This case report describes selective revascularization of the superior mesenteric artery in a patient in the third trimester of pregnancy with probable Marfan syndrome and an acute complicated type B aortic dissection. Open surgical revascularization was preferred over endovascular intervention, given the patient's anatomic and clinical high-risk features for retrograde type A aortic dissection.

Small bowel obstruction (SBO) is a rare yet life-threatening event in pregnancy. If not diagnosed and treated promptly, it can lead to an increase in maternal as well as foetal morbidity and mortality. OBJECTIVE: To evaluate the length of the third stage of labor in second trimester pregnancies, to determine the rate of complications such as hemorrhage or infection, and to determine the ideal timing for delivery of the placenta in these pregnancies.
STUDY DESIGN: A review of 121 consecutive vaginal deliveries between 16 and 27 gestational weeks was performed. Length of the third stage of labor was determined, as well as the composite endpoint of need for manual or instrumented removal of the placenta, or development of chorioamnionitis or endometritis.

Primary hyperparathyroidism (pHPT) in pregnancy may be associated with significant maternal and fetal morbidity and mortality. Medical management of pHPT in pregnancy is limited, and surgery is the only definitive therapeutic option. The ideal timing for surgery is mid-second trimester, but surgery may also be safely performed in the third trimester. Delayed parathyroid surgery may result in a hypercalcaemic crisis postpartum owing to loss of active placental calcium transfer. Subglottic stenosis is seldom encountered during pregnancy. Definitive treatment is tracheal resection and reanastomosis, although not during the later stages of pregnancy. We describe 2 patients who presented during the third trimester of pregnancy with significant tracheal obstruction from subglottic stenosis, one caused by Wegener granulomatosis and the other idiopathic in nature. Both patients refused temporary tracheostomy. We describe the surgical and anesthetic management of these patients subsequent to which they were both able to have normal, full-term vaginal deliveries.

The simultaneous occurrence of maternal primary hyperparathyroidism (PHPT) and acute pancreatitis during pregnancy is very rare. We report a 31-year-old woman with a relapse of multiple sclerosis during the third trimester of pregnancy. She had been treated palliatively after the relapse until occurrence of a new symptom and new lesions on MRI, and then she had Cesarean section. After surgery, steroid pulse therapy was started and the symptoms and lesions on MRI resulted in improvement. As a strategy to treat patients with multiple sclerosis in their third trimester of pregnancy, it is appropriate to evaluate disease activity using MRI and to start steroid pulse therapy after delivery.

Intracranial neoplasms presenting during pregnancy are uncommon. We report the case of a woman with a giant acoustic neuroma, presenting with hyperemesis gravidarum, and detail the surgical excision of the tumour during the third trimester.
