Prevalence of Postoperative Hypothermia in the Post-anesthesia Care Unit.

BACKGROUND: Unwanted postoperative hypothermia is an unpleasant event that can cause various complications. OBJECTIVES: As this serious complication and its provoking causes have not been investigated sufficiently, this study was designed and conducted to determine the prevalence of hypothermia and its associated factors in the post-anesthesia care unit after elective surgery. METHODS: Four hundred patients undergoing elective surgeries were enrolled in the present cross-sectional research after termination of surgery and at the time of arrival at the post-anesthesia care unit of Poursina Pu

Delirium in Postoperative Patients Admitted to the Intensive Care Unit.

Postoperative delirium (POD) is common among surgical patients admitted to the intensive care unit (ICU) and is associated with increased resource utilization, morbidity, and death. Our primary aim was to compare rates of POD using administrative International Classification of Diseases, Ninth Revision, records and automated interrogation of electronic health records from Confusion Assessment Method for the ICU (CAM-ICU) screening. The secondary aim was to assess POD risk associated with patient and perioperative characteristics. Electronic health records of surgical patients admitted to the I

Tonsillectomy in Australia: an audit of surgical technique and postoperative care.

OBJECTIVE: To assess current tonsillectomy practice among Australian otolaryngologists. STUDY DESIGN: An audit based on an anonymous 19-item postal questionnaire on tonsillectomy technique and perioperative management sent to all Australian otolaryngology specialists. SUBJECTS AND METHODS: Two hundred eighty-four otolaryngologists registered with the Australian Society of Otolaryngology-Head and Neck Surgery database were sent the questionnaire. RESULTS: A 72.5 percent response rate was obtained. Monopolar diathermy was the most common technique for dissection (45%) and hemostasis (54%). Bi

Pre- and postoperative nutritional care in liver transplantation in children.

Orthotopic liver transplantation (OLT) is now a definitive treatment option for most cases of endstage liver disease (ESLD) in children. Efforts now focus on active supportive treatment to maintain, if not improve, the patient's clinical status before OLT and to ensure normal patterns of growth and development after OLT. Malnutrition adversely affects the outcome of OLT and is probably the single area in pre-operative management where the largest potential improvement can be made. Our studies indicate significant abnormalities of protein energy metabolism and body composition in children refer

Postoperative care after thoracic surgery in the times of ERAS.

This review documents the importance of postoperative interventions that accelerate the functional recovery of the thoracic surgical patient. Enhanced recovery after surgery (ERAS) pathways aim to mitigate the harmful surgical stress response. Improvements to the entire patient pathway, by removing unnecessary care elements while introducing evidence-based interventions, have synergistic effects. At the same time, some key care elements appear to be more important than others, including early removal of chest tubes, early mobilization, and the limited use of opioids. These care elements are al

The Edge of Unknown: Postoperative Critical Care in Liver Transplantation.

Perioperative care of patients undergoing liver transplantation (LT) is very complex. Metabolic derangements, hypothermia, coagulopathy and thromboses, severe infections, and graft dysfunction can affect outcomes. In this manuscript, we discuss several perioperative problems that can be encountered in LT recipients. The authors present the most up-to-date information regarding predicting and treating hemodynamic instability, coagulation monitoring and management, postoperative ventilation strategies and early extubation, management of infections, and ESLD-related pulmonary complications. Use of Ultrasonography for Lung Transplant Recipients on Postoperative Care.

The authors report their findings regarding lung ultrasound profiles in a population of transplant recipients. Twenty-two patients were studied once each in multiple different ultrasound windows focusing on pleural, lung, and diaphragmatic signatures. All studies were performed in presumably otherwise healthy recipients at an outpatient follow-up visit at least 3 months after transplantation. Those with recent pulmonary infections or decline in lung function were excluded from enrollment. The majority of scans revealed otherwise normal lungs with lung sliding, but there were more abnormalities

Postoperative pain management in the postanesthesia care unit: an update.

Acute postoperative pain remains a major problem, resulting in multiple undesirable outcomes if inadequately controlled. Most surgical patients spend their immediate postoperative period in the postanesthesia care unit (PACU), where pain management, being unsatisfactory and requiring improvements, affects further recovery. Recent studies on postoperative pain management in the PACU were reviewed for the advances in assessments and treatments. More objective assessments of pain being independent of patients' participation may be potentially appropriate in the PACU, including photoplethysmograph

Impact of obesity on care of postoperative coronary bypass patients.

Obesity has become a major health problem in the United States and is well known to be a risk factor for the development of cardiovascular disease. Many clinicians perceive obesity, particularly severe or morbid obesity, to be associated with increased risk for mortality and morbidity following coronary artery bypass graft (CABG) surgery. This article provides a review of the literature related to mortality and morbidity, including the impact of diabetes, risk for acute respiratory failure, and sternal wound infection associated with obese patients undergoing CABG surgery. Implications for nur

[Progress in postoperative care and postoperative combined therapy of esophageal carcinoma by total parenteral nutrition (TPN)].

Intravenous hyperalimentation which consisted of 30% hypertonic glucose solution and 10% amino acid solution via central venous catheterization and 10% lipid solution via peripheral venous lines has been applied to the postoperative patients with esophageal cancers since 10 years ago. sixty-five percent of the patients who underwent surgical treatment showed abnormal signs of the glucose tolerance function test. Positive application of insulin for these patients was performed according to the algorism of continuous insulin infusion method devised by us. Serum glucose levels have been kept with

Lived experiences of cardiovascular postoperative care in pediatric and neonatal critical care: The parents' experience.

INTRODUCTION: Heart disease in childhood is the fourth leading cause of illness and, in many cases, requires complex surgical procedures and intensive care unit care. This situation has a significant impact on family caregivers, especially at the psychosocial and emotional levels, due to the uncertainty about the children's prognosis. Therefore, it is essential to provide support to facilitate coping with this critical experience. OBJECTIVE: To describe the experiences of parents and family caregivers caring for children postoperatively after cardiovascular surgery, hospitalized in in pediatr

Use of therapeutic caffeine in acute care postoperative and critical care settings: a scoping review.

BACKGROUND: Caffeine is the most utilised psychoactive drug worldwide. However, caffeine withdrawal and the therapeutic use of caffeine in intensive care and in the perioperative period have not been well summarised. Our objective was to conduct a scoping review of caffeine withdrawal and use in the intensive care unit (ICU) and postoperative patients. METHODS: PubMed, Embase, CINAHL Complete, Scopus and Web of Science were systematically searched for studies investigating the effects of caffeine withdrawal or administration in ICU patients and in the perioperative period. Areas of recent sys
