Prazosin alone and combined with a beta-adrenoreceptor blocker in treatment of hypertension.

We recorded intra-arterial ambulatory blood pressure in 13 patients with essential hypertension before and after long-term twice-daily prazosin therapy (mean dosage 13.8 mg, SD 4.2 mg). Nine other patients with essential hypertension inadequately controlled with beta-adrenoreceptor blocking drugs were studied before and after the addition of long-term twice-daily prazosin therapy (mean dosage 8.8 mg, SD 6.7 mg). Ten patients, responders from both groups, then received once-daily prazosin, and intraarterial monitoring was repeated for a third time. Circadian curves from pooled hourly data showe

Clinical pharmacology of prazosin in hypertensive patients with chronic renal failure.

The clinical pharmacology of prazosin was studied in 10 hypertensive patients with chronic renal failure (group I) and in 9 hypertensive patients with normal renal function (group II). Prazosin, 2 mg, was given orally and blood samples were drawn at intervals for spectrofluorimetric assay. Blood pressure and heart rate were obtained at the same time. In the renal failure group, prazosin induced a significant decrease in systolic and diastolic blood pressures (-19 and -23%, respectively) at 90 min after intake, and these alterations were more rapid and marked than in the normal renal function g

Acute and chronic haemodynamic effects of prazosin in left ventricular failure.

We evaluated the acute and chronic effects of prazosin treatment in 11 patients with chronic congestive heart failure, NYHA functional class III and IV. Before treatment mean arterial pressure averaged 100 +/- 15 mmHg, left ventricular filling pressure 29 +/- 11 mmHg, and systemic vascular resistance 2372 +/- 1121 dynes s cm-5. Prazosin administration resulted in haemodynamic improvement in all but one patient with significant lowering of the mean arterial pressure, left ventricular filling pressure, and systemic vascular resistance. Nine patients completed a 10-week course of ambulatory treat

Antagonism of pre and postsynaptic alpha-adrenoreceptors by BE 2254 (HEAT) and prazosin.

1 The alpha-adrenoreceptor blocking effects of BE 2254 and prazosin have been studied in the pithed rat and guinea-pig ileum preparations. 2 BE 2254 was a competitive and potent antagonist of NA, Phenylephrine (PE) or stimulation-induced vasoconstriction in the pithed rat, but only exerted a weak antagonism of postsynaptic alpha 2-adrenoreceptor vasoconstriction induced by N,N-diMe-6,7-diOHATN (TL 99) or azepexole (BHT 933). The potency of BE 2254 against NA-induced vasoconstriction was markedly reduced by pretreatment with prazosin and propranolol. The antagonist effects of BE 2254 against TL

Effect of prazosin vs placebo on chronic left ventricular heart failure.

The effect of the vasodilator prazosin vs placebo on exercise duration until marked dyspnea, and on left ventricular function measured by echocardiography, was evaluated in a double-blind, randomized study in 24 patients with chronic left ventricular failure despite digitalis and diuretic therapy. Compared with the double-blind placebo, prazosin reduced resting systolic and diastolic blood pressure and systolic blood pressure times heart rate, improved clinical symptoms, decreased cardiothoracic ratio measured by chest roentgenography, decreased left ventricular and left atrial dimensions, imp

A case of prazosin in treatment of rapid eye movement sleep behavior disorder.

UNLABELLED: Rapid eye movement (REM) sleep behavior disorder (RBD) is characterized by dream-enactment behaviors that emerge during a loss of REM sleep atonia. Untreated RBD carries risks for physical injury from falls or other traumatic events during dream enactment as well as risk of injury to the bed partner. Currently, melatonin and clonazepam are the mainstay pharmacological therapies for RBD. However, therapeutic response to these medications is variable. While older adults are most vulnerable to RBD, they are also particularly vulnerable to the adverse effects of benzodiazepines, includ

Evaluation of low dose prazosin for PTSD-associated nightmares in children and adolescents.

INTRODUCTION: Knowledge about fundamental sleep disorders and dysregulation that occurs in children with PTSD is limited. Prazosin is an alpha-1 receptor antagonist often used off label for the treatment of PTSD-associated nightmares in adults; however, evaluation of its use in pediatrics and adolescents is limited. The primary objective of this study was to assess the impact of prazosin on nightmares associated with PTSD in this population. Secondary objectives included assessing side effects, changes in blood pressure, and 30-day readmission rates. METHODS: This was a retrospective, single-

Using Prazosin to Treat Posttraumatic Stress Disorder and Associations: A Systematic Review.

OBJECTIVE: A central adrenergic hyperactivation is described in the neurobiology of posttraumatic stress disorder (PTSD) with probable variable symptomatic impact. Few studies have evaluated using the alpha-1 adrenergic antagonist prazosin for such symptoms; however, given the likely pathophysiology involved, this drug may play an important role in the pharmacological approach to PTSD. METHODS: This study assessed articles already published on the use of prazosin through a systematic review along a timeline in view of the symptomatic target of difficult access by standardized treatments. The

Clinical Use of Prazosin in a Patient With Acute Stress Disorder: A Case Report.

Prazosin is an alpha-1 blocker that is commonly given to patients with posttraumatic stress disorder (PTSD) to reduce nightmares and flashbacks. Its use in acute stress disorder (ASD), however, has not been well characterized. There is a moderately positive correlation between ASD and the subsequent development of PTSD, which indicates that there may be some common neurobiological mechanisms that connect the 2 conditions. We present the case of a 51-year-old man who was experiencing symptoms of ASD following a motor vehicle accident that occurred a few days earlier. He was reporting flashbacks

Comparison of terazosin and prazosin for treatment of vesico-urethral reflex dyssynergia in dogs.

Nineteen dogs with vesico-urethral reflex dyssynergia (VURD) were treated with prazosin or terazosin 0.5 mg/kg twice daily to compare efficacy and side effects. Dogs were referred because of signs of (partial) urethral obstruction. Physical examination, abdominal ultrasonography, urinalysis and a radiographic contrast study of bladder and urethra (urethrocystography) were routinely performed. If no mechanical causes of obstruction or disease of the distal urinary tract were observed, the diagnosis VURD was presumed and the dogs were included in our study. Follow-up information was obtained fro

Prazosin for the treatment of nightmares related to posttraumatic stress disorder: a review of the literature.

BACKGROUND: Posttraumatic stress disorder (PTSD) is a psychiatric disorder with symptoms that include insomnia due to hyperarousal and recurring nightmares. These symptoms are believed to be due to a conditioned response that is regulated by norepinephrine. Prazosin, an α(1) antagonist, can decrease levels of norepinephrine in the central nervous system, thereby reducing nightmares related to PTSD. DATA SOURCES: A literature search was conducted for all studies evaluating the effectiveness of prazosin as therapy for nightmare symptoms of PTSD. MEDLINE was utilized to identify all English-lang

Advances in the design and synthesis of prazosin derivatives over the last ten years.

INTRODUCTION: Mechanistic, translational and pharmacological studies led to the identification and discovery of the preferred localization, binding characteristics, structure and functional properties of α1-adrenoceptor (α1-AR) subtypes in the bladder neck, bladder and prostate gland. The evidence gathered on α1-ARs, provided a molecular platform for the development of subtype-selective antagonists, resulting in more effective approaches targeting those receptors for the treatment of outlet bladder obstruction and benign prostate hyperplasia. In vitro electrostimulation-induced urethral relaxation in the guinea pig is blocked by prazosin.

The purpose of this study was to characterize the electrostimulation-induced relaxation in guinea pig urethral rings. From sacrificed male guinea pigs, urethral rings were cut between the bladder neck and the penile crura and mounted in an in vitro bath. The drop in baseline tension in response to electrical stimulation (rectangular pulses of 0.8-msec pulse duration, a frequency of 3, 5, 10, and 20 Hz, and 75-mA current) was measured in the presence of various pharmacologic agents. In urethral tissue precontracted with 10(-5) norepinephrine, a significant relaxation of 34.8% was found at 10 Hz

Cardiovascular and side effects of flesinoxan in conscious hypertensive dogs. Modulation by prazosin.

Previous studies on anaesthetized animals indicate that flesinoxan exerts hypotensive effects via stimulation of central 5-HT1A receptors. The purpose of the present study was to investigate the cardiovascular and side effects of flesinoxan in conscious, renal hypertensive dogs at rest and during exercise. Animals were pretreated with prazosin (2.5 or 7.5 nmol/kg) to verify a reduction of dose-dependent side effects, as occurred in normotensive dogs. A decrease in blood pressure without reflex tachycardia was observed only with the lower dose of flesinoxan (0.1 mumol/kg). The higher dose (0.2

[Therapeutic effectiveness and safety of prazosin in the symptomatic treatment of benign prostatic hypertrophy].

74 patients diagnosed as suffering from benign prostatic hypertrophy (BPH) were treated with prazosin for 8 weeks. We look at the evolution of the symptomatology and the urodynamic and morphologic parameters during the treatment and they are compared with the basal ones. The irritative symptoms (nocturia, pollakiuria, imperiosity and dysuria), evaluated by means of a quantitative scale, undergo significant statistical improvement. The same occurred with the obstructive symptoms. Urodynamically, an increase took place in total vesical capacity, maximum, and mean flow, as well as a drop or reduc
