[Semen analysis under photochemotherapy (PUVA-therapy) (author's transl)].

In 9 male patients with psoriasis vulgaris a semen analysis before and during photochemotherapy with 8-methoxypsoralen and UVA (PUVA) was performed to rule out drug-induced toxic damage of spermatogenesis or impairment of fertility due to scrotal hyperthermia. Two hours after oral application of 40--60 mg 8-methoxypsoralen the patients had been irradiated in UVA high intensity treatment units. PUVA-treatments were performed four times weekly until total body clearing was achieved. For complete remission 13--26 (mean 20.5) PUVA-treatments were necessary. Corresponding total UVA-doses were 35.3-

Photochemotherapy for mycosis fungoides: long-term followup study.

A 3-year followup on ten of 12 patients with mycosis fungoides (MF) treated with photochemotherapy (PUVA; psoralens + ultraviolet light) reveals eight patients whose diseases have remained clear while receiving maintenance PUVA. An additional 35 patients with MF have been successfully treated with PUVA. Eight cases of parapsoriasis en plaque have cleared with PUVA. PUVA is indicated in early eczematous and plaque stages of MF. It may be used as adjunctive therapy in tumor-stage MF treated with X-ray therapy or chemotherapy.

Vitiligo-like leucoderma during photochemotherapy for mycosis fungoides.

We describe four patients with mycosis fungoides (MF) in whom depigmentation, and also leucotrichia in one, occurred following the resolution of the eruption during phototherapy (psoralen plus ultraviolet A treatment in three patients, climatotherapy in one). In all cases, the depigmentation was localized to the area of the pre-existing MF lesions. There was no clinically obvious phototoxicity. Biopsy study including S100 staining in all cases, and electron microscopy in one case, demonstrated the total absence of melanocytes, with no evidence of MF. It is suggested that the phototherapy may h

[PUVA-bath photochemotherapy in Hallopeau's acrodermatitis continua suppurativa].

A 73-year-old man presented with severe, relapsing acrodermatitis continua of Hallopeau, which had been resistant to prior local and systemic therapy for eight years. The patient was treated with selective hand PUVA-bath photochemotherapy. The cumulative dose of UVA used over 10 weeks of treatment was 54.6 J/cm2 on the palms and 26.8 J/cm2 on the dorsum of the hands. The single UVA doses ranged from 0.5 to 2.5 J/cm2 on the palms and 0.2 to 1.4 J/cm2 on the dorsum of the hands. After 16 treatment sessions, the acrodermatitis continua started to improve, and after 24 treatments, had cleared comp

Oral photochemotherapy in vitiligo: follow-up, patient compliance.

BACKGROUND: Vitiligo is a common, acquired condition of unknown etiology. The only practical treatment for patients with vitiligo is psoralen ultraviolet A (PUVA). No satisfactory method is available to measure the influence of certain factors on the therapeutic response. METHODS: Twenty-nine patients with generalized vitiligo were studied for the effectives of PUVA therapy. The comparative study revealed the importance of certain variables (i.e., the proper dose of psoralen, the regular attendance of the patient, and the duration of the treatment). A scoring system had been designed and empl

[Photochemotherapy (PUVA) of acute graft-versus-host disease].

For several years, psoralen and UV-A light (PUVA) therapy has been used in the treatment of chronic graft-versus-host disease (GvHD) following allogeneic bone marrow transplantation (BMT). There is little experience with PUVA treatment of acute GvHD. Allogeneic BMT was performed in 25 patients, 10 of whom developed acute GvHD despite immunosuppressive therapy. Six patients with acute cutaneous GvHD grade II-III (n = 2 grade II, n = 4 grade III) were treated with PUVA. We present the results. All the PUVA-treated patients improved markedly after 5-12 sessions of irradiation. In 5 patients the s

Sézary syndrome and seronegative polyarthritis: treatment with extracorporeal photochemotherapy.

We describe a patient with therapy-resistant cutaneous T-cell lymphoma, Sézary syndrome variant, in association with concurrent polyarthritis and vitiligo, who was successfully treated with extracorporeal photochemotherapy (ECP). The combination of Sézary syndrome with seronegative rheumatoid arthritis is rare. In our patient the T-cell lymphoma was refractory to standard treatments that included psoralen-UVA, lymph node irradiation, and polychemotherapy. ECP has been shown to be effective in the treatment of selected cases of Sézary syndrome. There is a strong suggestion that ECP as a monothe

Extracorporeal photochemotherapy: a new therapeutic approach for allograft rejection.

Photopheresis (ECP) is a new immunomodulatory therapy in which recipient lymphocytes are treated extracorporeally with 8-methoxypsoralen (8-MOP) and ultraviolet light. The treatment seems to induce an inhibition of both umoral and cellular rejections after transplantation. More than 160 transplanted patients have been treated with ECP (107 heart, 30 kidney, 24 lung and I liver) in different studies. Indication for ECP included acute rejection, recurrent/refractory rejection, prophilaxis of rejection, need of reducing standard immunosuppression. Patient survival is satisfactory. Only one study

Treatment of refractory epidermolysis bullosa acquisita with extracorporeal photochemotherapy.

Treatment of epidermolysis bullosa acquisita (EBA) with conventional therapy frequently does not result in improvement. Extracorporeal photochemotherapy (ECP) is a novel immunomodulating technique which might be of benefit in the treatment of autoimmune disease. We prospectively studied three patients with refractory EBA who were treated with ECP. All three patients had improvement in objective measurement of their disease severity while two of the three had significant subjective improvement in their skin fragility and the clinical activity of their disease. Both the objective and subjective

Cutaneous malignant melanoma appearing during photochemotherapy of mycosis fungoides.

We report a case of malignant melanoma that appeared in a 56-year-old man with mycosis fungoides (stage Ia) during treatment with PUVA. The cumulative UVA dose was 1,177 J/cm2. The pigmented lesion was removed and PUVA therapy discontinued. Histological examination revealed a superficial spreading malignant melanoma (1.77 mm thick, Clark level IV). The delayed-type cutaneous hypersensitivity was studied. The presence of a second malignancy after mycosis fungoides and PUVA therapy may have been coincidental. Nevertheless, this case suggests that the immunosuppression induced by mycosis fungoide

Treatment of severe cardiac allograft rejection with extracorporeal photochemotherapy.

Two patients were treated with photopheresis for marked cardiac allograft rejection with hemodynamic compromise that had become unresponsive to standard therapy. Multiple episodes of rejection had occurred, and initial response to standard therapy was favorable. However, progressive deterioration was documented by serial endomyocardial biopsies, fever, congestive heart failure, and abnormal cardiac catheterization findings. In the absence of retransplantation, death seemed imminent. Photopheresis was begun. Both patients received oral 8-methoxypsoralen and > or = 5 x 10(9) mononuclear cells we

Treatment of Adult T Cell Leukaemia by Extracorporeal Photochemotherapy.

Four patients with chronic or smouldering type adult T cell leukaemia (ATL) were treated by extracorporeal photochemotherapy (photopheresis). From 4 to 8 months after starting photo-pheresis, skin lesions with ATL cell infiltration began to disappear. Cell surface markers in three patients showed improvement. In one patient, a serial decrease of soluble interleukin-2 receptor levels (950 U/ml to 620) in the serum was observed after 4 months. This pilot study suggests that photopheresis may be successfully applied in ATL. It is still necessary to continue follow-up observations in these patient

Oral methoxsalen photochemotherapy of extensive pityriasis alba. Preliminary report.

Six patients with extensive pityriasis alba were treated with oral methoxsalen, followed by exposure to midday summer sun or exposure to long-wave ultraviolet radiation in a psoralens plus ultraviolet A (PUVA) cabinet. Complete clearing or marked improvement was obtained in five patients in less than 4 weeks of treatment. The patient with the most extensive skin involvement and the longest duration of the disease achieved a marked degree of improvement after 15 weeks of therapy. A long-term follow-up is still required to define the maintenance treatment and its efficacy.

[Laboratory studies of long-term photochemotherapy of psoriasis vulgaris].

The study of 40 laboratory parameters from a total of 40,000 individual samples taken from patients who had been treated by means of systemic PUVA therapy for a period up to three years did not reveal any evidence for negative influences on selected peripheral blood enzymes, substrates, metabolites, proteins, lipid fractions, electrolytes, antistreptolysin titer, rheumatoid factors and coagulation values, or urinalysis values. The changes observed were either statistically insignificant, remained within normal ranges, or could not be classified. Under the conditions described, no evidence for

[Histopathology and histochemistry of psoriasis under photochemotherapy (author's transl)].

Fifty-two biopsies from involved and clinically normal looking skin of patients with psoriasis vulgaris and from normal control subjects, all being treated with 8-MOP-UVA (PUVA) were obtained between days 2 and 300. The following parameters were investigated: 1. initial PUVA effects; 2. initial regression of psoriasis under PUVA-therapy; 3. late effects of PUVA-therapy on regression of psoriatic lesions; 4. EFFects on melanocytes and pigmentation; and 5. long-term effects of PUVA-therapy. Paraffin embedded and cryostat sections were prepared with routine stains for light microscopy and enzyme

Treatment of localized mycosis fungoides with digital UV photochemotherapy.

BACKGROUND/PURPOSE: Photochemochemotherapy with 8-methoxypsoralen and UV-A light (PUVA) is a well-established treatment for mycosis fungoides (MF), although evidence for this therapy by means of prospective studies is scarce. However, long-term risks of PUVA are premature skin aging and development of nonmelanoma skin cancer. We therefore evaluated a device for targeted UV therapy, which reduces irradiation of unaffected skin in MF patients. METHODS: Ten patients with patch- or plaque-type MF affecting less than 10% body surface area were included in a prospective study. A total of 14 lesions

[Extracorporeal photochemotherapy for treatment of clonal T cell proliferations].

Extracorporeal photochimiotherapy (ECP) is based on the exposure of peripheral blood mononuclear cells to the photosensitizing agent (psoralen or 8MOP) and UVA radiation. Mononuclear cells are harvested by cytapheresis and reinfused to the patient after irradiation. This cell therapy has been shown to be effective in the treatment of selected diseases mediated by clonal T cells proliferation such as Sezary T cells lymphoma, rejection after solid organ transplantation and graft-versus-host disease but results obtained in autoimmune diseases are less convincing. ECP is well tolerated with very f
