The Effects of Automatic Inspiratory Rise Time and Flow Termination on Operation of Closed-Loop Ventilation.

BACKGROUND: Adaptive ventilation mode (AVM) is a automated mode of mechanical ventilation. AVM is comprable to adaptive support ventilation (ASV). Both recommend a tidal volume (VT) and breathing frequency (f) combination based on lung mechanics, but AVM also automatically adjusts rise time and flow termination of pressure support breaths. How these added features of AVM affect VT and f recommendations compared to ASV is not clear. The present study compared these 2 modes in a test lung with obstructive and restrictive mechanics. METHODS: The experiment was performed in a simulated lung model

[Mechanical ventilation in pediatrics (III). Weaning, complications and other types of ventilation. Noninvasive ventilation].

Noninvasive ventilation (NIV), i.e. without tracheal intubation, has been reintroduced for the treatment of respiratory failure to reduce the complications of mechanical ventilation. Nowadays, NIV with positive pressure is the preferred method, applied through a mask held in place by a harness. Several masks can be used (nasal, bucconasal facial) and a variety of means can be used to keep them in place. Many respirators can be selected, ranging from those traditionally used in the intensive care unit(ICU) to specific NV respirators and conventional ICU respirators with specific software for NI

[The obesity-hypoventilation syndrome].

Obesity, well-known as a cardiovascular risk factor is also a "respiratory" risk factor and can have profound adverse effects on the respiratory system, such as alterations in pulmonary function tests, respiratory mechanics, respiratory muscle strength and endurance, gas exchange, control of breathing and exercise capacity. ABG are frequently altered in obese subjects and abnormalities are directly proportional to BMI. Two main pathophysiological mechanisms may account for gas exchange abnormalities: V/Q inequality, responsible for isolated hypoxemia, and alveolar hypoventilation responsible f

Efficiency of manual ventilation using the original EMO system.

The efficiency of the Oxford Inflating Bellows for intermittent positive pressure ventilation using a partially closed Heidbrink expiratory valve has been assessed by measuring the arterial carbon dioxide tension (PaCO2) after various intervals of IPPV on eight patients. It was found that satisfactory alveolar ventilation was achieved if a degree of hyperventilation was employed.

Plateau Pressure Exceeding Peak Inspiratory Pressure During Pressure Mode Ventilation With Active Inspiratory Effort.

BACKGROUND: In patients receiving pressure modes of mechanical ventilation, plateau pressure can be higher than peak inspiratory pressure when the patient has a strong drive to breathe. This report provides comprehensive evidence to support this observation. METHODS: Peak inspiratory pressure, plateau pressure, esophageal pressure, and tidal volume were observed at various levels of pressure support in a patient with absent cortical function but preserved brainstem function who was receiving mechanical ventilation. RESULTS: Positive end-expiratory pressure was 5 cm H2O for all pressure suppo

Complex Heart-Lung Ventilator Emergencies in the CICU.

This review aims to enhance the comprehension and management of cardiopulmonary interactions in critically ill patients with cardiovascular disease undergoing mechanical ventilation. Highlighting the significance of maintaining a delicate balance, this article emphasizes the crucial role of adjusting ventilation parameters based on both invasive and noninvasive monitoring. It provides recommendations for the induction and liberation from mechanical ventilation. Special attention is given to the identification of auto-PEEP (positive end-expiratory pressure) and other situations that may impact

Low-cost, easy-to-build noninvasive pressure support ventilator for under-resourced regions: open source hardware description, performance and feasibility testing.

AIM: Current pricing of commercial mechanical ventilators in low-/middle-income countries (LMICs) markedly restricts their availability, and consequently a considerable number of patients with acute/chronic respiratory failure cannot be adequately treated. Our aim was to design and test an affordable and easy-to-build noninvasive bilevel pressure ventilator to allow a reduction in the serious shortage of ventilators in LMICs. METHODS: The ventilator was built using off-the-shelf materials available via e-commerce and was based on a high-pressure blower, two pressure transducers and an Arduino

COPD patients and mechanical ventilation with positive end expiratory pressure (PEEP).

Application of positive and expiratory pressure (PEEP) is widely used in mechanically ventilated patients with acute respiratory failure (ARF) due to adult respiratory distress syndrome. Recent studies have suggested that application of PEEP can be useful in patients with chronic obstructive pulmonary disease (COPD) to reduce the mechanical inspiratory load due to intrinsic positive and expiratory pressure (PEEPi). In any ventilatory mode (controlled mechanical ventilation, assisted mechanical ventilation) and during weaning, application of moderate levels of PEEP replace in part PEEPi without

Tracheal and bronchial injury in high-frequency oscillatory ventilation compared with conventional positive pressure ventilation.

We compared airway histopathologic findings in premature baboons given standard positive pressure ventilation with those seen after high-frequency oscillatory ventilation. Six animals received standard frequency conventional ventilation for a mean of 9.2 days; seven received high-frequency oscillatory ventilation at 10 Hz using a piston oscillator for a mean of 10.2 days; five baboons served as controls, and were killed immediately after birth. A semiquantitative histopathologic scoring system was used to grade tissue changes in the trachea, carina, and both mainstem bronchi. Compared with the

Heart and lung support interaction--modeling and simulation.

Mechanical support of the lungs used to preserve life or during any kind of surgery may have an adverse effect on the cardiovascular system. Usually, positive pressure in alveoli diminishes lung perfusion, venous return and cardiac output. Positive pressure during the respiratory cycle is transfered into the thoracic space. The aim of this study was to assess how synchronization of the respirator with spontaneous breathing influences the distribution of pressure and ventilation in nonhomogeneous lungs and how it should influence hemodynamics. For this purpose a multicompartmental model of resp

Positive Airway Pressure Therapy and Respiratory Assist Devices: Indications for Use.

Continuous positive airway pressure is the gold standard for treatment of obstructive sleep apnea. For certain conditions, such as central sleep apnea, hypoventilation, hypercapnic respiratory failure, and restrictive lung disease, noninvasive ventilation (NIV), which includes respiratory assist devices and home mechanical ventilators, may be indicated. This article will review the various positive airway pressure and NIV devices currently available as well as the modes, settings, indications, and contraindications of these devices in both the inpatient and outpatient settings. Invasive Respiratory Support in Critical Pediatric Asthma.

In the United States and Canada, severe asthma requiring mechanical ventilation has declined over the past decade reflecting a rise in noninvasive therapies. When aggressive noninvasive therapies fail, endotracheal intubation and mechanical ventilation are lifesaving and should be planned for in advance. As speed is important, the most experienced practitioner should intubate and rapid correction of hypercarbia and respiratory acidosis should be avoided. An elevated minute ventilation may cause pulmonary hyperinflation leading to air-leak syndrome and/or hemodynamic instability. Patients with

Pathophysiology and clinical applications of PEEP in acute brain injury.

Mechanical ventilation is a life-sustaining treatment needed in patients with acute brain injury to maintain airway permeability, optimize gas exchange, and prevent secondary brain damage. Positive end-expiratory pressure (PEEP), a key component of mechanical ventilation, helps prevent atelectasis, improve oxygenation, and stabilize alveolar recruitment, offering potential benefits in terms of lung protection. However, neurological tolerance of PEEP can be poor in brain-injured patients. The variability in lung and chest-wall elastance, lung recruitability, cardiac function, and fluid status,

Strategies for lung- and diaphragm-protective ventilation in acute hypoxemic respiratory failure: a physiological trial.

BACKGROUND: Insufficient or excessive respiratory effort during acute hypoxemic respiratory failure (AHRF) increases the risk of lung and diaphragm injury. We sought to establish whether respiratory effort can be optimized to achieve lung- and diaphragm-protective (LDP) targets (esophageal pressure swing - 3 to - 8 cm H2O; dynamic transpulmonary driving pressure ≤ 15 cm H2O) during AHRF. METHODS: In patients with early AHRF, spontaneous breathing was initiated as soon as passive ventilation was not deemed mandatory. Inspiratory pressure, sedation, positive end-expiratory pressure (PEEP), and

[Effects of continuous positive airway pressure on Cheyne-Stokes breathing in congestive heart failure].

To investigate the effects of a continuous positive airway pressure (CPAP) of 5 cmH2O for three months on sleep disordered breathing and cardiac function in congestive heart failure (CHF), we performed physiological tests before and after the CPAP treatment in six male patients with stable CHF, whose echocardiographic left ventricular ejection fraction (LVEF) was below 30%. The control polysomnography revealed that all six had Cheyne-Stokes breathing (with central sleep apnea in four, and central sleep hypopnea in two). Prior to the CPAP, the number of episodes of apnea/hypopnea per hour of sl

[Comparison of high-frequency jet ventilation and conventional ventilation in the adult respiratory distress syndrome].

Sixteen adult patients with respiratory distress syndrome requiring mechanical respiratory assistance entered this study, the purpose of which was to obtain the same blood gas values under high frequency jet ventilation as under conventional ventilation. When high frequency jet ventilation without spontaneous breathing was compared to continuous positive pressure ventilation, peak airway pressure was the same, but mean airway pressure, positive end-expiratory pressure and pleural pressure were higher and cardiac index lower. When high frequency jet ventilation with spontaneous breathing was co
