PURPOSE: Photosensitivity, or photoparoxysmal response (PPR), is an abnormal EEG reaction to intermittent photic stimulation (IPS), consisting of spikes, spike-waves, and intermittent slow waves. Depending on the spread of the abnormal activity, PPR subgroups have been defined as having either propagating PPR or localized, occipital spikes (OS) only. Recent research suggests an enhanced excitability in the occipital cortex may underlie this reaction during IPS, but it remains unclear whether changes in excitability affect the function of the occipital cortex to other, less provocative visual s

The aim of this study was to systematically describe the semiology of non-embolic transient monocular visual field loss (neTMVL). We conducted a retrospective case note analysis of patients from Moorfields Eye Hospital (1995-2007). The variables analysed were age, age of onset, gender, past medical history or family history of migraine, eye affected, onset, duration and offset, perception (pattern, positive and negative symptoms), associated headache and autonomic symptoms, attack frequency, and treatment response to nifedipine. We identified 77 patients (28 male and 49 female). Mean age of on

OBJECTIVES: We hypothesized that the hyperexcitability of occipital cortex neurons may predispose migraine subjects to develop spreading depression, the putative basis of migraine with aura (MwA). To date there is no direct physiologic correlate confirming this in patients. Accordingly, we evaluated the differences in the threshold of occipital cortex excitation between MwA patients and normal controls (C) using transcranial magnetic stimulation (TMS).
METHODS: TMS was performed using the Cadwell MES 10 stimulator. A circular coil 9.5 cm in diameter was applied to the occipital scalp (7 cm ab

Transient visual field defects (VFDs) and phosphenes were induced in normal volunteers by means of transcranial magnetic stimulation (TMS) using a circular magnetic coil of 12.5 cm diameter placed with its lower rim 2-4 cm above the inion in the midline. Subjects had to detect small, bright dots presented randomly for 14 ms in one of 60 locations on a computer screen resulting in a plot of the central 9 degrees of the visual field. In 8 of 17 subjects, transient VFDs were inducible at peak magnetic field strenghts of 1.1-1.4 T. In the central 1-3 degrees, detection of targets was impaired in b

BACKGROUND: Noninvasive transcranial alternating current stimulation (tACS) of the cochlea might be a promising new therapeutic option for patients with chronic tinnitus. However, electric stimulation of small, sensitive target regions, such as the inner ear, can cause adverse side effects (SEs).
OBJECTIVE: The objective of this study was to identify stimulation parameters with low SE profiles while reliably stimulating the cochlea, thereby improving patient comfort and safety. This pilot study also aims to identify stimulation settings suited for developing a medical device intended to treat

A non-fatal case of Nerium oleander (common oleander) self-poisoning in a 45-year-old female is presented. Initial symptoms were nausea and vomiting, abdominal pain, phosphenes, cardiovascular shock and sinus brady-cardia. Blood and urine were assayed for oleandrin, the major cardiac glycoside of N. oleander, using a highly specific HPLC/MS procedure. The blood concentration of oleandrin at admission was 1.1 ng/ml. This is the first report of an oleander intoxication ascertained by the mass spectrometric identification of oleandrin in blood. HPLC/MS appears to be the method of choice for the f

PURPOSE: The resolution provided by present state-of-the-art retinal prostheses is severely limiting for recipients, partly due to the broad spread of activation in the retina in response to monopolar (MP) electrical stimulation. Focused multipolar (FMP) stimulation has been shown to restrict neural activation in the cochlea compared to MP stimulation. We extended the FMP stimulation technique to a two-dimensional electrode array and compared its efficacy to MP and hexapolar (HP) stimulation in the retina.
METHODS: Normally-sighted cats (n = 6) were implanted with a suprachoroidal electrode a

OBJECTIVE: The photoparoxysmal response (PPR) involves rapid spread of epileptic activity from visual to parietal and frontal areas. We used a transcranial magnetic stimulation (TMS) technique to assess the physiologic connections between primary visual (V1) and motor (M1) areas in patients with idiopathic generalized epilepsy (IGE). We hypothesized that in PPR-positive patients, M1 would respond excessively to inputs from V1.
METHODS: Eleven photosensitive patients with IGE who had a PPR at the time of the study were compared with 10 similar patients without a PPR, and with 11 healthy subjec

In humans, hyperventilation (HV) has various effects on systemic physiology and, in particular, on neuronal excitability and synaptic transmission. However, it is far from clear how the effects of HV are mediated at the cortical level. In this study we investigated the effects of HV-induced hypocapnia on primary motor (M1) and visual cortex (V1) excitability. We used 1) motor threshold (MT) and phosphene threshold (PT) and 2) stimulus-response (S-R) curves (i.e., recruitment curves) as measures of excitability. In the motor cortex, we additionally investigated 3) the intrinsic inhibitory and f

OBJECTIVE: To determine the excitability of the visual cortex by phosphene thresholds (PT) in patients with migraine using transcranial magnetic stimulation (TMS) with single- and paired-pulses.
METHODS: Nineteen patients with migraine with aura (MWA), 19 patients with migraine without aura (MWoA), and 22 control subjects were included. Patients were free from preventive anti-migraine treatment and were investigated within 3 days before or after an acute migraine attack. In each subject, PT were assessed by single-pulse and paired-pulse TMS with an interstimulus interval of 50 ms.

Visual prostheses represent a groundbreaking avenue for restoring vision in individuals with visual impairments. These devices utilize electrode arrays positioned in early visual processing areas, like the retina, thalamus, or primary visual cortex. Connected to a camera, they transform a stream of video to electrical stimulation to present the visual environment through patterned activation of phosphenes. Visual prostheses offer the potential to enhance visual function and thereby quality of life for users, however, understanding and replicating motion perception in a manner akin to natural v

In outer retinal degenerative diseases such as retinitis pigmentosa, choroideremia, and geographic atrophy, 30% of the ganglion cell layer in the macula remains intact. With subretinal and epiretinal prostheses, these inner retinal cells are stimulated with controlled electrical current by either a microphotodiode placed in the subretinal area or a microelectrode array tacked to the epiretinal region. As the patient learns to interpret the resulting phosphene patterns created in the brain through special rehabilitation exercises, their orientation, mobility, and quality of life increase. Impla

A medical expert's opinion had to be given on five patients complaining about temporary visual disturbances. 1. Haze caused by keratoconjunctivitis sicca; 2. changing refraction related to diabetes; 3. phosphenes in a paralyzed monoculus; 4. double vision with fatigue in myasthenia; 5. presumed elevation of the risk for accidents in congenital nystagmus. The loss of earning capacity in these cases should be calculated as the mean of the earning capacity during the phases of least and maximal disturbance, if it is not yet included in the loss caused by additional stationary defects. Alterations in the visual system may underlie visual hallucinations in dementia with Lewy bodies (DLB). However, cortical excitability as measured by transcranial magnetic stimulation (TMS) and functional magnetic resonance imaging (fMRI) activation of lower visual areas (V1-3) to visual stimuli appear normal in DLB. We explored the relationship between TMS-determined phosphene threshold and fMRI-related visual activation and found a positive relationship between the two in controls but a negative one in DLB. This double dissociation suggests a loss of inhibition in the visual system in DLB, w

Most of the astronauts experience visual illusions, apparent flashes of light (LF) in absence of light. The first reported observation of this phenomenon was in July 1969 by Buzz Aldrin, in the debriefing following the Apollo 11 mission. Several ground-based experiments in the 1970s tried to clarify the mechanisms behind these light flashes and to evaluate possible related risks. These works were supported by dedicated experiments in space on the following Apollo flights and in Low Earth Orbit (LEO). It was soon demonstrated that the LF could be caused by charged particles (present in the spac

Objective.Retinal prostheses aim to restore vision in patients blinded by photoreceptor degeneration, yet functional outcomes vary substantially across individuals. Scotomas-regions of reduced or absent visual sensitivity-are a defining feature of retinal degenerative diseases, but their perceptual status is often underappreciated in the design and evaluation of prosthetic vision. Here, we investigated how differences in scotoma perception influence perceptual visual task performance and perceptual representations.Approach.Psychophysical experiments were conducted using a virtual reality platf

There is very little published literature on treatment strategies for functional visual loss. We present two people with long duration of functional visual loss who achieved complete recovery with a novel combination of therapeutic approaches. These included: (1) the clinician being transparent regarding positive signs such as optokinetic nystagmus, in order to persuade family members of the diagnosis, (2) regularly positively acknowledging everyday events that indicated visual ability, (3) using occipital transcranial magnetic stimulation to induce phosphenes as an artificial temporary visual
