Non-invasive Monitoring of Pouchitis After Total Proctocolectomy Using Fecal Calprotectin Levels.

BACKGROUND: Fecal calprotectin (FC) is the most widely used marker for evaluating the disease activity of ulcerative colitis (UC). However, studies on FC in pouchitis after total proctocolectomy are scarce. We aimed to examine the correlations between the FC level and clinical findings and Pouchitis Disease Activity Index (PDAI) in UC patients who underwent total proctocolectomy (TP) with ileal pouch-anal canal anastomosis (IPAA) or ileal pouch-anal canal anastomosis (IACA). METHODS: Between April 2008 and March 2018, 15 patients, consisting of 8 males and 7 females, with an average age at op

Consumption of yoghurt favours remission after faecal microbiota transplantation for chronic pouchitis.

PURPOSE: The gut microbiota is conceivably a key factor in the aetiology of pouchitis. Faecal microbiota transplantation (FMT) has been suggested as a promising new treatment for chronic pouchitis, where treatment options often are few. However, little is known about the influence of the diet on the clinical effects of FMT. We assessed the diet of patients with chronic pouchitis undergoing FMT to investigate the influence of diet on the clinical outcome after FMT. METHODS: Nine patients with chronic pouchitis were allocated to treatment with FMT delivered by enema from five faecal donors for

NOD2/CARD15 mutations correlate with severe pouchitis after ileal pouch-anal anastomosis.

PURPOSE: Pouchitis and Crohn's-like complications can plague patients after IPAA. NOD2 is an intracellular sensor for bacterial cell wall peptidoglycan. NOD2 mutations compromise host response to enteric bacteria and are increased in Crohn's disease. We hypothesize that IPAA patients with complications (Crohn's disease-like/pouchitis) have a higher rate of NOD2 mutations compared with asymptomatic IPAA patients. METHODS: Patients were retrospectively subclassified into the following groups: 1) IPAA with Crohn's-like complications (n = 28, perianal fistula, pouch inlet stricture/upstream small

Incidence, risk factors, and treatment patterns of pouchitis in pediatric ulcerative colitis.

BACKGROUND: Despite advances, patients with ulcerative colitis or inflammatory bowel disease (IBD) unclassified require restorative proctocolectomy with ileal pouch-anal anastomosis (RPC-IPAA) for refractory disease. Pouchitis is the most common postoperative complication, yet its incidence, natural history, and treatment in pediatric patients are poorly defined. We characterized the clinical course of pouchitis and determined predictors of postoperative advanced IBD therapy. METHODS: We retrospectively identified patients 2.5 to 20.8 years of age with RPC-IPAA between 2010 and 2023. Data col

Cellular and molecular mechanisms in the pathogenesis of pouchitis: more than just the microbiota.

Pouchitis, defined as inflammation of the ileal pouch, is the most common complication following restorative proctocolectomy for refractory ulcerative colitis. Antibiotics remain the first line of therapy for pouchitis, but the majority of patients develop subsequent episodes and some are refractory to antibiotic therapy. This highlights the need for more effective treatment options and points to a more complex pathophysiology beyond the role of th pouch microbiome, similar to what is seen in inflammatory bowel disease. In this review, we outline the putative mechanisms of pouchitis, including

Cost Effectiveness of the 8-Strain Probiotic in Primary and Secondary Prophylaxis of Pouchitis.

BACKGROUND AND AIMS: The 8-strain probiotic formulation appears to be effective for primary and secondary prevention of pouchitis in patients with ulcerative colitis after ileal pouch anal anastomosis. We aimed to study its cost-effectiveness compared to no prophylaxis in these settings. METHODS: We constructed decision trees with Markov models for primary prevention of pouchitis and secondary prevention of relapsing pouchitis in patients with ulcerative colitis after ileal pouch anal anastomosis. All patients were followed for 2 years. In the primary prophylaxis model, Markov cycle length wa

Sensitivity and specificity of diagnostic modalities in pouchitis: a systematic review and meta-analysis.

Restorative proctocolectomy (RPC) is the most common surgical procedure to restore gastrointestinal continuity after surgical resection of the colon and rectum. Pouchitis is a common complication with several different modalities that can be used to aid diagnosis. This systematic review and meta-analysis aimed at exploring the sensitivity and specificity these diagnostic modalities. The medical literature was searched using MEDLINE, EMBASE and EMBASE classic and PubMed from 1978 to June 2022. We calculated descriptive statistics using sensitivity, specificity, and false-positive rate of the pr

Association Between Portal Vein Thrombosis and Pouchitis in Patients with Ulcerative Colitis.

BACKGROUND: Pouchitis is the most common long-term complication in patients requiring colectomy ileal pouch-anal anastomosis with medically refractory ulcerative colitis or colitis-associated neoplasia. A previous small case series suggests associated between portal vein thrombosis (PVT) and ischemic pouchitis. AIM: To evaluate the association between PVT and other demographic and clinical factors and pouchitis. METHODS: We used Explorys Inc., a population-based database, to search medical records between 1999 and 2020 with SNOMED-CT code criteria for "construction of pouch" and "ileal pouch

Pyloric gland metaplasia and pouchitis in patients with ileal pouch-anal anastomoses.

BACKGROUND: Diagnosis and management of chronic antibiotic-refractory pouchitis and Crohn's disease of the pouch can be challenging. Pyloric gland metaplasia is a histological feature indicative of chronic mucosal inflammation. Its value in diagnosis and prognosis of pouch disorders has not been investigated. AIM: To assess the prevalence, diagnostic and prognostic value, and risk factors of pyloric gland metaplasia in pouch patients. METHODS: Patients were identified from our prospectively maintained Pouchitis Database. Pouch biopsy specimens were re-reviewed for pyloric gland metaplasia an

Risk of dysplasia in long-term ileal pouches and pouches with chronic pouchitis.

BACKGROUND & AIMS: Recent reports have suggested the mucosa of an ileal reservoir could be at risk of neoplasia. Risk factors may include the age of the pouch, chronic pouchitis, and previous colonic neoplasia. This study examined a group of such patients to determine the risk of dysplasia. METHODS: From a cohort of 1221 patients with ileal pouches, 171 patients with possible risk factors were selected. Successful contact was made with 138 patients who were invited for endoscopy and multiple biopsies. Biopsy specimens were stained with H&E and p53, scored for inflammatory changes including vi

Pouchitis is associated with mucosal imbalance between interleukin-8 and interleukin-10.

BACKGROUND: Mucosal lesions of pouchitis are characterized by a neutrophil infiltrate. Interleukin (IL)-8 is the main mediator involved in neutrophil recruitment and is down-regulated by IL-10. AIM: To look for an imbalance between IL-8 and IL-10 in patients with pouchitis. PATIENTS/METHODS: 18 patients having an ileoanal pouch for ulcerative colitis were studied. Eleven had pouchitis defined by the pouchitis disease activity index of > or =7 points and 7 had no history of pouchitis. Biopsies taken at the site of inflammation or in the normal mucosa were scored for the histologic lesions, th

Optimal Antibiotic Selection for the Treatment of Pouchitis After Ileal Pouch-Anal Anastomosis.

Hemophilia A is an X-linked bleeding disorder caused by a blood clotting protein factor VIII deficiency. Patients with hemophilia develop recurrent bleeding episodes. When bleeding occurs in the joints, hemophilic arthropathy (HA) may develop, resulting in hemarthroses and joint deformation. A novel congenic mouse model of severe hemophilia A was generated using CRISPR/CRISPR-associated protein 9 targeting of exon 1 of the F8 gene (F8em1-/-) to explore changes in the bleeding and inflammation during HA. F8em1-/- mice have a high penetrance of spontaneous bleeding, with joint bleeds progressing

Pyoderma gangrenosum associated with chronic refractory pouchitis: a case successfully treated with infliximab.

Pouchitis-associated pyoderma gangrenosum (PG) is rare, with only a few cases reported in the literature. Here we report a rare case of chronic refractory pouchitis-associated PG successfully treated with infliximab (IFX). A 43-year-old Caucasian male, with a past medical history of chronic refractory pouchitis after proctocolectomy and ileal pouch-anal anastomosis for severe ulcerative colitis, developed PG on his right lower leg. This subsided after treatment with intravenous IFX at a dose of 5 mg/kg at weeks 0, 2, 6 and then every 8 weeks. Pouchitis-associated PG is rare. Clinicians should

Endoscopic Evaluation and Management of Diversion Pouchitis and Strictures: In Comparison With Strictures in Nondiverted Pouches.

BACKGROUND AND AIMS: Postoperative complications after ileal pouch-anal anastomosis are common and may lead to pouch failure requiring long-standing fecal diversion. Patients with diverted pouches can develop diversion pouchitis and strictures; however, the endoscopic features and management of these conditions remain poorly characterized. METHODS: This historical case-controlled study included 54 consecutive inflammatory bowel disease patients with ileal pouch-anal anastomosis who underwent endoscopic evaluation and stricturotomy for distal pouch or anastomotic strictures from January 2020 t

Optimal Approaches to Treating and Preventing Acute and Chronic Pouchitis by Altering Microbial Profiles.

Pouchitis is the most common complication after restorative proctocolectomy with ileal pouch-anal anastomosis for ulcerative colitis. The authors review the role of the microbiota in both the pathogenesis of pouchitis, primarily via dysbiosis, as well as the resultant treatment strategies focused on correcting dysbiosis among patients with pouchitis. These include the role of antibiotics, probiotics, and potentially metabolomics in both treatment and risk stratification.
