Treatment of Nonreplantable Total Thumb Amputation at the CMC Level Using Index Finger Pollicization.

BACKGROUND: Pollicization of the index finger is a rarely performed reconstructive option for patients with total thumb amputations with nonsalvageable carpometacarpal (CMC) joint and thenar muscles. Successful pollicization can provide basic grasp and pinch to help patients carry out activities of daily living. We present a retrospective review of 4 patients who underwent index finger pollicization for traumatic total thumb amputations. METHODS: A retrospective review of 4 cases of pollicization using an injured index finger for traumatic thumb amputation was performed. Patients available fo

Evaluation and functional assessment of flexor tendon repair in the hand.

Seventeen patients with 28 flexor tendon injuries were examined after tendon repair. The current most frequently used evaluation systems, including grip and pinch strength, were compared with functional outcome as assessed by a questionnaire, evaluating Disabilities of Arm, Shoulder and Hand (DASH). Good correlation was found between Total Active Motion (TAM) and the Original Strickland test (kappa = 0.85), however with reduced categories. Only limited correlation was found between the DASH-score and TAM (r = -0.33) as well as between the DASH-score and pinch strength (r = -0.35). We suggest r

Hand strengths in carpal tunnel syndrome.

The purpose of this study was to determine whether a measurable decrease in isokinetic (dynamic) and isometric (static) hand strengths occurs in carpal tunnel syndrome (CTS) patients. Eighteen CTS patients and 20 healthy controls were included in the study. Isokinetic (eccentric and concentric) and isometric grip and pinch strengths were measured with a Biodex System 3 dynamometer (Biodex Medical System, Inc. New York). All strength measurements, except isometric and isokinetic (concentric/eccentric) three-point pinch and isokinetic (concentric) tip pinch, revealed statistically significant di

Prognostic indicators of postoperative objective improvement in carpal tunnel syndrome: A retrospective study.

BACKGROUND: Carpal tunnel syndrome (CTS) is the most common peripheral neuropathy affecting the upper limb, with surgical intervention as the primary treatment. This study aims to identify predictive factors influencing postoperative motor and sensory recovery following carpal tunnel release (CTR). METHODS: A retrospective analysis was conducted on 95 patients (117 hands), with objective outcomes measured by tip pinch strength and two-point discrimination (2PD) six months postoperatively. Motor recovery was defined as the recovery rate of tip pinch strength ≥ 1.2, while sensory recovery was d

Precision pinch performance in patients with sensory deficits of the median nerve at the carpal tunnel.

To investigate how sensory symptoms impact the motor control of hands, in this study we examined the differences in conventional sensibility assessments and pinch force control in the pinch-holding-up activity (PHUA) test between carpal tunnel syndrome (CTS) patients and healthy controls. CTS patients (n = 82) with 122 affected hands and an equal number of control subjects were recruited to participate in the threshold, discrimination, and PHUA tests. The patients showed significantly poorer hand sensibility and lower efficiency of force adjustment in the PHUA test as compared with the control

Text neck is not associated with neck and upper limb disability in young adults who use smartphones.

PURPOSE: The objective of the present study was to correlate neck and upper-limb disabilities with neck flexion in university students who are smartphone users. While handling smartphones, a posture with sustained neck flexion known as "Text Neck" is usually adopted, and some authors related to musculoskeletal symptoms on the neck and the upper limb. However, recent studies state that such an association is questionable. METHODS: This is an observational study when evaluating 192 university students through questionnaires such as the Neck Disability Index (NDI), Disabilities of Arm, Shoulder,

Concomitant endoscopic carpal and cubital tunnel release: safety and efficacy.

BACKGROUND: When performed alone, endoscopic carpal tunnel release and endoscopic cubital tunnel release are safe and effective surgical options for the treatment of carpal and cubital tunnel syndromes, respectively. However, there is currently no literature that describes the performance of both procedures concomitantly. We describe the results of 17 cases in which dual endoscopic carpal and cubital tunnel releases were performed for the treatment of concurrent carpal and cubital tunnel syndromes. METHODS: A retrospective review of all patients in a single surgeon practice that presented wit

The Efficacy of 100 and 300 mg Gabapentin in the Treatment of Carpal Tunnel Syndrome.

Carpal tunnel syndrome (CTS) is a neuropathy due to the compression of the median nerve. It is shown that gabapentin in high doses is effective in treatment of CTS patients. In this study we evaluated the efficacy of low doses of gabapentin in treatment of CTS patients. Ninety patients with CTS were randomly assigned to groups A, B and C. Gabapentin was administered to group A with dose of 100 mg/day and to group B with dose of 300 mg/day for 2 months. Group C received no treatment. Before and after treatment, patients were evaluated using Visual analogue scale (VAS) for pain and parasthesia,
