The British journal of general practice : the journal of the Royal College of General Practitioners2017eng
BACKGROUND: It has been suggested that interactions between patients and practitioners in primary care have the potential to delay progression of complications in type 2 diabetes. However, as primary care faces greater pressures, patient experiences of patient-practitioner interactions might be changing.
AIM: To explore the views of patients with type 2 diabetes on factors that are of significance to them in patient-practitioner interactions in primary care after diagnosis, and over the last 10 years of living with the disease.
DESIGN AND SETTING: A longitudinal qualitative analysis over 10

BACKGROUND: Bronchiectasis and pulmonary nontuberculous mycobacteria (NTM) infections are chronic diseases that can cause debilitating respiratory symptoms, exacerbations, and even lead to respiratory failure. Clinical care has historically been provided by individual specialists, but multidisciplinary care teams have recently emerged to improve evaluation of underlying etiology and interventions that disrupt the trajectory of disease progression.
RESEARCH QUESTION: Does a multidisciplinary care program reduce bronchiectasis exacerbations or improve sputum conversion rates in pulmonary NTM in

Following a comprehensive patient examination, including the assessment of periodontal and peri-implant diseases as well as considering the patient's needs, a pretherapeutic prognosis for each tooth and implant is given. Teeth and implants with a secure pretherapeutic prognosis require simple procedures and may be regarded as secure abutments for function and with a doubtful pretherapeutic prognosis usually need a comprehensive therapy. Such teeth and implants must be brought into the category with a secure prognosis by means of additional therapy such as endodontic, restorative, and surgical

Despite recent advances in disease-targeted pharmacotherapies for interstitial lung disease (ILD), disease progression occurs in a significant proportion of patients with consequent impact on health-related quality of life. Patients with ILD and their caregivers, clinicians, and researchers have expressed the need for more comprehensive care in ILD. The treatable traits approach is proposed as a person-centred framework for management of ILD, delivering the right treatment to the right patient at the right time, targeting distinct traits that affect health outcomes beyond the lung disease itse

BACKGROUND: In individuals with migraine, attacks may increase in frequency, severity, or both. Preventing migraine progression has emerged as a treatment goal in headache subspecialty practice, but there may be less awareness in general neurology or primary care settings where most people with migraine who seek treatment consult. Herein, we review the definition of and risk factors for migraine progression and consider strategies that could reduce its risk.
METHODS: A group of headache expert healthcare professionals, clinicians, and researchers reviewed published evidence documenting factor

PURPOSE: The aim of the present qualitative study was to analyse patients' attitudes towards oral health and experiences of periodontal treatment.
MATERIALS AND METHODS: Patients who were referred to a specialist clinic in periodontics were subjected to open-ended interviews before (Abrahamsson et al, 2008), as well as after, the phase of cause-related periodontal therapy. An interview guide was used with themes of interest. The time interval between the interviews was about 6 months. Because the study focused on psychosocial interactions, the qualitative method used for the analysis was Grou

OBJECTIVE: To describe two patients with lumbar facet synovial cysts causing sciatica and progressive neurological deficit.
CLINICAL FEATURES: A 52-year-old female with bilateral sciatica and a neurological deficit that progressed to a foot drop; and a 54-year-old female with worsening sciatica and progressive calf weakness were seen at a major tertiary care centre. Diagnostic imaging studies revealed the presence of spinal nerve root impingement by large facet synovial cysts.
INTERVENTIONS AND OUTCOMES: Activity modification, gabapentinoid and non-steroidal anti-inflammatory medications wer

PURPOSE: Prior studies have shown that communication-related participation restrictions in patients with degenerative disease do not always match clinician judgment or objective indices of symptom severity. Although there is a growing body of literature documenting that discrepancies between patients with dementia and their care partners' perception of participation restrictions exist, it is not known how care partner perceptions of communication participation restrictions specifically match or diverge from the patients' experiences, which may inform the use of care partner proxy in the contex

Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disease associated with progressive dysphagia, dysarthria, extremity weakness, and dyspnea. Although there are some disease-modifying pharmacological treatments available which can modestly slow disease progression, the disease is relentlessly progressive and is ultimately fatal. Patients living with ALS should be supported using the principles of palliative care, and in particular, the use of a holistic approach to support the patients and their families. Evidence would support management of patients living with ALS by a m

Critical care nurses within acute care settings are responsible for providing healthcare to a wide variety of patients and, consequently, knowledge regarding how to care for a patient with acute necrotizing fasciitis is imperative. A case study is presented to evidence the need for a multidisciplinary approach. Necrotizing fasciitis is defined and treatment options are presented. Caring for this patient is very challenging and demands a multidisciplinary team to coordinate all aspects of care to promote better patient outcomes.

A patient with end-stage motor neurone disease was admitted for hospice care with worsening bulbar symptoms. Although he initially walked onto the ward he became very distressed and asked for sedation. After much discussion, this man was deeply sedated, and after some harrowing days, died. Was it right to provide terminal sedation? What should the threshold be for such treatment? How should our personal reservations affect how we approach the distressed patient in an end-of-life situation?

Motor neurone disease (MND) is a debilitating and progressive neurological disorder in which degeneration of the motor neurones results in muscle weakness and wasting. Recent high-profile media coverage about assisted suicide and euthanasia has increased awareness of MND and yet still relatively little is known or understood about it, even among the healthcare profession. This article looks at the key areas in management of patients with MND, including the role of the nurse and the importance of a well-coordinated multidisciplinary team. Ways to deal with communication problems, respiratory dy

Background Patients with chronic disease prefer an adequately supported death at home, but often die in the hospital. We assessed temporal trends and sex differences in healthcare intensity and location of death among decedents with heart failure. Methods and Results This was a retrospective cohort study of adults with heart failure who died between April 1, 2004 and March 31, 2017 in Ontario, Canada. We used population-based administrative databases to assess healthcare utilization during the last 6 months of life and applied multilevel multivariable logistic regression to assess whether sex

BACKGROUND: There is a need to better evaluate Parkinson's disease (PD) impact on daily lives of People with Parkinson's (PwP).
OBJECTIVES: To conceptualize a novel digital tool, the MDS PD e-Diary that has been commissioned by the International Parkinson's disease and Movement Disorders Society.
METHODS: Using a modified Delphi methodology, we sought consensus among key stakeholders (PwP, care partners, PD specialists, industry, regulatory representatives) through online questionnaires, focus groups, and a broad community survey.
RESULTS: The consensus resulted in a multiplatform patient-r

It is recommended that women with vulvar lichen sclerosus be followed in specialist clinics where difficulty exists with symptom control or where there is clinical evidence of localized skin thickening. Follow-up is also recommended for women who have previously been treated for squamous cell carcinoma of the vulva (arising in lichen sclerosus or vulvar intraepithelial neoplasia) or where the pathologist expresses concern and is unable to make a definitive diagnosis of differentiated vulvar intraepithelial neoplasia.

Brain impairment is a distressing manifestation of human immunodeficiency virus (HIV) disease characterized by progressive cognitive impairment leading eventually to dementia and death. Patients with advanced brain impairment are clinically difficult to manage and usually require residential care. In 1997, a brain impairment unit opened at the Mildmay Hospital UK in London to meet the needs of this patient group. It began as a nurse-led unit, has adopted an interdisciplinary approach to care and aims to maximize the quality of life until death. In a study of patients admitted during its first
