Cortical spreading depression can be triggered by sensory stimulation in primed wild type mouse brain: a mechanistic insight to migraine aura generation.

BACKGROUND: Unlike the spontaneously appearing aura in migraineurs, experimentally, cortical spreading depression (CSD), the neurophysiological correlate of aura is induced by non-physiological stimuli. Consequently, neural mechanisms involved in spontaneous CSD generation, which may provide insight into how migraine starts in an otherwise healthy brain, remain largely unclear. We hypothesized that CSD can be physiologically induced by sensory stimulation in primed mouse brain. METHODS: Cortex was made susceptible to CSD with partial inhibition of Na+/K+-ATPase by epidural application of a lo

Reversible central neural hyperexcitability: an electroencephalographic clue to hypocalcaemia.

A 23-year-old male patient presented with cognitive decline and seizures. Examination revealed Chvostek's and Trousseau's signs. Investigations revealed hypocalcaemia, hyperphosphatemia and normal intact parathyroid hormone levels. Imaging showed calcifications in bilateral basal ganglia, thalamus and dentate nuclei. Interictal electroencephalogram showed theta range slowing of background activity and bilateral temporo-occipital, irregular, sharp and slow wave discharges, which accentuated during hyperventilation, photic stimulation and eye closure. Appearance of epileptiform discharges after

Video game induced seizures.

Fifteen patients who experienced epileptic seizures while playing video games are described together with a review of 20 cases in the English literature. Nine of the 15 cases and all but two of the reported cases experienced their first seizure while playing video games. Two thirds of patients had idiopathic generalised epilepsy and mainly reported generalised tonic clonic seizures, but some had typical absence seizures and myoclonic jerks while playing video games. In this series, 30% with idiopathic generalised epilepsy had juvenile myoclonic epilepsy. Overall, 70% of patients with idiopathi

Cerebral blood flow and EEG changes in preterm infants with patent ductus arteriosus.

It is unknown whether the decreased cerebral blood flow seen in infants with a large patent ductus arteriosus is associated with cerebral dysfunction. Decreased cerebral blood flow in adult human and animal models has been associated with altered electroencephalography (EEG), spectral-analyzed EEG, and EEG response to photic stimulation. Cerebral blood flow velocity, EEG, spectral analysis of EEG, and photic alteration of EEG spectra were evaluated in 8 infants before and after closure of a significant patent ductus arteriosus and in 10 control infants without a patent ductus arteriosus. All i

Visual Electroencephalography Assessment in the Diagnosis and Prognosis of Cognitive Disorders.

PURPOSE: Electroencephalography (EEG) is a noninvasive diagnostic tool that can be of diagnostic value in patients with cognitive disorders. In recent years, increasing emphasis has been on quantitative EEG analysis, which is not easily accessible in clinical practice. The aim of this study was to assess the diagnostic and prognostic value of visual EEG assessment to distinguish different causes of cognitive disorders. METHODS: Patients with cognitive disorders from a specialized memory clinic cohort underwent routine workup including EEG, neuropsychological testing and brain imaging. Electro

Electrographic Features of Epilepsy With Eyelid Myoclonia With Photoparoxysmal Responses.

PURPOSE: Epilepsy with eyelid myoclonia (EMA) is characterized by eyelid myoclonia, eyelid closure sensitivity, and photosensitivity. EEG may manifest with frontal-predominant (FPEDs) or occipital-predominant epileptiform discharges (OPEDs). Data on clinical and electrographic features of these two subtypes are lacking. The purpose of our research was to look at baseline electroclinical features of EMA subtypes and to study electrographic findings of patients with EMA during intermittent photic stimulation (IPS). METHODS: We retrospectively identified all patients who had photoparoxysmal resp

Informing patients about the impact of provocation methods increases the rate of psychogenic nonepileptic seizures during EEG recording.

Observation of psychogenic nonepileptic seizures (PNESs) during video-EEG represents the diagnostic gold standard for PNESs. Different provocative techniques have been used to increase PNES frequency during EEG. These techniques include placebo administration, suggestion strategies, or both. In order to avoid the appearance of deception, we investigated the following hypothesis: If patients with PNESs were informed about the possible reduction of seizure threshold caused by hyperventilation and photic stimulation prior to EEG without any other suggestive strategies, PNESs would occur more freq

Does melanopsin bistability have physiological consequences?

Recent publications in the Journal of Biological Rhythms have focused on the hypothesis that the property of melanopsin bistability is functionally translated to in vivo mammalian physiology. Physiological consequences of photopigment bistability likely can be inferred from the more extensive invertebrate literature. In invertebrates, photopigment bistability results in (a) photoreceptor independence from specialized chromophore regenerating systems, (b) long-wavelength enhancement of a blue light effect, (c) expression of a prolonged depolarization after potential following intense blue light

EEG photic driving: right-hemisphere reactivity deficit in childhood autism. A pilot study.

In 14 autistic boys, aged 6-14 years, free of drug treatment, with relatively intact verbal functions and without severe or moderate mental retardation (I.Q. 91.4+/-22.8), intermittent photic stimulation at 11 fixed frequencies of 3-24 Hz revealed latent deficiency of the right hemisphere in the photic driving reactivity, predominantly at the fast alpha and beta frequencies of stimulation. The left-side prevalence was observed: 1) in the total number of driving peaks evaluated for the first four harmonics in the EEG spectra of 14 cortical areas and 2) in the driving amplitude in the spectra of

Intensity dependence of auditory evoked potentials during light interference in migraine.

Migraine patients show interictally a strong intensity dependence of auditory evoked cortical potentials (IDAP) and a lack of habituation of evoked potentials. Photic drive on high-frequency flash stimulation is another well-known interictal feature in migraineurs, associated with alpha-rhythm hyper-synchronisation. We compared therefore the influence of light stimulation on IDAP in healthy volunteers (HV) and migraine patients. A continuous flash stimulation was delivered during the recording of auditory evoked potentials at suprathreshold increasing stimulation intensities. IDAP was measured

Paediatric-onset neuronal ceroid lipofuscinosis: first symptoms and presentation at diagnosis.

Neuronal ceroid lipofuscinoses (NCLs) are rare, progressive disorders. Through this series of 20 patients with NCL, we illustrate differences between subtypes in their presenting symptoms and clinical, imaging, and electrophysiological results to raise awareness of symptom diversity. Data were available on presenting symptoms, genetics, magnetic resonance imaging (MRI), electroencephalography (including with low-frequency intermittent photic stimulation), visual responses, and electron microscopy. Causal mutations were identified in 10 patients. Eleven patients had neuronal ceroid lipofuscinos

Preliminary findings of proton magnetic resonance spectroscopy in occipital cortex during sleep deprivation.

Proton magnetic resonance spectroscopy ((1)H MRS) has revealed biochemical alterations in various psychiatric disorders. Changes in brain metabolites may be caused not only by the disease's progression or response to treatment, but also by physiological variability. The aim of this study was to use (1)H MRS to assess the effects of specific short-term physiological states on major metabolites. Eight healthy women underwent (1)H MRS at the beginning and end of a 40-h period of sleep deprivation. The ratios of N-acetyl-aspartate (NAA), total creatine (tCr), and choline-containing compounds (Cho)

Changes in circadian rhythms and sleep quality with aging: mechanisms and interventions.

Literature is reviewed indicating that aging is characterized by changes in circadian rhythms and sleep quality. The most marked change is an attenuation of amplitude. An advance of phase, a shortening of period, and a desynchronization of rhythms are also evident. The mechanisms underlying these changes are unknown. However, age-related changes in the retina, suprachiasmatic nucleus, and pineal gland seem relevant along with behavioral changes such as a reduction in physical activity and exposure to photic stimulation. Changes in circadian rhythms are frequently associated with a reduction in

Seizures and adverse events during routine scalp electroencephalography: a clinical and EEG analysis of 1000 records.

OBJECTIVE: To quantify the incidence of seizures and adverse events during standard electroencephalography (EEG). METHODS: A retrospective random sample of 1000 of a total of 3391 reports of standard scalp EEG recordings during 2002 at Kings College Hospital were studied, and adverse events during standard EEG were recorded. Photic induced seizures and epileptiform activity were compared with the resting, hyperventilation and sleep EEG. RESULTS: Adverse events occurred in 131 records (13.1%), including seizures in 60 records (45 electro-clinical and 15 non-epileptic seizures). The overall in

Abnormal connectivity of the visual pathways in human albinos demonstrated by susceptibility-sensitized MRI.

We studied activation of the human visual cortex (VC) using susceptibility-sensitized MRI at 1.5 Tesla. Three albinos and six healthy controls underwent a series of monocular and binocular photic flash stimulation. Monocular stimulation in albinos caused predominant contralateral activation with a small, well-delineated area in the anterior part of the VC in the ipsilateral hemisphere. This finding was consistent with a chiasmal crossing anomaly in albinism. All controls had symmetric patterns of activation during monocular stimulation. Functional MRI represents a promising method for evaluati

Photosensitivity during the hypersomnic phase in a patient with Kleine-Levin syndrome.

The case of an adolescent with Kleine-Levin syndrome is presented who exhibited a consistent and predictable photoparoxysmal self-limited response to intermittent photic stimulation during all relapses of his hypersomnic phase. A possible cause of this association that has not been previously reported is speculated based on observations concerning the two disorders.

Eectroencephalogram in "locked-in" syndrome.

Twenty-three EEG studies in eight patients with "locked-in" syndrome were reviewed. The EEG was either normal or minimally slow in all except one patient. All showed reactivity to various stimuli. Photic stimulation elicited a photic driving response at one time or another in all the seven patients where it was attempted. Althouth the basic activity may be similar, it is the preservation of alertness and the presence of reactivity of the EEG which distinguishes the "locked-in" patients from those comatose due to an extensive brain stem lesion. In an unresponsive patient, an EEG resembling that

Utility of EEG Activation Procedures in Epilepsy: A Population-Based Study.

PURPOSE: No previous population-based study has addressed the contribution of activation procedures to the yield of epileptiform abnormalities on serial EEGs. We assessed yield of activation-related epileptiform abnormalities and predictors of finding an activation-related abnormality with multiple EEGs in a population-based study of newly diagnosed epilepsy. METHODS: We used the resources of the Rochester Epidemiology Project to identify 449 residents of Rochester, Minnesota with a diagnosis of newly diagnosed epilepsy at age 1 year or older, between 1960 and 1994, who had at least one EEG.

Abnormal brain activation to visual stimulation in cocaine abusers.

Chronic cocaine abuse has been associated with cerebrovascular pathology. This is likely to reflect its vasoactive effects; cocaine produces vasoconstriction and reduces cerebral blood flow. We propose that cerebrovascular pathology in chronic cocaine abusers would result in abnormal BOLD [blood oxygenation level dependent] responses to activation stimuli. Here, we used fMRI to compared the BOLD response to photic visual stimulation in neurologically intact active cocaine abusers to that in non-drug-using healthy controls. Cocaine abusers showed a significantly enhanced positive BOLD response
