Cost-benefit analysis of a controlled trial of nurse therapy for neuroses in primary care.

In a randomized, controlled trial neurotic patients (mainly phobics and obsessive-compulsives) in primary care were assigned to behavioural psychotherapy from a nurse therapist or to routine care from their general practitioner. At the end of one year clinical outcome was significantly better in patients cared for by the nurse therapist. Economic outcome to one year, compared with the year before entering the trial, showed a slight decrease in the use of resources by the nurse therapist group (N = 22), and an increase in resource usage in the GP-treated group (N = 28), mainly due to the latter

Use of restriction enzymes to investigate the source of a primary cytomegalovirus infection in a pediatric nurse.

The risk of transmission of cytomegalovirus (CMV) infection from congenitally infected infants to nonimmune medical attendants is unknown. The case of a CMV-seronegative, pregnant nurse who seroconverted after caring for an infant with symptomatic CMV infection is reported. She elected to be aborted and the fetal tissue contained CMV. Isolates from the nurse, the fetal tissue, and the infant to whom the nurse was exposed were examined for genetic relatedness by restriction enzyme analysis. As expected, the isolates from the nurse and the fetal tissue were identical. However, the virus isolated

Integrating Nursing-Teleophthalmology Improves Diabetic Retinopathy Screening in Primary Healthcare, Reducing Unnecessary Referrals to Specialist Healthcare.

AIM: This work aimed to evaluate the use of teleophthalmology by a primary healthcare nursing team for the diagnosis and referral for diabetic retinopathy to specialized healthcare in relation to numbers referred for specialized healthcare. METHODS: In this quantitative, cross-sectional service evaluation study, participants with type 2 diabetes mellitus underwent a fundoscopy examination between February and June 2020. Using a portable retinal camera attached to a smartphone, nurses acquired fundus images that were stored on a cloud platform, enabling remote reading by a retinal specialist.

Primary Impact Trajectories of a Psychoeducational Program for Nursing Home Caregivers: Insights from Clinical Case Studies.

The decision to place a loved one with dementia in a specialized facility is often associated with significant psychological distress among family caregivers, which often manifests itself in feelings of guilt, helplessness, and emotional tension. We conducted a pilot implementation of a seven-session psychoeducational group program designed to support family caregivers during the critical transition from home care to nursing home placement. The intervention included weekly modules addressing fundamental topics such as how nursing homes operate, managing behavioral disorders, end-of-life consid

[Effectiveness of a Multimodal Nursing Intervention for Primary Caregivers in Preventing Medical Adhesive-Related Skin Injury].

BACKGROUND: Although medical adhesive products are routinely used in clinical settings, research into both the medical-adhesive-associated skin outcomes of patients and the caregiving practices of patients and their primary caregivers remains limited. PURPOSE: The primary aim of this study was to evaluate the effectiveness of a multi-strategy nursing educational program in improving knowledge of medical adhesive-related skin injury (MARSI), health beliefs, self-efficacy, and skin care behaviors among primary caregivers of ostomy patients. In addition, peristomal transepidermal water loss (TEW

Inappropriate use of antiplatelet agents for primary prevention in nursing homes: An Italian multicenter observational study.

AIM: The benefits of antiplatelets to prevent cardio- and cerebrovascular disease are recognized and appropriate only for "secondary prevention." This multicenter retrospective study was designed to (i) examine the prevalence of residents receiving antiplatelets for primary and secondary cardio- and cerebrovascular prevention, and (ii) evaluate the predictors of inappropriate antiplatelet prescription. METHODS: This study was conducted in a sample of Italian long-term care nursing homes (NHs). Appropriate use of antiplatelets was defined in accordance with the strongest evidence-based indicat

The prevalence of primary headache disorders and their associated factors among nursing staff in North China.

BACKGROUND: Epidemiological data on the prevalence of headache in nursing staff in Mainland China are lacking. We therefore performed a study to assess the prevalence of headache, and factors associated with headaches, in nursing staff in three hospitals in North China. METHODS: Stratified random cluster sampling was used to select 1102 nurses from various departments in three hospitals. A structured questionnaire was used to collect epidemiological data, headache characteristics and associated factors. RESULTS: The response rate was 93.0%. Among nursing staff, the 1-year prevalence of prima

Where there is no nurse: an observational study of large-scale mentoring of auxiliary nurses to improve quality of care during childbirth at primary health centres in India.

OBJECTIVE: Clinician scarcity in Low and Middle-Income Countries (LMIC) often results in de facto task shifting; this raises concerns about the quality of care. This study examines if a long-term mentoring programme improved the ability of auxiliary nurse-midwives (ANMs), who function as paramedical community health workers, to provide quality care during childbirth, and how they compared with staff nurses. DESIGN: Quasi-experimental post-test with matched comparison group. SETTING: Primary health centres (PHC) in the state of Bihar, India; a total of 239 PHCs surveyed and matched analysis b

Developing a Basic Package of Oral Care Training Program for Primary Health Care Nurses.

INTRODUCTION: Nurses are key in the delivery of the basic package of oral care in the primary health care system. Whereas oral health care training is essential for apt oral care delivery, the scope and training approaches for the primary health care nurse are not well established. A mentorship approach has rarely been explored for oral health training. OBJECTIVE: This study aimed to develop a basic package of oral health care training program for primary health care nurses in Kenya. It also evaluated and compared the effects of workshop and mentorship approaches on a basic package of oral ca

A Phenomenological Study of Primary Healthcare Nurses' Experiences in Assessing and Managing Diabetic Foot Complications.

BACKGROUND: In South Africa, diabetic foot complications (DFC) often present for the first time at the primary healthcare (PHC) level, where nurses are central to diabetes management and prevention. Despite their key role, there are limited data on nurses' experiences in managing DFC, particularly in resource-constrained primary healthcare (PHC) settings. This study's objective was to explore and describe nurses' experiences in managing DFC at the PHC level, with the aim of informing future interventions and policy development. METHODS: A descriptive phenomenological study was conducted with

Effects of task-shifting from primary care physicians to nurses: an overview of systematic reviews.

BACKGROUND: Task-shifting from primary care physicians (PCPs) to nurses is a means of overcoming PCP shortages and meeting the needs of patients receiving primary care. The aim of this overview of systematic reviews is to assess the effects of delegation or substitution of PCPs' activities by nurses on patient relevant, clinical, professional and health services-related outcomes. METHODS: We conducted a systematic literature search for secondary literature in Medline, Embase, Pubmed, the Cochrane Library, and the Cumulative Index of Nursing and Allied Health Literature (CINAHL). We included s

Why nurses in primary care need to be research active: the case of venous leg ulceration.

Venous leg ulceration (VLU) is predominantly managed in primary care by district nurses, however much of the research takes place in secondary care. This study aimed to identify to what extent nurses are involved in publishing VLU research and to ascertain how much VLU research is conducted in primary care. Three searches of literature published between 2015 and 2020 were undertaken, reviewing VLU publications on interventions, quality of life and qualitative research. Some 37% of intervention studies had one or more nurse authors, compared with 65% of quality of life studies and 86% of qualit
