Genetic basis of calcifying cystic odontogenic tumors.

Calcifying cystic odontogenic tumors (CCOTs) are benign cystic tumors that form abnormally keratinized ghost cells. Mutations in CTNNB1, which encodes beta-catenin, have been implicated in the development of these tumors, but a causal relationship has not been definitively established. Thus, mutational hot spots in 50 cancer genes were examined by targeted next-generation sequencing in 11 samples of CCOT. Mutations in CTNNB1, but not in other genes, were observed in 10 of 11 cases. These mutations constitutively activate beta-catenin signaling by abolishing the phosphorylation sites Asp32, Ser

Aspiration biopsy cytology of primary cutaneous tumors.

Primary cutaneous neoplasms may undergo fine needle aspiration during the workup of patients with suspected metastatic disease. Such aspirates present a wide range of epithelial and nonepithelial proliferations that must be separated from more common metastatic deposits. The majority of primary cutaneous tumors are composed of squamous or basaloid cells that can be confused with metastatic squamous cell carcinoma. In addition, primary cutaneous lymphomas and mesenchymal tumors morphologically overlap their more common systemic or metastatic counterparts in appearance. Both clinical and cytolog

Congenital lesions of epithelial origin.

Defects of embryologic development give rise to a variety of congenital lesions arising from the epithelium and are among the most common congenital lesions of the head and neck in the pediatric population. This article presents several congenital lesions of epithelial origin, including congenital midline cervical cleft, pilomatrixoma, dermoid, foregut duplication cysts, and preauricular sinuses and pits. In addition, the management of these lesions is reviewed.

Nasal hair matrix cyst mimicking dermoid cyst.

Dermoid cysts are primarily observed in the head and neck region. Nasal dermoids are uncommon. The terms nasal dermoid, nasal epidermoid, and nasal sebaceous cysts are used interchangeably by clinicians. Hair matrix cyst is a variant of epidermal cyst resembling the pattern of pilomatrixoma. Formerly, nasal hair matrix cyst has not been reported at all. Here, an additional pathologic proof is presented as well.

A newly growing asymptomatic facial lesion.

A 23-year-old man presenting with a 1-year history of a lesion of the right cheek. We highlight this case for awareness as this tumour may mimic other benign lesions, such as pilomatrixoma or benign cysts, as it does not have any uniquely identifying clinical or dermoscopic features. Additionally, it is of concern as malignant transformation can occur and therefore surgery should be considered as both for diagnostic and therapeutic benefit.

Subcutaneous dermoid cysts on the eyebrow and neck.

We report a case of dermoid cysts on the right lateral eyebrow and anterior neck. Multiple concurrent dermoid cysts, as in the present case, are very rare. The differential diagnosis of dermoid cyst includes epidermoid (epidermal inclusion) cyst, trichilemmal cyst, pilomatrixoma, lymphatic malformation, and lipoma. In particular, thyroglossal duct cyst and midline anterior neck inclusion cyst are part of the differential diagnosis when the lesion is in the anterior neck.

Occurrence and characteristics of head cysts in children.

BACKGROUND: Lumps arising in the head and neck constitute an important diagnostic category in children. As malignancy in this age group is relatively rare, lumps that are not due to inflammatory or infective causes often prove to be cysts. Correct diagnoses of the different subcutaneous lumps are often missed because of the lack of recognition or uncertainty with management. OBJECTIVES: The purpose of this study was to review the characteristics of most common pediatric cysts appearing in the head. METHODS: A retrospective study was designed to review all the children treated for a cyst in t

Unusual multiple pilomatrixomata: case report and review of the literature.

Pilomatrixomata are rare, benign, calcifying cutaneous tumours of the hair matrix cells. They usually present in childhood with a pre-surgical diagnosis of sebaceous cyst, because of their size and appearance. The usual sites of occurrence are the scalp, head and neck. Treatment is usually surgical excision. We report a case of unusual, multiple, benign pilomatrixomata in an Indian adult. The plastic surgeon should be aware of the various guises of pilomatrixoma and be alert to the important, although rare, occurrence of malignancy.

Primary mucinous carcinoma of the skin arising from the upper eyelid: A case report and literature review.

Primary mucinous carcinoma of the skin (PMCS) is a rare malignant neoplasm of the sweat glands that has an incidence of 1 per 150,000 population. Because of the lack of typical characteristics, it is often misdiagnosed as an epidermoid cyst, pilomatrixoma, or chalazion before resection, with subsequent enucleation performed unintentionally. We present a case of a 51-year-old patient with PMCS in the upper eyelid that was successfully treated at our hospital. Additionally, we reviewed the literature and discussed the diagnosis, primary and adjuvant therapy, and follow-up procedure.

Rare and unusual benign tumors of the sinonasal tract and pharynx: Case series and literature review.

There are a number of benign tumors of the nose and pharynx that are seldomly reported in literature but that can sometimes prove difficult to treat and extremely important for differential diagnosis. The present study presents cases of rare benign tumors localized in the pharynx, nasal and sinus cavities, as well as reviews of literature and historical references for each type of tumor. Unilateral nasal hemangioma in a 72-year-old male which, although not a rare pathology, raised problems due to auto-resection of the tumor. The surgeon was able to pull it out with ease without bleeding; it is

Keratinous cyst of the palpebral conjunctiva.

A whitish-opalescent, mildly elevated superior tarsal conjunctival lesion measuring 3.0 mm in diameter caused a refractory corneal abrasion in a 54-year-old man. Complete local excision without entering the tarsus produced relief of symptoms. There has been no recurrence during 9 months of follow-up. Histopathologically, a unique keratinous cyst was delimited mostly by basaloid cells resembling the matrical cells in pilomatrixoma. There were no keratohyalin granules in the lining cells, which focally transformed in ghost cells. These histopathologic features are characteristic of trichilemmal

Cytodiagnosis of lesions presenting as salivary gland swellings: a report of seven cases.

The cytologic and histologic findings of seven cases presenting clinically as salivary gland swellings are described. These included two cases of pilomatrixoma of skin in the parotid region, two cases of neurilemmoma in the submandibular area, a case of Kimura's disease in the peri-mandibular gland lymphoid tissue, a case of Castleman's disease in intraparotid lymph node, and a case of branchial cleft cyst in the parotid region. It is important to recognize lesions that masquerade as salivary gland tumors so that misdiagnosis and overdiagnosis on smears can be avoided and the patients can be t

A patient with pilomatricoma in the parotid region: case report and review of the literature.

Pilomatricoma is a benign skin tumor originating from the matrix cells of the hair follicles. Sometimes, its diagnosis can be difficult, especially in the preauricular region, where in the differential diagnosis, in addition to other dermal and subcutaneous masses, primary and secondary parotid gland tumor lesions must be also considered. A 34-year-old female was referred to our Institution with a right preauricular swelling over 12 months, which enlarged in the last two months. The ultrasonography confirms the origin of the tumoral mass in the skin of the preauricular region and not from the

Metachronous T-Lymphoblastic Lymphoma and Burkitt Lymphoma in a Child With Constitutional Mismatch Repair Deficiency Syndrome.

Constitutional mismatch repair deficiency (CMMRD) is a cancer predisposition syndrome associated with a high risk of developing early-onset malignancies of the blood, brain, and intestinal tract. We present the case of a patient with T-lymphoblastic lymphoma at the age of 3 years, followed by Burkitt lymphoma 10 years later. This patient also exhibited numerous nonmalignant findings including café au lait spots, lipomas, bilateral renal nodules, a nonossifying fibroma, multiple colonic adenomas, and a rapidly enlarging pilomatrixoma. The spectrum of malignant and nonmalignant neoplasms in this

Topical Tacrolimus as a Novel Treatment for Refractory Idiopathic Facial Aseptic Granuloma in a Pediatric Patient.

This report describes the case of a four-year-old girl with idiopathic facial aseptic granuloma (IFAG) who developed persistent, treatment-resistant eyelid and facial inflammation despite multiple courses of antibiotics and corticosteroids. Her presentation included recurrent hordeola, severe blepharitis, and a cheek nodule, with a clinical picture more consistent with IFAG than pilomatrixoma despite histologic evidence of the latter. Standard therapies provided only transient benefit and were limited by systemic side effects. Initiation of topical tacrolimus 0.03% ointment led to rapid and su

Adnexal neoplasms of the eye.

Adnexal neoplasms of the eyelid encompass a wide variety of benign and malignant tumors of sebaceous, follicular, and sweat gland origin. Due to the specialized structures of the eyelid, these neoplasms present differently when compared with those of other locations. Although most dermatologists and ophthalmologists are familiar with the commonly reported adnexal tumors of the eyelid, such as hidrocystoma, pilomatrixoma, and sebaceous carcinoma, many other adnexal neoplasms have been reported at this unique anatomic site. Accurate and timely identification of these neoplasms is essential, as a

Giant molluscum contagiosum presenting as lid neoplasm in an immunocompetent child.

A three-year-old boy presented to our oculoplastic clinic with complaints of painless gradually increasing right upper lid mass for the last 6 months. On examination a firm mass measuring roughly 1x1 cm was present on the upper lid. The mass was non tender with fine superficial vessels running over it. A differential diagnosis of epidermoid cyst, vascular malformation, pilomatrixoma, and juvenile xanthogranuloma was considered. The patient underwent excisional biopsy of the mass. On gross examination the mass had a brain like appearance. Histopathological examination confirmed the diagnosis of

Multiple pilomatricomas in twins with Rubinstein-Taybi syndrome.

Pilomatricomas are benign tumors originating from the capillary matrix, which may present as solitary lesions or, less commonly, multiple. Myotonic dystrophy and familial adenomatous polyposis are the most frequently associated disorders with multiple pilomatricomas. There are few reports relating these tumors to other genetic syndromes. Rubinstein-Taybi syndrome is a rare autosomal dominant disorder characterized by intellectual disability and typical dysmorphic characteristics. There are five case reports relating to multiple pilomatricoma to Rubinstein-Taybi syndrome, an association that ne

Odontogenic ghost cell tumor: a case report with cytologic findings.

Odontogenic ghost cell tumor is a rare, neoplastic form of calcifying odontogenic cyst (Gorlin cyst) whose cytologic features have not been previously reported. We present a case of odontogenic ghost cell tumor diagnosed by fine-needle aspiration biopsy (FNAB). The aspirate was characterized by (1) tissue fragments with basaloid epithelial cells, (2) "ghost" cells, (3) scattered multinucleated giant cells, (4) rare, eosinophilic, densely hyalinized "dentinoid" material in close association with the basaloid cells, and (5) calcific debris.
