longitudinal, and observational clinical trial. METHODS: Voice samples of sustained /ɑ/ and /i/ recorded from 70 adults before and approximately 2 weeks, 3 months, and 6 months after thyroid surgery were analyzed for jitter, shimmer, harmonic-to-noise

Coordination between posture and phonation in vocal effort behavior.

BACKGROUND: Postural correlates of vocal effort are rarely described in the literature, while they are extensively dealt with in speech therapy. OBJECTIVES: This study aims at determining whether body movement is a side effect of vocal effort or an integral part of communication effort behavior. The answer to this question is mainly based on correlations between posture and phonation. METHOD: Twenty healthy subjects participated in this study. They had to communicate with a listener under 3 conditions requiring different levels of vocal effort. RESULTS: The vocal parameters increased and co

Phonation threshold pressure: a missing link in glottal aerodynamics.

Phonation threshold pressure has previously been defined as the minimum lung pressure required to initiate phonation. By modeling the dependence of this pressure on fundamental frequency, it is shown that relatively simple aerodynamic relations for time-varying flow in the glottis are obtained. Lung pressure and peak glottal flow are nearly linearly related, but not proportional. For this reason, traditional power law relations between vocal power and lung pressure may not hold. Glottal impendance for time-varying flow should be defined differentially rather than as a simple ratio between lung

Aerodynamic relationships associated with normal phonation and paralytic dysphonia.

Physiological differences between normal speakers and those with unilateral vocal cord paralysis are presented. Pressure readings taken intraorally and subglotically are compared during phonation of consonants requiring laryngeal closure and those not requiring cord approximation. From our studies, it is apparent that: (1) a significant difference in air pressure patterns exists between normal speakers and those with vocal cord paralysis, (2) the respiratory apparatus tends to alter normally constant airflow in cases of vocal fold paralysis and (3) either voice therapy or teflon cord injection

Effect of three semi-occluded vocal tract therapy programmes on the phonation of patients with dysphonia: lip trill, water-resistance therapy and straw phonation.

BACKGROUND: To date, the immediate effects of a semi-occluded vocal tract (SOVT) configuration have been thoroughly demonstrated. However, it is not yet sufficiently confirmed whether a therapy programme (i.e. longer than one session) using SOVT exercises leads to an enhanced phonation and improved vocal quality. AIMS: The aim of this study was to investigate the effect of three SOVT therapy programmes: lip trill, water-resistance therapy (WRT) and straw phonation, on the vocal quality, vocal capacities, psychosocial impact and vocal tract discomfort of patients with dysphonia. METHODS & PRO

A study on tracheoesophageal phonation based on a collapsible channel model.

Laryngeal cancer afflicts a large number of people worldwide, and some will need surgery to contain the disease. Currently, tracheoesophageal (TE) speech is a common method of voice rehabilitation for patients who have had their larynges excised. However, despite the relatively high success rate, not everyone is capable of producing the TE voice, usually due to the tonicity of the pharyngoesophageal segment (PES). The present work studies how the tonicity of the muscles of the PES affects TE phonation, focusing mainly on hypotonicity. A simplified collapsible channel model is used. Steady-stat

The relationship between biomechanics of pharyngoesophageal segment and tracheoesophageal phonation.

This study examined the relationship between biomechanical features of the pharyngoesophageal (PE) segment, acoustic characteristics of tracheoesophageal (TE) phonation, and patients' satisfaction with TE phonation. Fifteen patients using TE phonation after total laryngectomy completed the Voice Symptom Scale (VoiSS) and underwent acoustic voice analysis for cepstral peak prominence (CPP) and relative intensity. High resolution manometry (HRM) combined with videofluoroscopy was used to evaluate PE segment pressure and calculate the pressure gradient (ΔP), which was the pressure difference betw

Biomechanical Description of Phonation in Children Affected by Williams Syndrome.

The voice of persons with Williams syndrome (WS) is described as hoarse with a deep and unstable fundamental frequency (f0). These observations may be justified by the deficit of elastin due to a haplo-insufficiency in the ELN gene characteristic of the syndrome. In view of the possible relationship between elastin deficit and dysphonia, a study of the dynamic function of WS phonation was conducted by means of biomechanical analysis. In order to assess the presence of dysphonic symptoms and their degree of severity, the biomechanical description of WS phonation has been evaluated in terms of d

Evaluation of Analytical Modeling Functions for the Phonation Onset Process.

The human voice originates from oscillations of the vocal folds in the larynx. The duration of the voice onset (VO), called the voice onset time (VOT), is currently under investigation as a clinical indicator for correct laryngeal functionality. Different analytical approaches for computing the VOT based on endoscopic imaging were compared to determine the most reliable method to quantify automatically the transient vocal fold oscillations during VO. Transnasal endoscopic imaging in combination with a high-speed camera (8000 fps) was applied to visualize the phonation onset process. Two differ

Differential roles for the thyroarytenoid and lateral cricoarytenoid muscles in phonation.

OBJECTIVES/HYPOTHESIS: Laryngeal adductor muscle dysfunction is a common cause of voice disorders. Reconstitution of adductor muscle function is often the target of therapy, but the effects of these muscles on voice production remain to be fully understood. This study investigated the differential roles of thyroarytenoid (TA) and lateral cricoarytenoid (LCA) muscles on voice production. STUDY DESIGN: Basic science study using an in vivo canine model of phonation. METHODS: The TA and LCA muscle nerve branches were stimulated to obtain seven graded levels of muscle activation, from threshold t
