Determinants of guideline-concordant breast cancer screening by family physicians for women aged 40-49 years: a qualitative analysis.

BACKGROUND: Although the current Canadian Task Force on Preventive Health Care guideline recommends that physicians should inform women aged 40-49 years of the potential benefits and harms of screening mammography to support individualized decisions, previous reports of variation in clinical practice at the physician level suggest a lack of guideline-concordant care. We explored determinants (barriers and facilitators) of guideline-concordant care by family physicians regarding screening mammography in this age group. METHODS: We conducted qualitative semi-structured interviews by phone with

Human papillomavirus (HPV) testing for normal cervical cytology in low-risk women aged 30-65 years by family physicians.

PURPOSE: The purpose of this study was to assess ordering of human papillomavirus (HPV) testing for normal cervical cytology among low-risk women aged 30 to 65 years. METHODS: Audits of 833 cytology request forms for low-risk women completing a Papanicolaou smear, from January 2008 to April 2011, from 5 Michigan family medicine clinics determined HPV orders completed by the clinician performing the Papanicolaou smear. Multivariate logistic regression models examined differences in HPV test ordering by patient age at Papanicolaou test, provider status and sex, and clinic across sites. A Poisso

Physician counseling on hormone replacement therapy and bone loss: do socioeconomic and racial characteristics of women influence counseling?

BACKGROUND: Although women consider multiple issues when deciding to take hormone replacement therapy (HRT), both women's use and physician counseling about HRT are strongly associated with sociodemographic factors. As prevention of bone loss is the best established long-term benefit of HRT, we sought to determine (1) if counseling about bone loss is included in discussions of HRT and (2) if sociodemographic factors affect the preventive content of HRT counseling. METHODS: We evaluated reports of counseling on estrogen and bone loss from 1404 women aged 40-60 who reported any counseling on HR

The behavior of Italian family physicians regarding the health problems of women and, in particular, family planning (both contraception and NFP).

The hospital-centered trend that has dominated medical culture and the management of health care during this entire century has, in the last few years, undergone a reversal in Italy. Conditions in other countries suggest that similar changes have or will become increasingly common. The family physician today manages many of the functions previously handled by hospitals and specialists. In the field of reproductive health, family physicians are responsible not only for diagnosis and treatment, but also for prevention and education. The present study considers this new context with the objective

Impact of a mailed intervention on annual mammography and physician breast examinations among women at high risk of breast cancer.

Only a few studies have examined repeat annual mammography rates, and most studies find that such regularity is low, ranging from < 5% in the general population to between 14 and 20% of first-degree relatives. The present study tested the effectiveness of a mailed intervention designed to improve compliance with breast cancer screening guidelines among women at elevated familial risk. The study used a pretest-posttest control group design; 369 twin sisters of breast cancer cases were assigned alternately to an intervention or a control group on the basis of sequential registration numbers. A case of maternal varicella: Expected role of primary care physicians in confirming immune status for varicella in women at childbearing age.

At 19-week gestation, a 32-year-old Japanese woman presented with a 3-day history of fever and vesicular rashes on the skin and was diagnosed with varicella, which resolved after antiviral therapy. In the primary clinic, her immunity to rubella and measles, but not to varicella, was confirmed at preconception counseling. Maternal varicella infection can cause congenital varicella syndrome characterized by congenital malformations and neurological deficits. This case recommends that all women of childbearing age should be assessed for immunity to varicella before pregnancy and that primary care
