Improving Outcomes in Patients With Refractory Idiopathic and Neurogenic Detrusor Overactivity: Management Strategies.

Neurogenic detrusor overactivity (NDO) is a lower urinary tract dysfunction commonly seen in rehabilitation settings. The emotional, medical, and financial consequences of NDO can be substantial and management typically requires a multidisciplinary team approach. Physiatrists need to be able to identify patients who require referral to specialists for diagnostic testing or higher-tiered treatment and need to engender open lines of communication between their patients and all treating clinicians. This requires an understanding of the evaluation, diagnosis, and treatment of neurogenic lower urin

Obstructive Sleep Apnea in Stroke Rehabilitation: A Physical Medicine and Rehabilitation Perspective.

Obstructive sleep apnea (OSA) affects up to 70% of stroke survivors and is independently associated with poorer functional outcomes, stroke recurrence, and elevated mortality, yet remains systematically underdiagnosed in rehabilitation settings. This narrative review, conducted following SANRA guidelines using PubMed, Embase, CINAHL, and Web of Science through March 2026, examines the mechanisms by which OSA impairs stroke recovery and evaluates treatment evidence within the rehabilitation context. OSA disrupts neuroplasticity through intermittent hypoxia, fragments sleep architecture critical

Resource utilization for non-operative cervical radiculopathy: Management by surgeons versus non-surgeons.

OBJECTIVES: To compare the estimated resource utilization for non-operative treatment of cervical radiculopathy if managed by surgeons versus non-surgeons. PATIENTS AND METHODS: A Cervical Spine Research Society-sponsored survey was administered at a national spine surgery conference to surgeons and non-surgeons, as classified above. The survey asked questions regarding resource utilization and perceived costs for the "average patient" with cervical radiculopathy managed non-operatively. Resource utilization and perceived costs were compared between surgeon and non-surgeon participants, and b

Diagnosis of a C-2 fracture during physiatric consultation: a case report.

Consultation in the acute care setting allows physiatrists to make significant contributions. In our case, a 90-year-old woman presented to the emergency room after falling down five stairs. Six view C-spine films taken at that time revealed no fracture. She was sent home on anti-inflammatory medication and was later admitted for gastritis. A repeat six-view C-spine series again revealed no fracture. Physiatry was then consulted to prescribe modalities and therapies for "cervical strain." On examination, the patient demonstrated severely decreased neck range of motion limited by pain, with pai

A general rehabilitation inpatient with exercise-induced vasculitis.

While on our general inpatient rehabilitation floor, a 58-year-old man with no hematologic or dermatologic history developed an erythematous patch on his medial ankle that turned more purpuric, with a slight orange tint, and was associated with mild pruritus. The diagnosis of exercise-induced vasculitis was made after initially being mistaken for cellulitis. This common exanthem is often misdiagnosed. Due to its association with exercise, the physiatrist should be aware of its presence in both the inpatient and outpatient settings.

Rehabilitation and medical management of the adult with spina bifida.

.As the life expectancy of individuals with spina bifida increases, a lifelong need for management of many health issues in a rehabilitation setting has emerged in recent years. Physiatrists, in consultation with a variety of adult specialists, are particularly well suited to manage the common musculoskeletal, skin, bowel, bladder, renal, neurological, and other issues that arise in the adult population. This article reviews the last 20 yrs of literature pertinent to the rehabilitative care of this population, summarizes current evidence-based practice, and identifies key areas in which scient

[Belated diagnosis of medullar compression in a case of post-polio syndrome].

The physiatrist observes about his practice individuals with sequela of old poliomyelitics. A part of them have unusual fatigue and muscular pains and weakness. The hypothesis of an evolution of neuro-biological mechanism suggested by few authors isn't, actually, demonstrated. More probably, the modifications of lesional and, functional changes with disability observed are the consequence of elderly effects and decreasing of physical activites. We report a case of spinal cord compression by intramedullar tumor, associated with a post-polio syndrome.

Survey of Canadian Physician Use of Botulinum Toxin A (BoNT-A) for Paratonia: Real World Insights.

Paratonia is a form of hypertonia characterized by an inability to relax muscles in the setting of cognitive impairment. Paratonia results in pain, refusal of care, and caregiver burden. We sent surveys to 67 Canadian physiatrists and neurologists regarding their experience treating paratonia with botulinum toxin A (BoNT-A). Twenty-seven survey respondents were included in the analysis. Thirteen percent of survey respondents treating paratonia with BoNT-A reported a significant clinically relevant improvement; 74% endorsed a moderately clinically relevant improvement; 13% endorsed a slight cli

Palliative Phase of Cancer Rehabilitation.

Palliative cancer rehabilitation plays a critical role in optimizing function, alleviating symptom burden, and enhancing quality of life for individuals with serious or terminal illness. Despite its benefits, rehabilitation and other supportive care services remain underutilized, often due to the lack of ongoing need assessments throughout the survivorship continuum. Physiatrists are uniquely positioned to lead palliative rehabilitation across diverse care settings, using a multidisciplinary approach to address physical decline, psychosocial distress, and caregiver burden. These interventions

Ultrasound-guided piriformis injection: technique description and verification.

Piriformis injections are commonly used in the evaluation and treatment of patients presenting with buttock pain syndromes. Because of its small size, deep location, and relation to adjacent neurovascular structures, the piriformis is traditionally injected by using electromyographic, fluoroscopic, computed tomographic, or magnetic resonance imaging guidance. This report describes and verifies a technique for performing ultrasound-guided piriformis injections. Ultrasound offers several advantages over traditional imaging approaches, including accessibility, compact size, lack of ionizing radia

Physiatric management of two neonates with limb deficiencies and prenatal cocaine exposure.

We evaluated two neonates born within nine weeks of each other who had limb malformations and whose urine toxicology screenings were positive for cocaine metabolites and other drugs. Review of the literature indicates that fetal exposure to cocaine could lead to limb reduction deficits. The purpose of this report is to alert the physiatrist to a possible increase in incidence of limb malformation related to maternal cocaine use during pregnancy. The two cases are detailed, etiology is discussed, and the initial physiatric management is explained.

Occurrence of breast-cancer-related lymphedema after reverse lymphatic mapping and selective axillary dissection versus standard surgical treatment of axilla: A two-arm randomized clinical trial.

BACKGROUND: The need for axillary dissection (AD) is declining, but it is still essential for many patients with nodal involvement who risk developing breast-cancer-related lymphedema (BCRL) with lifelong consequences. Previous nonrandomized studies found axillary reverse mapping and selective axillary dissection (ARM-SAD) a safe and feasible way to preserve the arm's lymphatic drainage. METHODS: The present two-arm prospective randomized clinical trial was held at a single comprehensive cancer center to ascertain whether ARM-SAD can reduce the risk of BCRL, compared with standard AD, in pati

Optimizing Perioperative Outcomes in Liver Transplantation: The Role of Inpatient Rehabilitation.

Liver transplantation is the definitive treatment for individuals with end-stage liver disease. However, a range of medical and physical complications frequently arise following transplantation, which can hinder a patient's overall recovery. Post-operative rehabilitation is often necessary to support and enhance functional outcomes. This review explores the role of early mobilization and inpatient rehabilitation, outlines the rehabilitation process, defines the role of the physiatrist and the interdisciplinary rehabilitation team, and examines the potential functional gains for liver transplan

Limbs lost, lives changed: The unprecedented limb amputation crisis in Gaza.

Historically in armed conflicts, traumatic amputations result in long-term impacts on individuals, healthcare systems, and society. Though amputations are not new, the ongoing conflict in Gaza has resulted in an unusually high number of amputations. The scale of conflict-related amputations in Gaza, accompanied by systemic barriers to acute and rehabilitative care, necessitates feasible rehabilitation-focused responses relevant to physiatrists and global health stakeholders. The World Health Organization documentation reflects between 3105 and 4050 amputations between January 10 and May 16, 20
