Phthalazinone inhibitors of inosine-5'-monophosphate dehydrogenase from Cryptosporidium parvum.

Cryptosporidium parvum (Cp) is a potential biowarfare agent and major cause of diarrhea and malnutrition. This protozoan parasite relies on inosine 5'-monophosphate dehydrogenase (IMPDH) for the production of guanine nucleotides. A CpIMPDH-selective N-aryl-3,4-dihydro-3-methyl-4-oxo-1-phthalazineacetamide inhibitor was previously identified in a high throughput screening campaign. Herein we report a structure-activity relationship study for the phthalazinone-based series that resulted in the discovery of benzofuranamide analogs that exhibit low nanomolar inhibition of CpIMPDH. In addition, the

Effect of N-Acetyltransferase 2 Genotype on the Pharmacokinetics of Hydralazine During Pregnancy.

Hydralazine, an antihypertensive agent used during pregnancy, undergoes N-acetylation primarily via N-acetyltransferase 2 (NAT2) to form 3-methyl-1,2,4-triazolo[3,4-a]phthalazine (MTP). To characterize the steady-state pharmacokinetics (PK) of hydralazine during pregnancy and evaluate the effects of NAT2 genotype on hydralazine and MTP PK during pregnancy, 12 pregnant subjects received oral hydralazine (5-25 mg every 6 hours) in mid- (n = 5) and/or late pregnancy (n = 8). Serial blood samples were collected over 1 dosing interval, and steady-state noncompartmental PK parameters were estimated.

[OLAPARIB TREATMENT IN METASTATIC CASTRATION-RESISTANT PROSTATE CANCER WITH BRCA2 MUTATION AND HIGH TUMOR MUTATIONAL BURDEN: A CASE REPORT].

In recent years, pharmacologic treatment for advanced prostate cancer has included novel hormonal agents and chemotherapeutic drugs from the initial stages. When resistance develops, personalized therapy based on genomic profiling has become increasingly common. We report a case of castration-resistant prostate cancer (CRPC) with both a BRCA2 mutation and high tumor mutational burden (TMB-high), in which olaparib produced a temporary response. The patient had received hormone therapy, radiotherapy, and chemotherapy, all of which became ineffective. Based on companion diagnostic testing, both o

Effects of Azelastine on Glioblastoma Cells.

BACKGROUND/AIM: Glioblastoma is an aggressive brain tumor with poor prognosis and limited treatment options. Azelastine (AZL), a histamine H1 receptor antagonist known to cross the blood-brain barrier, has shown anticancer activity in other malignancies. This study investigated the anti-proliferative effects of AZL on glioblastoma cells and its potential to enhance the efficacy of standard anticancer agents. MATERIALS AND METHODS: Human glioblastoma cell lines U251 and T98G were treated with AZL. Cell viability was assessed via WST-8 assay. The impact of AZL on actin cytoskeleton and cell cyc

Olaparib-associated toxicity in cancer patients: a systematic review and meta-analysis.

PURPOSE: To investigate the characteristics of olaparib-associated adverse events (AEs) in cancer patients. METHODS: Databases were searched for phase II and III randomized controlled trials (RCTs) involving olaparib treatment up to March 2024. A systematic assessment was performed. RESULTS: Twenty-seven RCTs involving 9542 patients were included. This meta-analysis indicates that olaparib could significantly increase the risk of developing any all-grade (RR, 1.08; 95% CI, 1.03-1.13; p = 0.001) and high-grade (RR, 1.45; 95% CI, 1.19-1.77; p = 0.0003) AEs in cancer patients. Targeting BRPF3 moderately reverses olaparib resistance in high grade serous ovarian carcinoma.

PARP inhibitors (PARPi) kill cancer cells by stalling DNA replication and preventing DNA repair, resulting in a critical accumulation of DNA damage. Resistance to PARPi is a growing clinical problem in the treatment of high grade serous ovarian carcinoma (HGSOC). Acetylation of histone H3 lysine 14 (H3K14ac) and associated histone acetyltransferases (HATs) and epigenetic readers have known functions in DNA repair and replication. Our objectives are to examine their expression and activities in the context of PARPi-resistant HGSOC, and to determine if targeting H3K14ac or associated proteins ha

Effectiveness of Olaparib Treatment in a Patient with Gallbladder Cancer with an ATM-Inactivating Mutation.

Here, we report a case of postoperative recurrence of gallbladder carcinoma (GBC) in a patient who declined systemic chemotherapy. ATM S1905Ifs*25 and STK11 K262Sfs*25 mutations were detected by next-generation sequencing. Oral administration of olaparib was initiated. One month later, the patient experienced relief of clinical symptoms, a decrease in CA19-9 level, and a reduction in abnormal signal in the subcapsular region. The tumor response remained stable for approximately 13 months. This is the first case to demonstrate the clinical benefits of olaparib treatment in a patient with GBC ha

A case report of a dramatic response to olaparib in a patient with metastatic pancreatic cancer harboring a germline BRCA2 mutation.

RATIONALE: Pancreatic cancer (PC) is considered as one of the deadliest cancers all over the world. Germline and somatic BRCA1/2 mutations have been widely studied in breast and ovarian carcinomas as they have been found to enhance the risk for disease progression. Olaparib, an oral poly(adenosine diphosphate-ribose)polymerase (PARP) inhibitor, has been approved for the treatment strategy of ovarian cancer with any BRCA1/2 mutations. There is a lack of studies which focus on the treatment of other cancer with BRCA-Mutation. PATIENT CONCERNS: This report describes a patient whose presenting co

Efficient 11C-carbonylation of isolated aryl palladium complexes for PET: application to challenging radiopharmaceutical synthesis.

We describe the successful implementation of palladium-aryl oxidative addition complexes as stoichiometric reagents in carbonylation reactions with (11)CO to produce structurally challenging, pharmaceutically relevant compounds. This method enables the first (11)C-carbonyl labeling of an approved PET tracer, [(11)C]raclopride, for the dopamine D2/D3 receptor by carbonylation with excellent radiochemical purity and yield. Two other molecules, [(11)C]olaparib and [(11)C]JNJ 31020028, were efficiently labeled in this manner. The technique distinguishes itself from existing methods by the markedly

[Magnesium deficiency dermatitis--changes in leukotriene levels and effects of azelastine hydrochloride on the dermatitis and leukotriene B4].

To estimate the pathogenesis of magnesium deficiency dermatitis (MDD) in the hairless rats fed with a hypomagnesic diet, plasma concentrations of leukotrienes (LTB4, LTC4, LTD4) were measured. Inhibitory effect of azelastine hydrochloride (AZ), which is known as an inhibitor of 5-lipoxygenase, on MDD was also evaluated. In the hairless rats with magnesium deficiency condition, plasma level of leukotriene B4 was increased, whereas LTC4 and LTD4 showed no changes. In AZ-administered rats, MDD was depressed with the dose-dependent tendency. However, the decrease of LTB4 level did not follow thera

Olaparib-induced interstitial lung disease: A case series analysis.

Olaparib-induced interstitial lung disease (OILD) is a rare but potentially serious adverse event, and its imaging characteristics and clinical course remain unclear. Ishimoto et al. previously reported three cases of OILD, but further characterisation is needed. We present three additional cases, all presenting with fever and bilateral ground-glass opacities (GGO) and fine reticulonodular opacities on computed tomography (CT). Bronchoalveolar lavage fluid (BALF) analysis showed marked lymphocytosis (>75 %). Prednisolone (0.5-1 mg/kg/day) was effective without fibrosis. BALF lymphocytosis may

Lymphocytic and granulomatous cutaneous small-vessel vasculitis after polyadenosine diphosphate-ribose polymerase inhibitor therapy.

Olaparib is a polyadenosine diphosphate-ribose polymerase (PARP) inhibitor that is used in the treatment of advanced ovarian cancer. PARP inhibitors result in the accumulation of single-strand DNA breaks that are toxic to tumor cells. PARP inhibition with a concomitant Breast cancer susceptibility gene (BRCA) mutation results in what is known as synthetic lethality to the tumor cells. Cutaneous adverse effects have been reported with PARP inhibition, namely pruritus, photosensitivity reactions, edema, vasculitis, and panniculitis. We present a case of a 36-year-old woman who developed dermatit
