Prospective evaluation of postnatal steroid administration: a 1-year experience from the California Perinatal Quality Care Collaborative.

OBJECTIVE: Postnatal steroids (PNSs) are used frequently to prevent or treat chronic lung disease (CLD) in the very low birth weight (VLBW) infant, and their use continues despite concerns regarding an increased incidence of longer-term neurodevelopmental abnormalities in such infants. More recently, there has been a suggestion that corticosteroids may be a useful alternative therapy for hypotension in VLBW infants, but there have been no prospective reports of such use for a current cohort of VLBW infants. METHODS: The California Perinatal Quality Care Collaborative (CPQCC) requested members

Local understandings of care during delivery and postnatal period to inform home based package of newborn care interventions in rural Ethiopia: a qualitative study.

BACKGROUND: Despite a substantial decrease in child mortality in Ethiopia over the past decade, neonatal mortality remains unchanged (37/1000 live-births). This paper describes a qualitative study on beliefs and practices on immediate newborn and postnatal care in four rural communities of Ethiopia conducted to inform development of a package of community-based interventions targeting newborns. METHODS: The study team conducted eight key informant interviews (KII) with grandmothers, 27 in-depth interviews (IDI) with mothers; seven IDI with traditional birth attendants (TBA) and 15IDI with fat

Advancing maternal and newborn healthcare measurement: developing quality of care indices for postnatal and small and/or sick newborn care in low- and middle-income countries.

BACKGROUND: High-quality healthcare for pregnant women and newborns, particularly postnatal care (PNC) and small and/or sick newborn care (SSNC), is essential to reducing maternal and newborn morbidity and mortality in low- and middle-income countries (LMICs). Poor quality of care (QoC) is a major contributor to preventable morbidity and mortality, emphasising the need for its improvement in health service delivery through systematic measurement and monitoring. Although indicators measuring QoC have been identified, there is a current gap in the availability of composite indicators that can su

Exploring the Lived Experience of Women Receiving Antenatal, Intrapartum, and Immediate Postnatal Care: The Essence of Being with the Midwives.

BACKGROUND: Midwives have essential roles in shaping policies, advancing practice, and ensuring women-centered education that leads to positive maternal and neonatal outcomes. In Qatar, healthcare systems integrated midwifery-led services to increase the workforce supporting women, but no studies have examined women's experiences with midwives. Thus, this study explores women's lived experiences of midwifery care in Qatar to identify its impact and areas for improvement. MATERIALS AND METHODS: A qualitative descriptive-phenomenological design was used. Twelve women who received perinatal care

Level of skin-to-skin care practices among postnatal mothers in Ethiopia. A systematic review and meta-analysis.

INTRODUCTION: Even though skin-to-skin contact offers several advantages for the survival of the newborn, it is not often practiced in Ethiopia. For instance, hypothermia which increases the risk of neonatal mortality by five times is prevented by this practice. Despite this, there are inconsistent findings that can affect policymaking. Consequently, this metanalysis aimed to produce trustworthy national data regarding skin-to-skin care practice and its determinants among postpartum mothers in Ethiopia. METHODS: A search of the publications was conducted using MEDLINE, PubMed, Embase, Scopus,

Exploring the influence of postnatal depression on neonatal care practices among mothers in Western Kenya: A qualitative study.

BACKGROUND: Postnatal depression (PND) is associated with adverse infant neurodevelopmental outcomes. Evidence is limited on how PND influences neonatal (<28 days old) outcomes in low- and middle-income countries, such as Kenya, which bear the global burden of neonatal morbidity and mortality. OBJECTIVES: To explore how PND influences neonatal feeding and care practices among women in the early postnatal period in rural Western Kenya. DESIGN: A cross-sectional study. METHODS: Semi-structured interviews were conducted at 2-weeks postpartum among mothers of newborn infants identified <72 h ol

Essential components of postnatal care - a systematic literature review and development of signal functions to guide monitoring and evaluation.

BACKGROUND: Postnatal Care (PNC) is one of the healthcare-packages in the continuum of care for mothers and children that needs to be in place to reduce global maternal and perinatal mortality and morbidity. We sought to identify the essential components of PNC and develop signal functions to reflect these which can be used for the monitoring and evaluation of availability and quality of PNC. METHODS: Systematic review of the literature using MESH headings for databases (Cinahl, Cochrane, Global Health, Medline, PubMed, and Web of Science). Knowledge of postnatal mothers on essential newborn care practices at the Kenyatta National Hospital: a cross sectional study.

INTRODUCTION: Of the 130 million babies born yearly, nearly 4 million die in the neonatal period. Kenya Demographic Health Survey (KDHS) 2014 places neonatal mortality rate at 22 deaths per 1,000 live births, well above the Sustainable Development Goal (SDG) 3 that aims to reduce these mortalities to at least 12 deaths per 1000 live births by 2030. The aim of the study was to assess maternal knowledge on selected components of essential newborn care: breastfeeding, cord care, immunisation, eye care and thermoregulation. METHODS: A hospital based cross-sectional study was conducted on 380 post

Effectiveness of transcutaneous bilirubin measurement in managing neonatal jaundice in postnatal ward of a tertiary care hospital in Pakistan.

INTRODUCTION: Neonatal jaundice is a common cause of concern in immediate newborn period for parents as well as for the caregivers. Babies with visible jaundice are identified by the healthcare provider, and blood samples are sent for confirmation. Clinical expertise varies from person to person and may lead to sending excessive blood sampling. Obtaining blood bilirubin samples is a painful procedure; it predisposes the baby to infections and requires skilled health personnel. Moreover, laboratory tests are costly and time consuming, leading to unnecessary delays in commencing phototherapy and

Congenital malformations, palliative care and postnatal redirection to more intensive treatment - a review at a Swiss tertiary center.

PURPOSE: The so-called lethal malformations pose ethical challenges. Most affected fetuses die before or at birth. Live-born neonates commonly receive palliative care. If the postnatal course is better than expected, redirection towards more treatment may occur. We aimed to analyze this in a Swiss patient cohort. MATERIALS AND METHODS: Over 6 years, fetal malformation was suspected in 1113 cases. We identified patients prenatally assigned to palliative care, assessed pre- and postnatal diagnoses, and outcomes. RESULTS: Fourteen neonates received palliative care. Eleven patients received pall

Postnatal care and acceptability of emollient therapy in very low birthweight infants in Harare, Zimbabwe: a qualitative analysis.

BACKGROUND: Preterm birth (birth before 37 completed weeks of pregnancy) is the leading cause of neonatal and child under-five mortality globally, both of which are highest regionally in sub-Saharan Africa. The skin barrier plays a critical role in neonatal health and increasing evidence supports the use of topical emollient therapy to promote postnatal growth and reduce hospital-acquired infections in preterm infants. The World Health Organization (WHO) currently recommends emollient therapy in preterm or low birthweight infants globally but calls for further research on impacts of emollient

Postnatal women's perception on person-centered maternity care in twin cities of Rawalpindi and Islamabad: a descriptive study.

BACKGROUND: Person-Centered Maternity Care (PCMC) is known as one of the most important components of maternal care. Every woman has the ultimate right of respectful health care. Previous research documents that lack of supportive care and respectful behavior experienced by pregnant women can act as a barrier to the utilization of health care services. Few studies have used PCMC tool to document this phenomenon. The objective of this descriptive study was to assess the women's perception of PCMC in Pakistan. METHODS: Three hundred and seventy-seven (377) postnatal women of ages 18-49 years pa

What is the Impact of Prenatal Counseling on Postnatal Cleft Treatment? Multidisciplinary Pathway for Prenatal Orofacial Cleft Care.

In the pediatric general surgery literature, it has been shown that prenatal diagnosis of a congenital anomaly is an independent predictor of parental psychological distress. Surgical prenatal counseling can decrease parental anxiety by helping families understand the surgical needs and potential outcomes of their infant. In this retrospective analysis (n = 440), the authors sought to present our care pathway for prenatally diagnosed cleft lip and palate (CL/P) and explore the impact of cleft lip and palate-specific prenatal counseling on patient care by comparing the timing of clinical and su

Factors associated with postnatal care for newborns in Zambia: analysis of the 2013-14 Zambia demographic and health survey.

BACKGROUND: The importance of postnatal care cannot be overemphasised. Various studies undertaken worldwide have found that PNC is critical for the survival of newborns. However, in Zambia, despite much emphasis by the government and various international Organisations on the need for PNC, coverage continues to be low. This study attempted to assess the demographic and socio-economic factors associated with newborns' receipt of PNC and the timing of first PNC in Zambia. METHODS: Based on data from the 2013-14 Zambia Demographic and Health Survey (ZDHS), this study used bivariate, stepwise bin
