Transient Passovoy defect during a febrile illness.

The Passovoy defect is a recently characterized hemorrhagic diathesis. We describe a patient with a febrile illness, possibly from Epstein-Barr (EB) virus, who acquired this defect transiently. Prothrombin time; assays for factors VIII, IX, XI, XII; and Fletcher (prekallikrein) and Fitzgerald (high molecular weight kininogen) factors were normal. No definite circulating inhibitor could be demonstrated. The transient Passovoy defect could possibly be ascribed to the infectious process or sulfisoxazole, which the patient had received.

Reduced plasma kininogen concentration during sickle cell crisis.

Eight patients with sickle cell anemia (SS hemoglobin) were found to have decreased plasma levels of kininogen compared to normal control subjects. The kininogen level of the patients with sickle cell anemia was decreased further during sickle cell crises. The results suggest that components of the kinin system are profoundly affected in patients with sickle cell anemia, and during crises may play a role in the clinical presentation of patients.

A new player in hemolytic uremic syndrome: the plasma kallikrein-kinin system, a target of heme.

The study by Gerogianni et al. demonstrates activation by heme of coagulation factor XII and plasma prekallikrein in vitro, resulting in the release of the bioactive peptide kinins. It further shows that the plasma kallikrein-kinin system is activated in hemolytic uremic syndrome and suggests that heme is involved in this effect. The commentary examines the pathophysiological significance of these findings, discusses the translation of the biochemical findings to clinical situations, and considers the putative clinical consequences of plasma kallikrein-kinin system activation in hemolytic urem

Modulation of plasma exudation by PGE2 and that of leukocyte migration by LTB4 in inflammatory models.

Des-Phe8-Arg9-BK could be detected in the entire course of rat carrageenin pleurisy up to 24 h, together with a reduction of the residual levels of high molecular weight kininogen and prekallikrein. On the basis of this continuous release of bradikinin, prostaglandin E2 was released up to 5 h in the pleural exudate and enhanced the plasma leakage. In rat cardiac infarction, the initial increase in the number of polymorphonuclear leukocytes in the cardiac tissue was accompanied by leukotriene B4 in the tissue and this was followed by the second increase and activation of the complement system.

The Effect of Serine Proteases and Their Inhibitors on In Vitro Fertilization Outcomes.

BACKGROUND: Proteolysis imbalance can result in infertility. The purpose of the current study was to investigate the function of kininogen, neutrophil elastase, trypsin-like proteases, α1-protease inhibitor (α1-PI), and α2-macroglobulin (α2-MG) in blood, saliva, and follicular fluid as an indicator of the success of in vitro fertilization (IVF). METHODS: In general, twenty-one women undergoing an IVF cycle, as well as a control group consisting of ten healthy women, were included. The proteinase activity was measured by the rate of hydrolysis of specific substrates. The proteinase inhibitor a

A model of zymogen factor XII: insights into protease activation.

In plasma, the zymogens factor XII (FXII) and prekallikrein reciprocally convert each other to the proteases FXIIa and plasma kallikrein (PKa). PKa cleaves high-molecular-weight kininogen (HK) to release bradykinin, which contributes to regulation of blood vessel tone and permeability. Plasma FXII is normally in a "closed" conformation that limits activation by PKa. When FXII binds to a surface during contact activation it assumes an "open" conformation that increases the rate of activation by PKa. Mutations in FXII that disrupt the closed conformation have been identified in patients with con

Proteolytic properties of single-chain factor XII: a mechanism for triggering contact activation.

When blood is exposed to variety of artificial surfaces and biologic substances, the plasma proteins factor XII (FXII) and prekallikrein undergo reciprocal proteolytic conversion to the proteases αFXIIa and α-kallikrein by a process called contact activation. These enzymes contribute to host-defense responses including coagulation, inflammation, and fibrinolysis. The initiating event in contact activation is debated. To test the hypothesis that single-chain FXII expresses activity that could initiate contact activation, we prepared human FXII variants lacking the Arg353 cleavage site required

Comparative incidence of thrombosis in reported cases of deficiencies of factors of the contact phase of blood coagulation.

Thrombotic manifestations occurring in patients with coagulation defects have drawn considerable attention during the last decade. It concerned mainly patients with hemophilia, vW disease or FVII deficiency. Occasional reports involved also the deficiencies of the contact phase of blood coagulation, mainly FXII deficiency. The purpose of the present study was to evaluate the comparative incidence of thrombosis in all reported patients with FXII, Prekallikrein and Kininogens deficiencies. Out of the reported 341 cases with these conditions that could be tracked there were 43 cases with thrombos

Prokallikrein and kallikrein inhibitors in human urine.

Kallikrein activity (KA) and kallikrein inhibitors (KI) were evaluated in urine from normal subjects and essential hypertensives. Kallikrein inhibitors were almost completely removed by dialysis against water, confirming previous reports of their low molecular size. The negative relationship found between KA and KI in urine samples from 12 normal subjects suggests that KI might play a role in the modulation of KA excreted by the kidney. Heating (85 degrees C for 20 min) partially reduced (-52%) KI, indicating thermostable substances other than peptides as KI. When cations were evaluated as pos

The study of local fibrinolysis in abortion.

The etiology of abortion, which is still a moot question, is now under intense investigation. The present study discussed the possible role of local fibrinolysis (villous tissue fibrinolysis) in the pathogenesis of abortion by studying tissue fibrinolytic activity in tissue culture as well as blood coagulation, fibrinolysis and the kinin system. Subjects studied include: 1) normal villous tissue (n=46), early pregnancy loss, whose fetal cardiac activity had once been identified (n=22) designated as Group A, and Group B whose fetal cardiac activity had never been identified (n=82). The villous

Relation between renin and prorenin in plasma from hypertensive patients and normal people: evidence for different renin:prorenin ratios.

Renin and prorenin concentrations in plasma correlate closely, but the proportion of active renin to prorenin shows some variation in different forms of hypertension. The proportion of active renin to prorenin is higher in normal renin essential hypertensives and in patients with renal hypertension than in normal subjects, and it is lower in low renin essential hypertension and primary hyperaldosteronism. Plasma deficient in plasma prekallikrein shows a lower proportion of active renin than might be expected. Active renin and plasma angiotensin II concentration show a strong correlation while

Activation of the kallikrein-kinin system in Rocky Mountain spotted fever.

Activation of the kallikrein-kinin system, as indicated by increased plasma kallikrein and depleted plasma kininogen, prekallikrein, and kallikrein inhibitor, was observed in five patients with Rocky Mountain spotted fever. Four of the patients had petechial rashes characteristic of vasculitis. Three patients had alterations in coagulation consistent with disseminated intravascular coagulation, although no hemorrhagic syndrome was found. Our data, along with the known physiologic actions of kinins, suggest a possible role for the kallikrein-kinin system in the pathophysiology of vasculitis, di

High-molecular-weight hydroxyethyl starch accelerates kallikrein-dependent clot initiation.

BACKGROUND: A decrease in reaction time (R; seconds) has been considered a thrombelastographic hallmark of hypercoagulability. However, the cause of changes in R has been influenced by the method of activation (e.g., celite) and the clinical/laboratory setting (e.g., hemodilution). Although antithrombin deficiency has been implicated as a cause of decreased R in unactivated samples after crystalloid dilution, dilution with hydroxyethyl starch (HES) solutions such as Hextend (6% HES solution; average molecular weight 450 kDa) or Voluven (6% HES solution; average molecular weight 130 kDa) has de

Serotonin vascular reactivity and other clinical pharmacological investigations in two intestinal carcinoid patients.

In two patients with carcinoid syndrome, a reduced venoreactivity to serotonin (5-HT) was found by means of the venoconstriction test. Parachlorophenylalanine (PCPA) treatment increased 5-HT venoreactivity. A normal noradrenaline venoreactivity was detected before and during PCPA administration. The altered platelet monoamine oxidase (MAO) activity corrected by PCPA treatment. The abnormal 5-HT turnover may alter the synthesis of MAO and provoke the production of isoenzymes. Low plasma amine oxidase and high plasma histaminase activity were found in 1 of these 2 patients. A low level of plasma

Factor XII interacts with the multiprotein assembly of urokinase plasminogen activator receptor, gC1qR, and cytokeratin 1 on endothelial cell membranes.

Investigations were performed to define the factor XII (FXII) binding site(s) on cultured endothelial cells (HUVECs). Biotin- or fluorescein isothiocyanate (FITC)-FXII in the presence of 10 microM Zn(2+) specifically binds to HUVEC monolayers or cells in suspension. Collagen-stimulated platelets release sufficient Zn(2+) to support FXII binding. On laser scanning confocal microscopy or electron microscopy, FITC-FXII or Nanogold-labeled FXII, respectively, specifically bind to HUVECs. Antibodies to gC1qR, urokinase plasminogen activator receptor (uPAR) and, to a lesser extent, cytokeratin 1 (CK
