Periodontal considerations in operative dentistry.

Periodontal and preventive considerations, other marginal fit and occlusion, are discussed as they relate to the fabrication of restorations in operative dentistry. The development of buccal, lingual, proximal, and transitional line contours is presented as related to maintenance of healthy periodontal tissues. Proximal contacts and marginal ridge relationships are related to food retention and arch integrity. The gingival attachment is described, as it determines certain techniques of tooth preparation and impression making.

[Preventive oral healthcare for children: towards appropriate care].

In recent decades, oral healthcare has shifted from curation to prevention. Preventive oral healthcare for children consists mainly of oral health education to improve health behaviors and clinical prevention such as fissure sealants, application of fluoride varnish, removal of supragingival plaque and dental polishing. In addition, the periodic clinical examination is an important preventive instrument. These 5 preventive interventions for children up to 12 years of age can be discussed using the principles of 'appropriate care'. Appropriate care is an approach of the Dutch government to ensu

[Caries-- changing patterns].

This article briefly describes the pathogenesis of dental caries. It focuses on the relevant chemical processes and histological changes in dental enamel. Recent studies have shown that the mineral balance at the tooth-oral fluid interface are determinants for the occurrence of caries, and more important than the intrinsic tissue properties. During periods of low pH, following bacterial metabolism of fermentable carbohydrates, enamel is demineralized. However, loss of tissue may be compensated by mineral deposition from saliva during subsequent periods at neutral pH ('remineralization'). The m

Maintenance of space by innovative three-dimensional-printed band and loop space maintainer.

Preventive orthodontics is an aspect of pediatric dentistry, which requires a unique set of skills and understanding to aid the patient's treatment and properly align teeth. The safest way to prevent future malocclusions from tooth loss is to place a space maintainer (SM), which is advocated to hold the space until the eruption of permanent teeth. SM manufacturing is a laborious process that requires ample communication with the laboratory to properly complete and provide the patient with optimal results. Band and loop SM is mostly indicated for the premature loss of single primary molar, but

Evaluation of an oral preventive protocol in children with acute lymphoblastic leukemia.

This study was designed to assess the effectiveness of a preventive oral protocol in children receiving antineoplastic treatment for acute lymphoblastic leukemia (ALL) before initiating a larger intervention study. During a seven month period, fourteen children from two to ten years old with a diagnosis of ALL were evaluated. Patients with ALL who received a 0.12% chlorhexidine mouth rinse (seven children) were compared to a control group of patients who were not given the same preventive treatment (seven children) as to the occurrence of oral mucosal complications. Children in both groups rec

Preventive periimplant protocol.

The primary responsibility of the clinician treating a patient with implant dentistry is to provide a long-term favorable prognosis. In consideration of changes in implant materials and restorative designs, familiarity with the relationship of the periimplant tissues to the implant components is essential. Maintaining healthy tissues requires understanding the influence of the intraoral bacterial flora, recognizing deviations from health, and initiating an appropriate preventive protocol. Various implements that are available for plaque control and debridement are discussed.

Sealants and preventive restorations: review of effectiveness and clinical changes for improvement.

Sealants are effective caries-preventive agents to the extent they remain bonded to teeth. Preventive resin restorations (PRR) have a proven record, but are susceptible to failure as the overlying sealant fails. Careful analyses of studies reveal a measurable failure rate of sealants (5-10% per year) that must be addressed. Even under the best of circumstances, sealants fail. Therefore, dentistry (as well as third-party systems) must accept that sealants need vigilant recall and proper preventive maintenance. In addition, it is clear that cost-effective use of sealants will involve selective a

Analysis of pulpal reactions to restorative procedures, materials, pulp capping, and future therapies.

Every year, despite the effectiveness of preventive dentistry and dental health care, 290 million fillings are placed each year in the United States; two-thirds of these involve the replacement of failed restorations. Improvements in the success of restorative treatments may be possible if caries management strategies, selection of restorative materials, and their proper use to avoid post-operative complications were investigated from a biological perspective. Consequently, this review will examine pulp injury and healing reactions to different restorative variables. The application of tissue

Eminent science support from Jews to the young Turkish republic; a particular focus on dentistry.

During World War II, millions of people were mistreated and imprisoned in Nazi concentration camps. Due to the antisemitic pressure applied by the Nazi regime, many scientists had to leave Germany, and they immigrated to the United States, Switzerland, Turkey or South America. Alfred Kantorowicz was among those highly educated people who were forced out of their professional career. For a certain period, he had to stay away from the world of research and academia, which were his favorite occupations. However, these unexpected difficulties did not prevent him to pursue his success story with ma

Utilization Patterns and Clinical Indications of General Anesthesia in Pediatric Dentistry: A Systematic Review.

Background: General anesthesia (GA) plays a key role in pediatric dentistry by enabling comprehensive dental treatment in children who cannot be adequately managed using conventional behavioral techniques, local anesthesia, or sedation. While previous reviews have primarily focused on safety outcomes and adverse events, less attention has been given to patterns of GA utilization and their broader clinical and public health implications. Objective: The objective was to synthesize and critically analyze contemporary evidence on utilization patterns, clinical indications, and treatment characteri

The impact of climate change on paediatric dentistry.

The effects of global warming and climate change are happening at a faster rate than expected, and they are going to get worse. Global climate change has already started to show the effects on environment-fast melting glaciers, accelerated sea level rise and base shifting of native flora and fauna. There has been an increase in temperature globally with a few countries already showing intense heat waves and extreme cold temperatures. The interrelation between dentistry, environmental impact and human health is still in its nascent stage, but studies in medicine show that the healthcare industr

[Shock waves as a preventive impulse : New approaches to reducing surgical and postoperative complications, including tissue regeneration].

Extracorporeal shock wave therapy (ESWT) has evolved from its original use in the disintegration of kidney stones to become an innovative therapeutic modality in many different medical disciplines. While the focus used to be on treating existing pathologies, the literature now shows a growing interest in both prophylactic and preventive applications. This review systematizes the evidence for prophylactic ESWT strategies to prevent or minimize complications from surgical interventions. The article covers applications in orthopedics, dentistry, plastic surgery, and wound healing.

Evidence-Based Deep Carious Lesion Management: From Concept to Application in Everyday Clinical Practice.

This chapter aims to discuss the way dental practitioners manage deep carious lesions (DCL) in routine practice and the barriers and incentives/facilitators to changes in accepted practice. In concert with the philosophy of minimal intervention dentistry, the concept of quaternary prevention (actions taken to prevent overtreatment and reduce harm to the patient) emerges in dentistry. This can be applied to carious tissue removal (CTR) in DCL given the risks associated with this procedure (high risk of pulp exposure, pulpal complications, tooth substance loss, and unsuccessful invasive, expensi

Preventive resin restoration is minimally invasive procedure that should be the treatment of choice for small carious lesions in the posterior teeth. The Class I amalgam should not be placed as a first-time restorative material to treat incipient or small carious lesions under any circumstances. Life-threatening pathogens in severe/progressive periodontitis: Focal infection risk, future periodontal practice, role of the Periodontology 2000.

Severe/progressive periodontitis is associated with cardiovascular disease, cancer, Alzheimer's disease, and dozens of other serious diseases. Herpesviruses are implicated in severe periodontitis and in specific subsets of each of the above systemic diseases. That both periodontitis and herpesviruses are linked to the same nonoral diseases is consistent with a systemic pathogenic role of periodontal herpesviruses. Effective control of periodontitis-related systemic diseases requires collaboration between dentistry and medicine. Periodontology has emerged as an important preventive medical disc
