BACKGROUND: Caring for dying patients and their distressed families can be traumatic. For physicians, these encounters shape the way they see, act and feel as professionals, influencing the inculcation and display of their professional expectations and values-otherwise known as professional identity formation (PIF). When inadequately supported, such changes in PIF can impact patient care, family support and teamwork. With recurrent, brief yet emotionally charged interactions with patients and families especially likely to predispose to moral distress, compassion fatigue and burnout, we propose

OBJECTIVE: Limitations on resident duty hours require formal education programs to be high-yield and impactful. Hospital medicine (HM) topics provide the foundation for inpatient pediatric knowledge pertinent to pediatric residents and medical students. Our primary objective was to describe the creation of an innovative pediatric HM curriculum designed to increase learners’ medical knowledge and their confidence in communicating with patients and families about these topics; our secondary objective was to evaluate the level of innovation of the conference sessions perceived by the learners.

INTRODUCTION: Effectively discussing palliative care with patients and families requires knowledge and skill. The purpose of this study was to determine perceived needs of inpatient nurses for communicating with patients and families about palliative and end-of-life (EoL) care.
METHOD: A non-experimental design was used. In total, 60 inpatient nurses from one hospital in Idaho completed the End of Life Professional Caregiver Survey (EPCS), which examines three domains: patient and family-centered communication, cultural and ethical values, and effective care delivery.
RESULTS: The number of

Genetic counselling sessions are rich and complex sites of accounting practices for decision-making in which clinicians are meant to facilitate rather than control the decisions made by their clients. This often means the adoption of a non-directive stance as counsellors lay out various possible courses of action from which the client can choose, while both client and counsellor may need to bear in mind a wide range of practical and ethical issues. With regard to childhood predictive testing, the complexity of decision-making is manifest not only in relation to the severity of the genetic cond

Fungal sinusitis is broadly classified into invasive and non invasive types. Invasive type presents with fungal hyphae within the mucosa, submucosa, bone, or blood vessels of the paranasal sinuses and includes Acute Invasive Fungal Sinusitis, Acute Fulminant, Chronic Invasive Fungal Sinusitis, Chronic Granulomatous Fungal Sinusitis. Invasive forms of fungal rhinosinusitis are rare and confined to groups of patients who are immunocompromised (Key in Fungal rhinosinusitis). Noninvasive type shows absence of fungal hyphae within the mucosa paranasal sinuses. It includes Allergic Fungal rhinosinus

OBJECTIVE: This study aimed to describe the role of parent alliance in Family-Based Treatment (FBT) for adolescents with anorexia nervosa (AN). Differences between parent and child alliance with the therapist, mothers' and fathers' alliance, and their relationship to outcome were examined.
METHOD: Independent observers rated audiotapes of early therapy sessions to assess the therapeutic alliance of parents and adolescents with AN in FBT. Outcome was defined using a previously established cut-point for recovery from AN.
RESULTS: Mothers' and fathers' alliance scores with the therapist were si

Many families experience difficulty in talking about an inherited genetic condition that affects one or more of them. There have now been a number of studies identifying the issues in detail, however few have developed interventions to assist families. The SPRinG collaborative have used the UK Medical Research Council's guidance on Developing and Evaluating Complex Interventions, to work with families and genetic counsellors (GCs) to co-design a psycho-educational intervention to facilitate family communication and promote better coping and adaptation to living with an inherited genetic condit

All health professionals within a defined geographical area who had any knowledge of, or contact with, patients with Huntington's chorea or their relatives were contacted and interviewed. Genetic counselling was given no consideration at all by most of the professional workers. In no case did the neurologist making the diagnosis point out the implications for the patient's family to the appropriate general practitioners who, with one exception, remained in ignorance of the genetic risk. Serious opportunities for the prevention of Huntington's chorea are being neglected.

Pediatric cancer is a life-threatening disease that challenges the life of the diagnosed child, the parents, and possible siblings. Moreover, it also places considerable demands on family life. The aim of this study was to explore changes in the family functioning after a pediatric cancer diagnosis. Ten couples who had a child with leukemia or non-Hodgkin lymphoma were interviewed individually about their experiences. Interviews were semistructured, and the data were analyzed using Multi Family Member Interview Analysis. Three themes emerged from the data: (a) Family Cohesion: Strengthened Ver

Family-centered care (FCC) emphasizes collaboration, dignity, respect, and shared decision-making between families and health care teams. In the ICU, FCC relies on family presence at the bedside, facilitated by open visitation policies. However, the COVID-19 pandemic disrupted this model, as restrictive visitation policies eliminated family presence, leading to adverse outcomes such as loneliness and delirium in patients, distress and grief among families, and moral injury and burnout in staff. As health systems recover, there is a need to reestablish FCC by prioritizing open visitation while

BACKGROUND: The choice of wording in cases of suspected brain death is important. If brain death has not been proven by electrocerebral silence and by absence of spontaneous breathing in an apnoea test in a patient in intensive care, then words like 'brain dead', 'has died' and 'clinical brain death' should be avoided in conversations with the relatives of the patient. This is illustrated by three cases.
CASES: The first patient was a 46-year-old woman, with thrombosis of the basilar artery; the second was a 26-year-old man who was resuscitated after a bilateral pneumothorax, but developed se

Advances in organ recovery and transplantation have provided us with the skills and opportunity to save, extend, and improve the quality of life for many. But with these opportunities have come challenges to redefine our practice and relationships with donor and recipient families. Although most donor families and transplant recipients receive some information about each other, many still do not. In the past, communication between donor families and recipients has been anonymous and highly controlled, with much inconsistency among and within the transplant community, leaving many involved in t

This paper describes the psychological dynamics among family members of patients with prolonged coma. We discuss the conflicts these families meet and how they affect their interpersonal relationships. Special focus is placed on overt and covert aggression towards the attending staff, an issue that has not received much attention. The unconscious motives underlying these overt aggressive behaviours are described, with an emphasis on how projective mechanisms come into play. We suggest a partial solution to be implemented in units treating these kinds of patients.

OBJECTIVE: This systematic review analyzed the relationships between social support and quality of life (QOL) indicators among lung cancer patients. In particular, the patterns of relationships between different social support facets and sources (received and perceived support from healthcare professionals, family, and friends) and QOL aspects (emotional, physical symptoms, functional, and social) as well as the global QOL index were investigated.
METHODS: The review yielded 14 original studies (57% applying cross-sectional designs) analyzing data from a total of 2759 patients.

In 1978, in the course of a trial, health visitors in the North and South Nottingham Health Districts asked parents on two occasions 'Do you think your child is hearing normally?'. In that and the following year there was an abrupt increase in referrals of children under one year of age suspected of hearing impairments, which was not observed in the other Health Districts of the Nottinghamshire AHA. There was also an increase in the number of children under one year who were confirmed as having impaired hearing or needing surveillance. It is concluded that the active questioning by health visi
