A person-centered, transdiagnostic schema and mode profile approach to predict outcome in time-limited schema group therapy.

OBJECTIVE: This study employs a person-centered transdiagnostic approach to examine how schema and mode profiles predict symptom severity reduction in schema group therapy for patients with personality disorders and enduring clinical syndromes. METHOD: We analyzed symptom reduction in 248 patients across three formats of manualized, time-limited schema group therapy. Latent profile analysis and mixed multilevel modeling were used to determine the extent to which schema/mode classes predict symptom reduction, and whether the inclusion of individual schemas and modes enhances these predictions.

The process and outcomes of a multimethod needs assessment at a homeless shelter.

Many factors contribute to homelessness, including extreme poverty, extended periods of unemployment, shortages of low-income housing, deinstitutionalization, and substance abuse. As a result, the needs of people who are homeless are broad and complex. This needs assessment used literature reviews, review of local documents and reports, participant observation, locus groups, and reflective journals to guide the development of an occupational performance skills program at one homeless shelter in south Florida. Through these methods, the role of occupational therapy was extended beyond direct se

Surgical pathology to describe the clinical margin of debridement of chronic wounds using a wound electronic medical record.

BACKGROUND: Chronic wounds, including diabetic foot ulcers (DFU), pressure ulcers (PU), and venous ulcers (VU) result from multiple physiologic impairments. Operative debridement is a mainstay of treatment to remove nonviable tissue and to stimulate wound healing. Unlike tumor resection, however, operative wound specimens are not routinely sent for pathology. The objective of this study was to describe the pathology present in chronic wounds. STUDY DESIGN: Pathology reports of the skin edge and wound base from 397 initial debridements in 336 consecutive patients with chronic wounds were retro

Community-based multidisciplinary care for patients with stable chronic obstructive pulmonary disease (COPD): an evidence-based analysis.

UNLABELLED: In July 2010, the Medical Advisory Secretariat (MAS) began work on a Chronic Obstructive Pulmonary Disease (COPD) evidentiary framework, an evidence-based review of the literature surrounding treatment strategies for patients with COPD. This project emerged from a request by the Health System Strategy Division of the Ministry of Health and Long-Term Care that MAS provide them with an evidentiary platform on the effectiveness and cost-effectiveness of COPD interventions. After an initial review of health technology assessments and systematic reviews of COPD literature, and consultat

Effect of performance improvement programs on compliance with sepsis bundles and mortality: a systematic review and meta-analysis of observational studies.

BACKGROUND: Several reports suggest that implementation of the Surviving Sepsis Campaign (SSC) guidelines is associated with mortality reduction in sepsis. However, adherence to the guideline-based resuscitation and management sepsis bundles is still poor. OBJECTIVE: To perform a systematic review of studies evaluating the impact of performance improvement programs on compliance with Surviving Sepsis Campaign (SSC) guideline-based bundles and/or mortality. Specifying and Pilot Testing Quality Measures for the American Society of Addiction Medicine's Standards of Care.

OBJECTIVES: In 2013, the American Society of Addiction Medicine (ASAM) approved its Standards of Care for the Addiction Specialist Physician. Subsequently, an ASAM Performance Measures Panel identified and prioritized the standards to be operationalized into performance measures. The goal of this study is to describe the process of operationalizing 3 of these standards into quality measures, and to present the initial measure specifications and results of pilot testing these measures in a large health care system. By presenting the process rather than just the end results, we hope to shed ligh

Avoidable death as a measure of quality.

Age standardized mortality rates (SMR) vary between 43 and 162 per 100,000 inhabitants from cervical cancer in different areas in England, between 19 and 250 from tuberculosis, between 31 and 249 for asthma, between 0 and 263 from rheumatic heart disease, from 0 to 379 for acute respiratory disease, from 18 to 279 for abdominal hernias, and from 0 to 228 for appendicitis. Essentially similar differences were found for some of these diseases between five EEC countries. Many differences persist when correlated against social variables, and high quality health services should respond to possible

Pediatric recertification and quality of care: the role of the American Board of Pediatrics in improving children's health care.

American health care is in the middle of a second revolution in quality as profound as the Flexner revolution occurring almost 100 years ago. Although systems issues are the basis for most of the concern, physician quality and professional development are also pertinent. Specialty board certification and maintenance of certification are key drivers of professional development and improvement of care.

Diagnosis and Macintosh substitution for chronic anterior cruciate insufficiency.

Of 45 consecutive knees undergoing MacIntosh fascia lata anterior cruciate substitution, only five had accompanying collateral ligament laxity. Over a third of cases (16) had more than ten episodes of major instability causing falls to the ground pre-operatively. The initial injury caused haemarthrosis, usually without external trauma (36), was often accompanied by a sensation of knee disruption (18), and was always associated with immediate disability. The results of 26 lateral substitution operations with minimum 12 month follow-up show abolition of pivot shift jerk in all patients. Incomple

A naturalistic assessment of partial-hospital treatment.

Three hundred fifty-eight consecutive admissions to 13 partial hospitals were evaluated over an 8-month period. The design of the study focused on an unobstrusive measurement process involving clinical judgment of outcome. Analyses revealed that the clinical judgment process generated data which were both reliable and valid. The results of the study indicate that partial-hospital treatment is most effective for chronic and acute schizophrenic conditions, paranoid schizophrenia, and affective disorders and least so far patients diagnosed as adjustment disorders. Patients in the organic and neur

Pericardial Effusion Detection on Post-Mortem Computed Tomography Images Using Convolutional Neural Networks.

Pericardial effusion can be a sign of significant underlying diease and, in some cases, may lead to death. Post-mortem computed tomography (PMCT) is a well-established tool to assist death investigation processes in the forensic setting. In practice, the scarcity of well-trained radiologists is a challenge in processing raw whole-body PMCT images for pericardial effusion detection. In this work, we propose a Pericardial Effusion Automatic Detection (PEAD) framework to automatically process raw whole-body PMCT images to filter out the irrelevant images with heart organ absent and focus on peric

Use of Objective Metrics in Dynamic Facial Reanimation: A Systematic Review.

IMPORTANCE: Facial nerve deficits cause significant functional and social consequences for those affected. Existing techniques for dynamic restoration of facial nerve function are imperfect and result in a wide variety of outcomes. Currently, there is no standard objective instrument for facial movement as it relates to restorative techniques. OBJECTIVE: To determine what objective instruments of midface movement are used in outcome measurements for patients treated with dynamic methods for facial paralysis. DATA SOURCES: Database searches from January 1970 to June 2017 were performed in Pub

Deep brain stimulation parameters for dystonia: A systematic review.

INTRODUCTION: Programming of globus pallidus pars interna (GPi) deep brain stimulation (DBS) systems for dystonia is complex because clinical benefits are often gradual. Some groups have advocated starting DBS with higher electrical parameters, whereas others have suggested the opposite. This variability in programming, even within each dystonia subtype, makes it challenging to compare outcomes and program the generators. To determine how variable DBS for dystonia stimulation parameters are, we performed a systematic literature review. METHODS: A comprehensive systematic literature search for

Reentry interventions that address substance use: A systematic review.

Justice-involved individuals with substance use problems have heightened risk of relapse and recidivism after release from incarceration, making reentry a critical time to provide evidence-based treatments (EBTs) for substance use; however, the extent to which reentry interventions incorporate EBTs for substance use is unclear. This systematic review identified studies of reentry interventions in the past 10 years that address substance use, assessed whether EBTs were used, and explored which interventions were effective in reducing substance use and recidivism postrelease. Eligible studies in
