Clinical outcomes associated with orphenadrine deliberate self-poisoning: a retrospective poisons centre study.

INTRODUCTION: Orphenadrine overdoses can cause antimuscarinic toxicity, respiratory failure, refractory seizures and cardiotoxicity. The dose-toxicity relationship is poorly defined. Orphenadrine is marketed as immediate and sustained release formulations, and it is not known how the formulation impacts on toxicity. We determined the clinical toxicity of orphenadrine in patients referred to a regional poisons centre. METHODS: Retrospective case series of patients in New South Wales with orphenadrine deliberate self-poisoning from January 2016 to April 2022 referred to the New South Wales Pois

Another function of the inner ear: facilitation of the emetic response to poisons.

If a foot were surgically removed and it was observed that walking was then impaired, it could be concluded that the foot is part of the normal mechanism for walking and that one of the physiological functions of the foot is to facilitate walking. In seven dogs, the vestibular apparatus of the inner ear was surgically removed and it was observed that the emetic response to certain poisons was impaired. It was concluded that the inner ear is part of the normal mechanism for vomiting in response to poisons, and that one of the physiological functions of the inner ear is to facilitate the emetic

Amitraz Poisoning: The (Un) Common Poisoning.

Pesticide poisoning is always a clinical conundrum for the emergency physician (EP), the complexity of which increases when the pesticide has no antidote! Over the past decade, there has been a sharp increase in cases of Amitraz poisoning, a pesticide routinely used in veterinary medicine, available without a prescription. The usual presentation includes bradycardia, hypotension, poor sensorium, and miosis. In the absence of accurate history, these clinical features can be confused with the cholinergic toxidrome of organophosphorus poisoning. There is a dearth of literature regarding the prese

Intimate details of the most poisonous poison.

Crystal structures of type B botulinum neurotoxin, determined under conditions relevant to the extracellular binding and intracellular substrate cleavage steps of its toxic action, reveal that minimal structural changes occur upon binding a ganglioside or after substrate cleavage. The unique structure of this toxin provides information for developing preventive measures against botulism.

A high-throughput fluorescent DNA cleavage assay for discovering bacterial DNA gyrase poisons.

Antimicrobial resistance is a major global public health threat, yet no new antibiotic class has been introduced in over 30 years. Fluoroquinolones (FQs), a type of bacterial DNA gyrase poison, are potent antibiotics which stabilizes gyrase-DNA cleavage complexes and therefore cause lethal DNA double-stranded breaks. However, rising FQ resistance combined with adverse effects highlight the need to identify new chemical scaffolds for gyrase poisons. Discovery efforts in this area have been limited by the lack of robust high-throughput screening (HTS) assays. Here, we describe a fluorescence-bas

Effects of mitochondrial poisons on glutathione redox potential and carotid body chemoreceptor activity.

Low oxygen sensing in chemoreceptor cells involves the inhibition of specific plasma membrane K(+) channels, suggesting that mitochondria-derived reactive oxygen species (ROS) link hypoxia to K(+) channel inhibition, subsequent cell depolarization and activation of neurotransmitter release. We have used several mitochondrial poisons, alone and in combination with the antioxidant N-acetylcysteine (NAC), and quantify their capacity to alter GSH/GSSG levels and glutathione redox potential (E(GSH)) in rat diaphragm. Selected concentrations of mitochondrial poisons with or without NAC were tested f

The sentinel role of poisons centers in the protection of children's environmental health.

Growing concern exists about the threats posed by environmental pollutants and physical agents on children's health. A number offactors, including globalization, increased industrialization, and trade of chemicals and poverty have an impact on the quality of the environment and on children's health. Acute toxic exposures lead to poisoning, and chronic low-level exposures may cause functional and organ damage during periods of special vulnerability. The need to protect children's environmental health is urgent, in view of the magnitude of the problem and the fact that "children are not little a

Severe barbiturate and paracetamol overdose: the simultaneous removal of both poisons by haemoperfusion.

Charcoal haemoperfusion was successful in the treatment of severe butobarbitone poisoning complicated by resistant hypotension. At the same time the rapid removal of paracetamol may have lessened the severity of the subsequent hepatic injury. The mechanism and management of shock associated with barbiturate poisoning, and the possible application of haemoperfusion in paracetamol poisoning are discussed.

Bites by exotic snakes reported to the UK National Poisons Information Service 2009-2020.

Introduction: Snakebite is recognised as a neglected tropical disease and a cause of substantial morbidity and mortality. Whilst the most medically important snakes are typically native of Asia, Africa, Latin America and Oceania, the possibility of encountering these snakes is no longer limited by geography due to an increasing number of exotic (non-native) snakes being held in captivity. Methods: A retrospective review of snakebite enquiries to the UK National Poisons Information Service (NPIS) between 2009 and 2020. Vibrio cholerae ParE2 poisons DNA gyrase via a mechanism distinct from other gyrase inhibitors.

DNA gyrase is an essential bacterial enzyme required for the maintenance of chromosomal DNA topology. This enzyme is the target of several protein toxins encoded in toxin-antitoxin (TA) loci as well as of man-made antibiotics such as quinolones. The genome of Vibrio cholerae, the cause of cholera, contains three putative TA loci that exhibit modest similarity to the RK2 plasmid-borne parDE TA locus, which is thought to target gyrase although its mechanism of action is uncharacterized. Here we investigated the V. cholerae parDE2 locus. We found that this locus encodes a functional proteic TA pa
