Effects of physical therapy with neuromuscular electrical stimulation in acute and late septic shock patients: A randomised crossover clinical trial.

BACKGROUND: Patients with sepsis and immobility in the intensive care unit are associated with muscle weakness, and early mobilisation can counteract it. However, during septic shock, mobilisation is often delayed due to the severity of the illness. Neuromuscular electrical stimulation (NMES) may be an alternative to mobilise these patients early. This study aims to identify whether NMES performed within the first 72 hours of septic shock diagnosis or later is safe from a metabolic perspective. METHODS: This is the analysis of two randomised controlled crossover studies. Patients with acute s

Effects of chronic cholinergic stimulation associated with aerobic physical training on cardiac morphofunctional and autonomic parameters in spontaneously hypertensive rats.

We investigated hemodynamic, cardiac morphofunctional, and cardiovascular autonomic adaptations in spontaneously hypertensive rats (SHRs) after aerobic physical training associated with chronic cholinergic stimulation. Fifty-four SHRs were divided into two groups: trained and untrained. Each group was further subdivided into three smaller groups: vehicle, treated with pyridostigmine bromide at 5 mg/kg/day, and treated with pyridostigmine bromide at 15 mg/kg/day. The following protocols were assessed: echocardiography, autonomic double pharmacological blockade, heart rate variability (HRV), blo

Transcranial direct current stimulation (tDCS) for improving activities of daily living, and physical and cognitive functioning, in people after stroke.

BACKGROUND: Stroke is one of the leading causes of disability worldwide. Functional impairment, resulting in poor performance in activities of daily living (ADL) among stroke survivors is common. Current rehabilitation approaches have limited effectiveness in improving ADL performance, function, muscle strength, and cognitive abilities (including spatial neglect) after stroke, with improving cognition being the number one research priority in this field. A possible adjunct to stroke rehabilitation might be non-invasive brain stimulation by transcranial direct current stimulation (tDCS) to modu

Testing the role of cognitive inhibition in physical endurance using high-definition transcranial direct current stimulation over the prefrontal cortex.

The aim of this study was to clarify the role of the prefrontal cortex (PFC) in physical effort regulation. We hypothesized that the PFC would be progressively involved in physical endurance through the engagement of cognitive inhibition, which would be necessary to maintain effort by inhibiting fatigue-related cues. This hypothesis was examined using a double-blind, sham-controlled, within-subjects study (N = 20) using high-definition (HD) transcranial direct current stimulation (tDCS) over the right dorsolateral prefrontal cortex (dlPFC). Participants had to maintain a knee extensor contract

Adenosine A3 receptor stimulation reduces muscle injury following physical trauma and is associated with alterations in the MMP/TIMP response.

We have previously demonstrated that in response to traumatic injury in skeletal muscle, there is a dysregulation of the matrix metalloproteases (MMPs) and their inhibitors (TIMPs), a response hypothesized to interfere with proper skeletal muscle regeneration. Moreover, we have shown that pharmacological activation of the adenosine A(3) receptor by Cl-IBMECA in skeletal muscle can protect against ischemia-reperfusion and eccentric exercise injury. However, the mechanism by which Cl-IBMECA protects muscle tissue is poorly defined. This study evaluated the effects of Cl-IBMECA on MMP/TIMP expres

Transcranial direct current stimulation combined with physical exercise in rotator cuff tendinopathy: a protocol for a blinded randomised controlled trial.

INTRODUCTION: Pain is one of the primary symptoms affecting individuals with rotator cuff tendinopathy. Physical exercise serves as the main approach for managing this condition, aiming to restore movement patterns, optimise scapular kinematics and improve strength and local muscular endurance. Pain neuromodulation techniques, such as transcranial direct current stimulation (tDCS), may offer complementary non-pharmacological options for pain relief by promoting central modulation and altering cortical excitability. METHODS AND ANALYSIS: This protocol outlines a randomised, blinded clinical tr

Effects of electrical muscle stimulation on core muscle activation and physical performance in non-athletic adults: A randomized controlled trial.

BACKGROUND: Electrical muscle stimulation (EMS) activates muscles through electrical currents, resulting in involuntary muscle contractions. This study aimed to evaluate the immediate clinical effects of superimposing EMS on strength training compared with conventional exercise in healthy non-athletic adults. METHODS: This study was a randomised, controlled, parallel-group trial conducted at a single centre. Forty-one healthy young volunteers were recruited and randomised into two groups: strengthening with superimposed EMS (S+E) and strengthening (S) groups. All participants underwent the 30

Physical Therapy Combined with Transcranial Magnetic Stimulation Therapy: Treatment Practice Considering the Effect of Reducing Upper Limb Spasticity on Gait.

We perform physical therapy combined with repetitive transcranial magnetic stimulation (rTMS) in stroke patients with hemiplegia in the maintenance phase with the intent of improving the support of paralyzed leg. In gait evaluation in patients with hemiplegia, it is important to assess elements related to coordination carefully. rTMS therapy is effective in alleviating the tension of upper limbs. As rTMS helps upper-limb swing to become evident during gait, it makes trunk rotation necessary for left-right coordination appear more easily. As a result, rTMS has potential for improved upper-limb

Zapping the brain to enhance sport performance? An umbrella review of the effect of transcranial direct current stimulation on physical performance.

Concepts such as "neurodoping" have contributed to an expansion in the area of transcranial direct current stimulation (tDCS) and its impact over physical performance in recent years. This umbrella review examines meta-analyses to evaluate tDCS's impact on exercise performance in healthy individuals. We identified 9 meta-analyses that met our inclusion criteria, encompassing 50 crossover studies and 683 participants. Like previous meta-analyses, we found a small but significant effect across individual studies (gz = 0.28, 95%CI [0.18, 0.39]). However, we also found clear evidence of publicatio

How social is the cerebellum? Exploring the effects of cerebellar transcranial direct current stimulation on the prediction of social and physical events.

Congenital or acquired cerebellum alterations are associated with a complex pattern of motor, cognitive and social disorders. These disturbances may reflect the involvement of the cerebellum in generating and updating the internal models that sub-serve-the prediction of sensory events. Here, we tested whether the cerebellar involvement in using contextual expectations to interpret ambiguous sensory sceneries is specific for social actions or also extends to physical events. We applied anodic, cathodic and sham cerebellar transcranial Direct Current Stimulation (ctDCS) to modulate the performan

Longitudinal changes in drug use severity and physical health-related quality of life among untreated stimulant users.

The primary objective of this study was to investigate whether drug use severity is associated with physical health-related quality of life (HRQL) over time. Data are from a longitudinal, multi-state, natural history community study of users of cocaine and/or methamphetamine who were interviewed at 6-month intervals over 2 years with a 79% follow-up participation rate. Physical HRQL was assessed with the physical component summary (PCS) of the SF-8 Health Survey and drug, alcohol, and psychiatric severity were all assessed with the Addiction Severity Index (ASI). Random coefficient regression

Investigating the transfer of physical stimuli to the dreamed body: A comparative study of muscular, vestibular, and haptic stimulation on dream incorporation.

The relationship between the dreamed body and the physical body remains an open question in sleep and dream research. This study examined whether somatosensory stimulation applied during REM sleep modulates the corresponding dream content under controlled conditions. Using a within-subject design, we compared three stimulation modalities-electrical muscular, galvanic vestibular, and haptic vibration-each paired with a modality-specific sham stimulation. Twenty-four participants completed one adaptation night followed by three consecutive experimental nights in the sleep laboratory. Dream repor

The effect of physical activity on Parkinson's disease outcomes in patients with bilateral subthalamic nucleus deep brain stimulation: a systematic review.

BACKGROUND: Subthalamic nucleus deep brain stimulation (DBS-STN) is a recognized treatment for advanced Parkinson's disease. While it improves some clinical symptoms, especially bradykinesia and rigidity, it has limited application for late motor symptoms. Moreover stimulation-related motor adverse effects can increase the motor burden. There is a clear need to develop additional therapies that complement deep brain stimulation in controlling late motor symptoms. Physical activity is a commonly used and recommended form of therapy for patients with Parkinson's disease, supplementary to pharmac

Effects of transcranial direct current stimulation on mental fatigue reduction and physical performance improvement in healthy adults: a systematic review protocol.

BACKGROUND: Mental fatigue (MF) is a psychobiological state that impairs physical and cognitive performance, particularly in endurance and resistance tasks. Transcranial direct current stimulation (tDCS) has emerged as a promising noninvasive neuromodulation technique to mitigate MF and increase exercise capacity. However, evidence remains inconsistent due to methodological heterogeneity in stimulation parameters, fatigue induction protocols, and outcome assessments. This research aims to systematically review randomized controlled trials (RCTs) investigating the effects of active versus sham

How does combining physical therapy with transcranial direct stimulation improve upper-limb motor functions in patients with stroke? A theory perspective.

More than half of stroke survivors suffer from upper-limb dysfunction that persists years after stroke, negatively impacting patients' independence and, therefore, affecting their quality of life. Intense motor rehabilitation is required after a stroke to facilitate motor recovery. More importantly, finding new ways to maximize patients' motor recovery is a core goal of stroke rehabilitation. Thus, researchers have explored the potential benefits of combining the effects of non-invasive brain stimulation with physical therapy rehabilitation. Specifically, combining transcranial direct stimulat
