Laser resurfacing and remodeling of hypertrophic burn scars: the results of a large, prospective, before-after cohort study, with long-term follow-up.

OBJECTIVES: Hypertrophic burn scars produce significant morbidity, including itching, pain, stiffness, and contracture, but best management practices remain unclear. We present the largest study to date that examines long-term impact of laser therapies, a potentially transformative technology, on scar remodeling. METHODS: We conducted a prospective, before-after cohort study in burn patients with hypertrophic scars. Pulsed-dye laser was used for pruritus and erythema; fractional CO2 laser was used for stiffness and abnormal texture. Outcomes included (1) Vancouver Scar Scale (VSS), which docu

Association of TGFβ1 and clinical factors with scar outcome following melanoma excision.

All patients with primary cutaneous malignant melanoma undergo surgical excision to remove the tumour, resulting in scar formation. There is marked variation in the aesthetic appearance of scars following surgery but limited knowledge about the genetic factors affecting non-keloid, surgical scar outcomes. This study aimed to investigate the role of known clinical factors and genetic polymorphisms in pigmentation and wound repair genes in non-keloid scar outcome, following melanoma excision. Participants were 202 cases who underwent a standardized scar assessment following surgical melanoma exc

Toward predictable repair of regurgitant aortic valves: a systematic morphology-directed approach to bicommissural repair.

OBJECTIVES: Our purpose was to investigate a new approach to bicommissural repair of regurgitant aortic valves. BACKGROUND: Repair of regurgitant aortic valves is not widely accepted, but interest is increasing, particularly for bicuspid valves. We hypothesize that a systematic, segmental approach to morphology and corresponding morphology-directed repair will improve decision making and success. METHODS: From December 2001 to July 2007, a systematic surgical approach to valve analysis and bicommissural repair was applied prospectively to 63 consecutive patients with pure aortic valve regurg

Evaluating Skeletal Maturity at Time of Surgical Correction of Pectus Excavatum Based on Medial Clavicle Epiphyseal Ossification.

BACKGROUND: Surgical correction of pectus excavatum (SCOPE) is dependent upon chest wall pliability with optimal timing prior to complete skeletal maturation. Measures of skeletal maturity are not readily available for operative planning; therefore, surgeons use age as proxy despite patient-specific rates of skeletal maturation. We aimed to determine whether preoperative skeletal maturity is associated with postoperative pain as surrogate for chest wall pliability. METHODS: Children ≤18 years who underwent SCOPE from 2020 to 2022 were retrospectively identified. Preoperative CT within 3 month

Presumed acquired dynamic pectus excavatum in a cat.

An 11-year-old, spayed female American domestic shorthair cat was presented with open-mouth breathing and dyspnea due to respiratory tract disease. Radiographically, there was thickening of the retropharyngeal soft tissue and soft palate, focal pulmonary change, and severe pectus excavatum (PE), best seen on the left lateral view. Repeat thoracic radiographs 4 months later showed persistent PE, but only on the right lateral view; PE was absent on the left lateral view. The dynamic component of the observed PE was presumably acquired secondary to chronic upper respiratory disease and/or concurr

Efficacy and Safety of Pressure Therapy Alone and in Combination with Silicone in Prevention of Hypertrophic Scars: A Systematic Review with Meta-analysis of Randomized Controlled Trials.

BACKGROUND: At present, there are many kinds of hypertrophic scar treatment methods, among which pressure therapy and silicone therapy are very common and standard therapies, but whether they are used alone or in combination is still controversial. Therefore, the purpose of this systematic review was to compare the efficacy and safety of the combination of pressure therapy and silicone therapy (PTS) with pressure therapy alone (PT) in the treatment of hypertrophic scars to provide clinicians with information so that they can make better decisions. METHODS: Relevant randomized controlled trial

Intralesional Axitinib Injection Mitigates Hypertrophic Scar by Inhibiting Angiogenesis Pathway: A Preliminary Study in a Rabbit Ear Model.

OBJECTIVE: High levels of VEGF and excessive angiogenesis contribute significantly to hypertrophic scar (HS) formation. Our study aimed to preliminarily investigate the effect of axitinib, a selective VEGF receptor tyrosine kinase inhibitor, on angiogenesis of HS and to explore its possible mechanism in a rabbit ear model. METHODS: Ten male New Zealand white rabbits were used to establish HS models and then randomised to the control and axitinib groups. The scar tissues in the two groups were injected with axitinib or normal saline, and they were evaluated after one month of treatment. Macros

Physical and physiological effects of dobutamine stress echocardiography in low-gradient aortic stenosis.

Dobutamine stress echocardiography (DSE) is a useful tool for assessing low-gradient significant aortic stenosis (AS) and contractile reserve (CR), but its prognostic utility has become controversial in recent studies. We evaluated the impact of DSE on aortic valve physiological, structural, and left ventricular parameters in low-gradient AS. Consecutive patients undergoing DSE for low-gradient AS evaluation from September 2010 to July 2016 were retrospectively studied, and DSE findings were divided into four groups: with and without severe AS and/or CR. Relationships between left ventricular

Influence of dacron tissue thickness on the performance of the Pintucci biointegrable keratoprosthesis: an in vitro and in vivo study.

PURPOSE: In 1979 Pintucci developed a biointegrable keratoprosthesis with polymethylmethacrylate optical cylinder integrated with a Dacron tissue-colonizable supporting element to avoid the complications caused by the interaction between the haptic element and the eye. The purpose of this article is to compare the colonization of three Dacron fabrics (thicknesses of 0.25 mm, 0.6 mm, and 1.4 mm) in vitro and in vivo to optimize the device performance. METHODS: In vitro three different Dacron fabrics were cultured for 3 days with 3.5 x 10(5) human fetal lung fibroblasts and observed with a scan

All Organic Label-like Copper(II) Ions Fluorescent Film Sensors with High Sensitivity and Stretchability.

Deep learning and analysis of heavy metal concentration are very crucial to our life, for it plays an essential role in both environmental and human health. In this paper, we developed a new Cu (II) ions sensor made by all organic material with bending and stretching properties. The new sensor consists of chlorophyll-a extracted from fresh leaves of Common Garcinia, plant fiber and with the use of PDMS as a substrate. Fluorescence spectra study shows that chlorophyll-a is significantly much more sensitive to Cu (II) ions than any other heavy metal ions and the device sensitivity outperforms al

The effectiveness of whole-body-vibration training in improving hamstring flexibility in physically active adults.

CLINICAL SCENARIO: Hamstring tightness is common among physically active individuals. In addition to limiting range of motion and increasing the risk of muscle strain, hamstring tightness contributes to a variety of orthopedic conditions. Therefore, clinicians continue to identify effective methods to increase flexibility. Although hamstring tightness is typically treated with common stretching techniques such as static stretching and proprioceptive neuromuscular facilitation, it has been suggested that whole-body-vibration (WBV) training may improve hamstring flexibility. CLINICAL QUESTION:

The effects of ankle compliance and flexibility on ankle sprains.

PURPOSE: The goal of this study was to examine the influence of changes in subtalar joint flexibility and compliance on ankle sprain occurrence. METHODS: Muscle model driven simulations of 10 subjects performing the landing phase of a side-shuffle movement were performed. The passive flexibility or compliance of the subtalar joint was varied, and each subject-specific simulation was exposed to a set of perturbed floor conditions. RESULTS: Increases in flexibility and compliance both led to an increase in the occurrence of excessive supination, while changes in flexibility had a greater influ

Use of acellular intact fish skin grafts in treating acute paediatric wounds during the COVID-19 pandemic: a case series.

OBJECTIVE: More specific strategies are needed to support children requiring skin grafting. Our goal was to identify procedures that reduce operating times, post-operative complications, pain and length of hospital stay. Patient safety, optimal wound bed support and quick micro-debridement with locoregional anaesthesia were prioritised. Ultimately, a novel acellular fish skin graft (FSG) derived from north Atlantic cod was selected for use. METHOD: We admitted consecutive paediatric patients with various lesions requiring skin grafting for definitive wound closure. All FSGs were applied and b

New hope for scar revision with a modified skin culture technique: a case report.

Cultured epidermal autografts (CEA) are seldom used in scar revision despite evidence of favourable outcomes. We report a case using a modified technique, Kleintjes Cultured Skin (KCS), for revision of childhood burn scars on the lower legs. Keratinocytes were enzymatically isolated, seeded onto the dressing, and incubated for two weeks. KCS was applied directly onto the excised scars. Scar appearance, assessed with the Vancouver Scar Scale, improved from 4 preoperatively to 2 at a follow-up of two and six years, with progressive re-pigmentation, softness, and pliability. The biopsy site also

Other flaps for head and neck use: temporoparietal fascial free flap, lateral arm free flap, omental free flap.

Advances in microvascular surgery have led to the development of new donor sites for head and neck reconstruction. The superficial temporoparietal, lateral arm, and omental free flaps represent three flaps with primarily soft tissue uses. Their reliable blood supply and pliability make them well suited for a variety of head and neck defects. The superficial temporoparietal and lateral arm free flaps may be used as osteocutaneous flaps, but more rarely is this the case. Minimal donor site morbidity is an advantage with the superficial temporoparietal and lateral arm flaps. The morbidity of a la

Free Ulnar Forearm Flap: Design, Elevation, and Utility in Microvascular Nasal Lining Reconstruction.

Reconstruction of the nasal lining has traditionally been performed using the folded radial forearm free flap, given its reliable blood supply, pliability, and familiarity to surgeons with respect to its harvest. More recently, the free ulnar forearm flap has been proposed as an alternative reconstructive option for the nasal lining, with safe and reliable outcomes, and improved donor-site morbidity compared with its radial counterpart. In this article, the authors provide educational video footage with accompanying text description of the senior author's (E.D.R) approach to design and elevati
