Rational deprescribing in the elderly.

BACKGROUND: Polypharmacy, defined as being prescribed 5 or more medications, has been shown to be associated with a decline in mental and physical functioning in elderly patients. Despite this, elderly patients are currently being prescribed a median of 7 medications, which often causes more harm than benefit, and emphasizes the importance of deprescribing. METHODS: Five classes of potentially inappropriate medications for the elderly population, especially for the aging brain, are discussed, including anticholinergics, benzodiazepines, antipsychotics, opioids, and proton pump inhibitors. Development of the physician-pharmacist partnership intervention to deprescribe medications (P3iD) and its feasibility in clinical practice.

BACKGROUND: Polypharmacy and the use of potentially inappropriate medications (PIMs) are major concerns in managing older adults with multiple chronic conditions, as they can increase the risk of adverse drug reactions. However, data on deprescribing interventions for older persons in lower- to middle-income countries (LMIC) remains limited. PURPOSE: To develop and evaluate the feasibility of the Physician-Pharmacist Partnership Intervention to Deprescribe Medications (P3iD) among ambulatory older persons. METHODOLOGY/APPROACH: The P3iD, a health workforce model intervention for deprescribin

Understanding polypharmacy for people receiving home care services: a scoping review of the evidence.

BACKGROUND: Polypharmacy, defined as the concurrent use of five or more medications, is common amongst older adults receiving home care services. The relationship between home care and polypharmacy may be critical to older people's health, but there is little research on this topic. OBJECTIVE: To understand the extent and type of evidence on individuals receiving home care services and experiencing polypharmacy. METHODS: This review followed the Preferred Reporting of Items for Systematic Reviews and Meta-Analyses extension for scoping reviews. Three databases (MEDLINE, Embase, CINAHL) were

Evaluation of potentially inappropriate medications in older patients admitted to the cardiac intensive care unit according to the 2019 Beers criteria, STOPP criteria version 2 and Chinese criteria.

WHAT IS KNOWN AND OBJECTIVES: Potential inappropriate medications (PIMs) can increase the risk of medication-induced harm. However, there are no studies regarding PIMs in older and critically ill patients with cardiovascular diseases in China. Therefore, studies evaluating PIMs in these patients can help in the implementation of more effective interventions to reduce the risk of drug use. Our objective was to analyse the prevalence of PIMs in elderly patients admitted to the cardiac intensive care unit (CICU) comparing the 2019 Beers criteria (Beers criteria), Screening Tool of Older People's

Identification of Potentially Inappropriate Medications for Adults Below 65 Years: Protocol for a Modified Delphi Study.

BACKGROUND: Polypharmacy is increasingly common across all age groups and is often associated with the use of potentially inappropriate medications (PIMs), where harms may outweigh benefits, contributing to increased adverse drug events, reduced quality of life, and rising health care costs. However, existing deprescribing guidelines and PIM criteria, such as the Beers Criteria and STOPP/START (Screening Tool of Older Persons' Prescriptions/Screening Tool to Alert to Right Treatment), primarily target older adults, overlooking the risks faced by younger populations. OBJECTIVE: This study aims

Trends of potentially inappropriate prescribing among older outpatients in China between 2015-21.

BACKGROUND: Potentially inappropriate medication (PIM) remains a significant global concern due to its association with adverse drug events in older adults. The prevalence of potentially inappropriate prescriptions (PIPs) varies considerably across different countries. However, there is a lack of evidence using standardised Chinese national criteria to explore variations in prescribing trends. We aimed to evaluate the trends in the prevalence of PIP based on these criteria. METHODS: In this descriptive epidemiological study, we utilised prescription data from older outpatients across 59 hospi

Assessing the prevalence of Beers medication utilization in the Medicare Part D population in 2020.

BACKGROUND: Medication utilization has been increasing in the U.S. year-over-year for several decades. As older adults take more medications, there is a higher risk of them being exposed to drug-drug or drug-disease interactions. The American Geriatrics Society in 2019 updated their Beers Criteria for Potentially Inappropriate Medication (PIM) Use in Older Adults. The objective of this study was to assess the prevalence of utilization of medications included in the 2019 Beers Criteria. METHODS: An analysis was conducted using the Medicare Part D Provider Utilization and Payment Data Public Us

Association of potentially inappropriate medications and need for long-term care among older adults: a matched cohort study.

BACKGROUND: With an increase in the aging population, the number of older adults who require long-term care (LTC) is growing, enhancing drug-related issues. The reduced capacity of LTC users to precisely utilize medical services poses additional challenges owing to restrictions in daily activities. We compared older adults who required LTC with those who did not require LTC to confirm differences in the use of potentially inappropriate medications (PIMs), frequently used PIMs, and associating factors in Korea. METHODS: Using the Korean National Health Insurance Service cohort data, adults age

Impact of polypharmacy on oral health status in elderly patients admitted to the recovery and rehabilitation ward.

AIM: The oral health status of elderly adults has implications for general health problems. Polypharmacy for elderly adults is associated with an increased risk of potentially inappropriate medicines and many adverse drug events; however, no report has shown that polypharmacy itself is associated with complex oral problems. This study aimed to determine the association between polypharmacy and oral health status in patients admitted to the recovery and rehabilitation ward. METHODS: This cross-sectional study included 471 patients (age: 81.9 ± 7.7 years, women: n = 346). Oral health status was

Traditional Methods Hold Their Ground Against Machine Learning in Predicting Potentially Inappropriate Medication Use in Older Adults.

OBJECTIVES: Machine learning methods have gained much attention in health sciences for predicting various health outcomes but are scarcely used in pharmacoepidemiology. The ability to identify predictors of suboptimal medication use is essential for conducting interventions aimed at improving medication outcomes. It remains uncertain whether machine learning methods could enhance the identification of potentially inappropriate medication use among older adults compared with traditional methods. This study aimed to (1) to compare the performances of machine learning models in predicting use of
