Modified mandated choice for organ procurement.

Presumed consent to organ donation looks increasingly unlikely to be a palatable option for increasing organ procurement in the UK following the publication of the report into events at Alder Hey and elsewhere. Yet, given that the alternative to increasing the number of cadaveric organs available is either to accept a greater number of live donations, or accept that people will continue to die for the want of an organ, public policy makers remain obliged to consider other means of increasing the procurement rate. In this paper, we meet the main objections to mandated choice (namely that it und

Louisiana neurosurgeons and the organ procurement shortage: a comparative analysis of perceptions, attitudes, and ethical values.

The supply of transplantable organs continues to fall behind demand. Knowledge of the perceptions, attitudes, and ethical values held by health care workers involved in the process is critical. This is particularly true of neurosurgeons because of their pivotal role in the procurement process. Survey data are used to compare neurosurgeons (n = 59) with emergency physicians (n = 125), critical care nurses (n = 267), and other physicians (n = 445). Analysis revealed that attitudes among those health care workers regularly involved with organ procurement and transplantation--neurosurgeons, emerge

The impact of third-party authorization requirements on abortion-related outcomes: a synthesis of legal and health evidence.

OBJECTIVES: This review synthesizes legal and health evidence to demonstrate the health and human rights impacts of third-party authorization requirements (TPAs) on abortion seekers. RESULTS: The synthesized evidence substantiates the pre-existing position in international human rights law that requirements that abortion be authorized by third parties like parents, spouses, committees, and courts create barriers to abortion, should not be introduced at all, or should be repealed where they exist. CONCLUSIONS: The review establishes that rights-based regulation of abortion should not impose T

A case of carcinoid likely causing jejunal intussusception.

A 34-year-old female was admitted to Emergency Department with an abrupt onset of diffuse abdominal pain. A CT scan done prior to her transfer revealed significant dilated loops of bowel as well as multiple target signs with likely torsed bowel. The patient consented to an exploratory laparotomy. During surgery, the proximal jejunum was found to be intussuscepted, a rare finding in an adult. There was evidence of mesenteric foreshortening throughout the small bowel and multiple whitish lesions within the mesentery, both consistent with the desmoplastic response that is characteristic of carcin

Transplantation of organs: a European perspective.

The development of transplantation technology increasingly places before society a multitude of diverse, complex ethical and legal problems. The subject is the more complex because of the various divergent interests involved. There are the interests of the donor of organs, who has a right to protection of his legal position, and those of the patient in need of an often lifesaving organ. There are also the interests of the donor's relatives, after his death, and those of the transplantation surgeons. The international dimension of transplantation technology makes the picture even more complicat

From posthumous-assisted reproduction to early sperm preservation: Israeli men's perspectives on reproductive decision-making.

RESEARCH QUESTION: During a time of armed conflict and rising posthumous sperm retrievals in Israel, what are Israeli Jewish men's attitudes toward elective sperm preservation and posthumous-assisted reproduction (PAR), and which motivations and sociodemographic factors are associated with these attitudes? DESIGN: A cross-sectional study of 402 Israeli Hebrew-speaking Jewish men (ages 18-40 years) was conducted between 2025 and 2026. The questionnaire assessed demographics, role in the conflict and attitudes toward sperm preservation and PAR. Multivariable logistic regression analysis was use

Incision and drainage versus aspiration of fluctuant buboes in the emergency department during an epidemic of chancroid.

GOAL OF THIS STUDY: To compare outcomes of incision and drainage with packing to needle aspiration of purulent buboes in patients with presumed chancroid. The safety and efficacy of incision and drainage were compared with that of needle aspiration. STUDY DESIGN AND METHODS: The study was a prospective randomized, nonblinded clinical trial carried out from April 1992, to January 1994 in an inner city emergency department. A consecutive sample of 27 adults with fluctuant inguinal buboes presumed or proven to be chancroid or lymphogranuloma venereum were eligible for inclusion. Patients were ex

Beyond permission: Reconsidering anatomy education through a contemporary Islamic fatwa in Türkiye.

The ethical use of human body donors in anatomy education is shaped by scientific and professional standards as well as cultural, societal, and religious contexts. In Türkiye, body donation remains limited, and anatomical institutions continue to rely partly on unclaimed and imported bodies, within a setting shaped by legal, cultural, and religious considerations. In December 2025, the High Board of Religious Affairs under the Presidency of Religious Affairs in Türkiye issued a religious ruling (fatwa) defining conditions under which the use of human bodies for medical education is permissible

Values of ultrasonography, sestamibi scintigraphy, and intraoperative measurement of 1-84 PTH for unilateral neck exploration of primary hyperparathyroidism.

Unilateral neck exploration (UNE) is a controversial approach to the treatment of primary hyperparathyroidism (PHP), and most surgeons favor bilateral neck exploration. The aim of this study was to assess the value of ultrasonography, sestamibi scintigraphy, and intraoperative measurement of urinary cyclic AMP (UcAMP) or 1-84 PTH in 200 patients undergoing unilateral neck exploration under local anesthesia. Conditions for UNE were (1) a presumed solitary adenoma detected by ultrasonography, (2) no thyroid disease, and (3) no family history of PHP or multiple endocrine neoplasia. Patient's cons

Right Anterior Sectionectomy for Hepatic Helminthic Infection Mimicking Cholangiocarcinoma: A Case Report.

We present the case of a patient with an unexpected pathological finding of degenerate roundworm infection on histology for a liver resection for presumed intrahepatic cholangiocarcinoma. Helminthic infection of the liver is uncommon in developed and metropolitan regions and this patient had no risk factors for such. The only potentially risk factor for complications of the infection was the presence of long-term immunosuppression in the setting of Crohn's disease. The imaging, including computer tomography (CT), magnetic resonance imaging (MRI), and fluorodeoxyglucose positron emission tomogr

Incidental Neoplasm and Histopathological Spectrum of Suspected Acute Appendicitis in Appendectomy Specimen: An Observational Study.

INTRODUCTION: Acute appendicitis is the most common surgical emergency and histopathological studies are the gold standard for confirmation of clinical diagnosis and key methods of discovering appendiceal neoplasm. Appendiceal neoplasms are uncommon and are mostly identified by pathologic examination after appendectomy for presumed appendicitis as an incidental finding. METHODS: An observational cross-section study was conducted at a Medical College from 1st August 2022 to 31st July 2024. Appendectomy specimens were included in the study after obtaining consent. Guidelines for the appropriate use of do-not-resuscitate orders. Council on Ethical and Judicial Affairs, American Medical Association.

Cardiopulmonary resuscitation (CPR) is routinely performed on hospitalized patients who suffer cardiac or respiratory arrest. Consent to administer CPR is presumed since the patient is incapable at the moment of arrest of communicating his or her treatment preference, and failure to act immediately is certain to result in the patient's death. Two exceptions to the presumption favoring CPR have been recognized, however. First, a patient may express in advance his or her preference that CPR be withheld. If the patient is incapable of expressing a preference, the decision to forgo resuscitation m

Public attitudes and behavior regarding organ donation.

Organ transplantation has been the subject of much attention; unfortunately, relatively little has been published about public attitudes toward organ donation. To better document public perceptions on organ donation, a telephone survey of a nationally representative sample of 2,056 respondents was conducted. We found that nearly 94% of the population had heard about organ transplantation, but only 19% of these people carried donor cards. The results indicated that people are somewhat more likely to donate the organs of a relative who had just died (53%) than they are to donate their own organs

A cough conundrum in a patient with a previous history of BCG immunotherapy for bladder cancer.

We describe a non-smoker who presented with a persistent cough, weight loss and general malaise, and had a medical history of bladder carcinoma that had been successfully treated with intravesical BCG immunotherapy. Radiology revealed hilar lymphadenopathy, a predominantly mid-zone and lower-zone lung parenchymal nodular pattern with a perilymphatic distribution, a few thickened interlobular septae, and small pleural effusions bilaterally. The T-SPOT.TB blood test was negative. Video-assisted thoracoscopic surgery showed multiple pleural nodules, the histopathology of which showed multiple wel

The role of specialist nurses for organ donation: A solution for maximising organ donation rates?

AIMS AND OBJECTIVES: To explore the role that Donor Transplant Co-ordinators have played and the future potential of Specialist Nurses for Organ Donation (SN-ODs), within organ donation strategies in the UK and other countries. BACKGROUND: Organ donation and transplantation rates vary extensively around the world. However, there is a universal shortage of deceased donors, prompting different approaches to increase transplantation rates. Within the UK, the Clinical Lead for Organ Donation and Specialist Nurse in Organ Donation undertake a key role in the implementation of the Organ Donation St

Family Over Rules? An Ethical Analysis of Allowing Families to Overrule Donation Intentions.

Millions of people want to donate their organs after they die for transplantation, and many of them have registered their wish to do so or told their family and friends about their decision. For most of them, however, this wish is unlikely to be fulfilled, as only a small number of deaths (1% in the United Kingdom) occur in circumstances where the opportunity to donate organs is possible. Even for those who do die in the "right" way and have recorded their wishes or live in a jurisdiction with a "presumed consent" system, donation often does not go ahead because of another issue: their familie

Legal briefing: organ donation and allocation.

This issue's "Legal Briefing" column covers legal developments pertaining to organ donation and allocation. This topic has been the subject of recent articles in JCE. Organ donation and allocation have also recently been the subjects of significant public policy attention. In the past several months, legislatures and regulatory agencies across the United States and across the world have changed, or considered changing, the methods for procuring and distributing human organs for transplantation. Currently, in the U.S., more than 100,000 persons are waiting for organ transplantation. In China, m

Choroidal neovascularization in a young, healthy eye after LASIK.

BACKGROUND: Choroidal neovascularization (CNV) is associated with age-related macular degeneration (AMD), degenerative myopia, angioid streaks, presumed ocular histoplasmosis syndrome, and numerous other ocular and systemic conditions. Idiopathic CNV is also a common form of the condition. However, CNV has rarely been noted after laser-assisted in situ keratomileusis (LASIK) and a cause-and-effect relationship has not been found. A case of CNV after uneventful LASIK is reported here, along with a literature review of previously reported cases and suggested pathophysiology. A link between LASIK
