Pneumococcal infection of a prosthetic hip joint.

A case of late haematogenous pneumococcal infection of a prosthetic hip joint is presented. In view of the relatively benign nature of pneumococcal arthritis, the patient was treated with parenteral antibiotics and surgical drainage without removal of the prosthesis. This was followed by complete resolution.

The risk of pneumococcal infections after immunization with pneumococcal conjugate vaccine compared to non-vaccinated inflammatory arthritis patients.

OBJECTIVES: To examine the risk of putative pneumococcal infections in adult arthritis patients on different anti-rheumatic drugs immunized with heptavalent pneumococcal conjugate vaccine (Prevenar 7; PCV7) and non-vaccinated individually matched arthritis patients. METHOD: All individuals in a cohort of 505 patients with rheumatoid arthritis (RA) or spondylarthropathy (SpA) receiving different anti-rheumatic treatments were immunized with a single dose of PCV7 (exposed group). Of these, 497 patients (RA = 248; SpA = 249) were included. For each vaccinated patient, we identified four referenc

Early trends for invasive pneumococcal infections in children after the introduction of the 13-valent pneumococcal conjugate vaccine.

BACKGROUND: The 13-valent pneumococcal conjugate vaccine (PCV13) was introduced for routine administration to infants and children in 2010 in the United States. We have monitored clinical and microbiologic features of invasive pneumococcal infections among children before and after PCV13 use. METHODS: Infants and children cared for at 8 children hospitals in the United States with culture-proven invasive infections caused by S. pneumoniae were identified in an ongoing prospective surveillance study. Demographic and clinical data were recorded on a standard case report form. Serotype and antim

Overview of antibody-mediated immunity to S. pneumoniae: pneumococcal infections, pneumococcal immunity assessment, and recommendations for IG product evaluation.

Streptococcus pneumoniae (S. pneumoniae) strains colonize the nasopharynx and can cause mucosal infections in the upper airway and middle ear, pneumonias, and invasive infections like bacteremia, sepsis, and meningitis. Over 90 serotypes, defined by the structure of their capsular polysaccharides, are known. Twenty-three of these serotypes cause most infections and several of these serotypes can develop antibiotic resistance. Susceptibility factors that increase the susceptibility to S. pneumoniae mucosal and invasive infections include all forms of primary and secondary antibody deficiencies.

Acute organ injury and long-term sequelae of severe pneumococcal infections.

Streptococcus pneumoniae (Spn) is a major public health problem, as it is a main cause of otitis media, community-acquired pneumonia, bacteremia, sepsis, and meningitis. Acute episodes of pneumococcal disease have been demonstrated to cause organ damage with lingering negative consequences. Cytotoxic products released by the bacterium, biomechanical and physiological stress resulting from infection, and the corresponding inflammatory response together contribute to organ damage accrued during infection. The collective result of this damage can be acutely life-threatening, but among survivors,

[Costs of pneumococcal infections and cost evaluation of heptavalent conjugate vaccine].

The results of six economic evaluations on heptavalent conjugate pneumococcal vaccination in children are discussed. Although the data of most studies come from estimates, the last study from US, in which data are based on a large randomized trial, shows encouraging results. Similar results are obtained in an economic evaluation performed in Germany, although the study is based on estimated data. The paper concludes stressing the need to improve epidemiological, clinical and economical knowledge on the pneumococcal diseases, in order to identify the best cost-effectiveness strategy to prevent

Repeated invasive pneumococcal infections in young children without apparent underlying immunodeficiency.

During a 30-month interval at LeBonheur Children's Medical Center, 394 patients had a blood or cerebrospinal fluid culture positive for Streptococcus pneumoniae. Sixteen of these episodes (4%) were repeated infections; 6 of these 16 patients had sickle cell disease. Six of the remaining 10 patients had immunologic evaluations of varying completeness; no immunodeficiency was identified by these tests or on follow-up. Nine of the ten previously healthy patients with repeated pneumococcal disease were less than 2 years of age. In our experience, repeated invasive pneumococcal infections in otherw

[Pneumococcal meningitis and ENT infections. Review of 100 cases (author's transl)].

One hundred cases of pneumococcal meningitis admitted to the Intensive Care Unit of the Infectious Diseases Department of the Hôpital Claude-Bernard, France are reviewed, and the course of this serious affection in this series described. Mortality was 38 p. cent as against that reported by INSERM of 45 p. cent. The original lesion was an ENT infection in 50 p. cent of cases, raising the problem of the need for early surgical intervention, on an otological focus which may appear minimal on otoscopic examination, when the meningitis does not respond to antibiotics.

Protection against pneumococcal infection elicited by immunization with multiple pneumococcal heat shock proteins.

Heat shock proteins (HSPs) play important roles in the pathogenesis of pneumococcal infection, and they are considered as potential protein vaccine antigens. In this study, we investigated the efficacy of immunization with pneumococcal HSPs, including ClpP (hsp100/Clp peptidase subunit), DnaJ (hsp40) and GroEL (hsp60), to protect against pneumococcal carriage, lung colonization and sepsis in mouse models using different serotypes of Streptococcus pneumoniae. In a nasopharyngeal colonization model by serotype 6B or 14 and in a lung colonization model by serotype 19F, immunization with pneumococ

The potential to use PspA and other pneumococcal proteins to elicit protection against pneumococcal infection.

Pneumococcal proteins, alone, in combination with each other, or in combination with capsular polysaccharide-protein conjugates may be useful pneumococcal vaccine components. Four proteins with a potential for use in vaccines are PspA, pneumolysin, PsaA, and PspC. In a mouse model of carriage, PsaA and PspC were the most efficacious vaccine proteins. Of these, PsaA was the best at eliciting protection against carriage. However, a combination of PspA and pneumolysin may elicit stronger immunity to pulmonary infection and possibly sepsis than either protein alone. Recently, a phase one trial of

Characteristics of endovascular pneumococcal infections; a decade of nationwide surveillance study.

In order to characterize pneumococcal endovascular infection in the post-vaccination era, a retrospective nationwide study based on the Israeli Adult IPD database was conducted. Between 2010 and 2019, 0.6% (23 cases) of IPD cases were of endovascular type, occurring mainly in males (72.3%) with underlying medical conditions (78.2%). Additional pneumococcal source (10 patients) and concomitant infections were not uncommon. Penicillin and ceftriaxone susceptibility rates were 65.2% and 91.3%, respectively; 60.9% of the isolates were not covered by the pneumococcal conjugate vaccine. 21.7% of pat

[Swiss vaccinology update: new glycoconjugate vaccines against pneumococcal and meningococcal infections].

Two new glycoconjugate vaccines protecting against invasive diseases caused by 13 pneumococcal serotypes or 4 meningococcal serogroups will soon be available in Switzerland. They offer real new perspectives for young children, and for high-risk group adults. As their development is recent, the data one may wish to have is not yet fully available for each age and disease categories. This article attempts to delineate the interest of these new glycoconjugate vaccines and their limits. The additional information (and the answers to numerous expected questions) will be distributed to the 4300 subs

Bacteriological evidence of antibiotic failure in pneumococcal lower respiratory tract infections.

The global pandemic of antimicrobial resistance, particularly in the pneumococcus, has had a major impact on the management of community-acquired pneumonia. A number of prospective and retrospective studies have analysed the impact of penicillin resistance on clinical outcome in pneumonia. Pharmacodynamic principles predicting success when the antibiotic dose exceeds the minimum inhibitory concentration (MIC) for 40-50% of the dosing interval have proved remarkably accurate. There is no evidence of bacteriological failure of penicillins active against resistant strains. There is a single repor

Selective deficiency in pneumococcal antibody response in children with recurrent infections.

BACKGROUND: Impaired ability to respond to polysaccharide capsular antigens of Streptococcus pneumoniae may be associated with an IgG subclass deficiency and recurrent respiratory infections. OBJECTIVE: To identify children over three years of age with recurrent otitis media, sinusitis, or pneumonia who had low or absent pneumococcal antibody titers as their sole manifestation of immune deficiency and to determine their response to the 23-valent pneumococcal vaccine. RESULTS: Of 100 children with low pneumococcal antibody titers, 87 generated a protective antibody response to the pneumococca

The Yin and Yang of Pneumolysin During Pneumococcal Infection.

Pneumolysin (PLY) is a pore-forming toxin produced by the human pathobiont Streptococcus pneumoniae, the major cause of pneumonia worldwide. PLY, a key pneumococcal virulence factor, can form transmembrane pores in host cells, disrupting plasma membrane integrity and deregulating cellular homeostasis. At lytic concentrations, PLY causes cell death. At sub-lytic concentrations, PLY triggers host cell survival pathways that cooperate to reseal the damaged plasma membrane and restore cell homeostasis. While PLY is generally considered a pivotal factor promoting S. pneumoniae colonization and surv

[Pneumococcal infection is a vaccine-preventable disease now].

Analysis of pneumococcal disease burden in Russia, which is a serious threat to children's and adults' health and which was underrecognized until last decade, is presented. Extrapolating data about etiologic structure of pneumonia and meningitis on published incidence figures, we can estimate the incidence of pneumococcal pneumonia in children in Russia: 490 per 100,000 for children aged 1 month - 15 years, and 1060 per 100,000 for children aged 1 month - 4 years. For complicated forms, these figures are 90 and 181 per 100,000 respectively. Mean incidence of pneumococcal meningitis is 8 per 10
