Dynamics of thyrotropin-releasing hormone-induced thyrotropin and prolactin secretion by acutely dispersed rat adenohypophyseal cells. Evidence for 'all-or-none' secretion by heterogeneous secretory units, each with a specific response threshold.

We have examined the dynamics of thyrotropin-releasing hormone (TRH)-stimulated secretion of prolactin (PRL) and thyrotropin-stimulating hormone (TSH) using enzymatically dispersed rat adenohypophyseal cells suspended in a perfusion chamber with a volume of 0.2 ml to minimize mixing and dilution. One-min exposure to 3-300 nM TRH, the effective dose range, elicited immediate pulses of PRL and TSH secretion with dose-dependent amplitudes. At all TRH concentrations, following a brief burst of secretion lasting less than 1 min, release of both hormones declined precipitously. Increasing the durati

[Variations of the plasma levels of gonadotrophins (FSH and LH) and of its response to stimulation with gonadotrophin releasing hormone (GnRH) with different plasma levels of prolactin (PRL) in women with the syndrome of galactorrhea-amenorrhea (author's transl)].

In 15 women with either isolated amenorrhea or amenorrhea associated to galactorrhea the basal levels of PRL allowed a clear differentiation into three groups. The first group (n = 3) had normal PRL levels (x +/- SD, 8.0 +/- 4.8 ng/ml), the second group (n = 4) had moderately elevated PRL (25.6 +/- 6.5 ng/ml), and the third group (n = 8) had very high PRL (176.0 +/- 76.1 ng/ml). All the patients in the third group had a pituitary adenoma. In the three groups the basal levels of FSH and LH and their response to GnRH were measured with the purpose of uncovering possible relationships between the

Thyrotropin-releasing hormone and the pituitary. New insights into the mechanism of stimulated secretion and clinical usage.

Thyrotropin-releasing hormone, a hypothalamic tripeptide, has become a useful pharmacologic tool in clinical medicine. Evidence supporting a role for thyrotropin-releasing hormone as a physiologic regulator of thyroid-stimulating hormone (thyrotropin) but not prolactin secretion is reviewed. Data from animal studies employing thyrotropin- and prolactin-secreting cells that demonstrate that thyrotropin-releasing hormone elevates the concentration of calcium ion free in the cell cytoplasm are presented. These observations are consistent with the hypothesis that calcium ion couples stimulation by

Prolactin deficiency in pseudohypoparathyroidism.

Because thyrotropin deficiency may occur in pseudohypoparathyroidism, we studied pituitary function in two families with this condition. Six of eight patients had impaired pituitary prolactin secretion after administration of thyrotropin-releasing hormone and chlorpromazine, with preservation of other anterior pituitary functions. Peak serum prolactin levels after thyrotropin-releasing hormone and chlorpromazine in the prolactin-deficient patients were significantly lower than normal. Administration of diethylstilbestrol for five days, which normally enhances prolactin responses to thyrotropin

Pituitary function in Sheehan's syndrome.

The responses of plasma prolactin, follicle-stimulating hormone (FSH), luteinizing hormone (LH), and thyroid-stimulating hormone were measured in ten women with Sheehan's syndrome after intravenous administration of the appropriate releasing hormones. Growth hormone and cortisol responses to insulin-induced hypoglycemia were also measured. It was found that two patients were biochemically borderline euthyroid and eight were probably hypothyroid in the face of thyroid-stimulating hormone values in the reference range; in two patients thyroid-stimulating hormone response to thyrotropin-releasing

Evidence for a hypothalamic defect in septo-optic dysplasia.

A 21-year-old man demonstrated septo-optic dysplasia. Optic and retinal colobomas were present and panhypopituitarism was documented. Releasing hormone studies showed partial luteinizing hormone (LH) response and no follicle-stimulating hormone response to administration of gonadorelin (LH-releasing hormone); thyroid-stimulating hormone (TSH) and prolactin levels were increased normally after administration of protirelin (thyrotropin-releasing hormone). The LH, TSH, and prolactin responses are believed to be evidence of intact pituitary function and suggest that a hypothalamic defect accounts

Galactorrhea in sarcoidosis: dynamic studies of prolactin, growth and gonadotropic hormone levels.

Five of 300 women with sarcoidosis had galactorrhea. Basal prolactin levels were mildly but significantly elevated when compared to controls and sarcoidosis patients without galactorrhea. Prolactin levels responded to thyrotropin releasing hormone and L-dopa administration, but not chlorpromazine. Luteinizing hormone and follicle-stimulating hormone concentrations responded normally to luteinizing hormone-releasing hormone in all sarcoidosis patients studied, as did growth hormone to insulin hypoglycemia. These results indicate that galactorrhea in sarcoidosis is an uncommon phenomenon, probab

Gynecomastia associated with isolated ACTH deficiency.

A 57-year-old man with gynecomastia associated with isolated adrenocorticotropic hormone deficiency is reported. Basal levels of estrogens, luteinizing hormone and prolactin were elevated, whereas plasma testosterone levels were normal. Luteinizing hormone and prolactin showed exaggerated responses to gonadotropin-releasing hormone and thyrotropin-releasing hormone, respectively. These hormonal abnormalities were corrected and gynecomastia resolved with steroid replacement therapy. These findings suggest that gynecomastia may result from hormonal changes which were modified by glucocorticoid d

Influence of phlebotomy treatment on abnormal hypothalamic-pituitary function in genetic hemochromatosis.

To test the hypothesis that deficiencies in hypothalamic-pituitary function in genetic hemochromatosis result from cellular injury by iron deposits, we conducted provocative tests in 11 men with genetic hemochromatosis before and after iron depletion by serial phlebotomy and in 10 control subjects. We gave combination intravenous injections of insulin (0.15 U/kg), luteinizing hormone releasing hormone (LHRH, 100 micrograms), and thyrotropin releasing hormone (400 micrograms) and then measured plasma glucose, growth hormone, corticosteroids, follicle-stimulating hormone, luteinizing hormone, pr

Galactorrhea-amenorrhea syndrome: diagnosis and therapy.

Tests of prolactin regulation in the galactorrhea-amenorrhea syndrome were compared in 18 patients with normal pituitary fossae, seven patients with prolactin-secreting adenomas, and eight normal women. Mean basal prolactin was highest in patients with adenomas and was elevated in those with normal fossae when compared with normal subjects (278 versus 73 versus 10.2 ng/ml). Levodopa, water loading, or luteinizing hormone-releasing hormone testing were of no predictive value in the diagnosis of adenoma. Some patients with adenomas show a greater prolactin response after administration of thyrot

The clinical utility of a heterologous radioimmunoassay for human prolactin.

A specific and sensitive double antibody radioimmunoassay for human prolactin has been developed utilizing anti-human prolactin and purified ovine prolactin for radioiodination. Employing this radioimmunoassay, as little as 1.5 ng of prolactin can be quantified in a ml of human serum. The prolactin in serum detected by the radioimmunoassay behaved in a similar manner to purified human 125-I-labled prolactin on polyacrylamide gel electrophoresis. Exogenous prolactin could be completely recovered when varying amounts were added to a constant volume of serum. In normal control and primary hypothy

Pituitary responsiveness to gonadotrophin-releasing and thyrotrophin-releasing hormones in children receiving phenobarbitone.

The effect of long-term treatment with phenobarbitone on pituitary responsiveness to gonadotrophin-releasing hormone and thyrotrophin-releasing hormone was studied in 20 boys being treated with the drug to prevent febrile convulsions. Baseline concentrations of luteinising and follicle-stimulating hormones were reduced as well as the responses of these hormones to stimulation with gonadotrophin-releasing hormone. Baseline prolactin concentrations were raised in comparison with those in normal children. The response of prolactin to thyrotrophin-releasing hormone, however, was impaired only in t

Pituitary function testing in amenorrhea-galactorrhea-hyperprolactinemia.

Fifteen patients, age 16 to 55, presented with amenorrhea-galactorrhea-hyperprolactinemia. Pituitary function was evaluated by bolus injections of insulin, luteinizing hormone-releasing hormone (LHRH), and thyrotropin-releasing hormone (TRH) in 13 and by LHRH and TRH in 2. Responses to growth hormone (GH), thyroid-stimulating hormone (TSH), cortisol (F), luteinizing hormone (LH), follicle-stimulating hormone (FSH), and prolactin were measured. GH, TSH, and F responses were normal in most cases. LH responses were decreased (P less than 0.025) in patients with abnormal sellar tomography, whereas

Ectopic prolactin secretion from a gonadoblastoma.

A 6.5-year-old girl developed isosexual, pseudoprecocious puberty secondary to a gonadoblastoma. The tumor was found to produce and secrete both immunoassayable and bioassayable prolactin based on immunohistochemical techniques and the presence of a prolactin gradient between the tumor vein and peripheral vein. The source of the prolactin was a Sertoli-like cell. Neither growth hormone nor growth hormone-releasing hormone was detected within the tumor. This case confirms the ectopic production of prolactin by neoplastic tissue.

Pituitary insufficiency following head injury.

A 32-year-old man developed panhypopituitarism and diabetes insipidus shortly after sustaining a head injury. Hormonal investigation showed that basal prolactin levels were moderately elevated the first two years after the accident, but later returned to normal. There was no rise in prolactin after administration on chlorpromazine, and the response to thyrotropin-releasing hormone was attenuated. Basal luteinizing hormone and follicle-stimulating hormone levels were low and there was no change after administration of luteinizing-hormone-releasing hormone. There was also no growth hormone eleva

The aetiology of galactorrhoea in women with regular menstruation and normal prolactin levels.

Fourteen primary infertile women with expressible galactorrhoea associated with regular ovulatory cycles and normal basal prolactin levels (group A) were matched for age and weight with 14 infertile women with regular menstruation but no galactorrhoea (group B). Both groups showed equivalent increases in prolactin levels after stimulation with 200 micrograms thyrotrophin-releasing hormone (TRH) during the follicular and luteal phases of the menstrual cycle. Patients in group A had a greater increase in luteinizing hormone levels after 100 micrograms i.v. injection of a luteinizing hormone-rele

Hypothalamic-hypopituitarism due to temporal arteritis.

Hypothalamic hypopituitarism developed in a 74-year-old woman who was in the active phase of temporal arteritis. Hypopituitarism was established by low basal gonadotrophin, thyroxine, and thyroid-stimulating hormone levels. The failure of growth hormone and prolactin to respond to insulin hypoglycemia and an inadequate prolactin response to the administration of chlorpromazine provided further evidence of hypopituitarism. The hypothalamic origin of the hypopituitarism was suggested by quantitatively normal prolactin and gonadotrophin responses to the administration of gonadorelin (gonadotrophi

Assessment of pituitary function in patients with serum prolactin levels greater than 100 ng/ml.

Twelve women with galactorrhea-amenorrhea and prolactin levels greater than 100 ng/ml were evaluated with dynamic pituitary challenge testing. Forty-two per cent of the patients had positive findings on polytomography and subsequent surgical confirmation of a pituitary tumor. Patients with tumors had a delayed elevation of growth hormone (GH) and cortisol following induction of insulin hypoglycemia. Patients had increased thyroid-stimulating hormone levels after injection of thyrotropin-releasing factor, but showed blunting of prolactin secretion. Patients with tumors had decreased basal level

In vivo and in vitro modulation of growth hormone and prolactin of a mixed somatotropic-lactotropic pituitary microadenoma.

Both in vivo and in vitro responses of prolactin and growth hormone to stimuli were studied in an acromegalic, amenorrheic woman with a chromophobe adenoma of the pituitary gland. Preoperative testing revealed a prolactin rise after thyrotropin-releasing hormone and chlorpromazine and a decline after L-dopa and bromocriptine administration. During 3.5-hour incubations, cultured tumor cells produced significant increases of growth hormone and prolactin after dibutyryl adenosine 3',5'-cyclic monophosphate stimulation, while bromocriptine inhibited both hormones.

Pediatric pituitary adenomas.

Previous series of pediatric pituitary adenomas have been small and have not been analyzed by age group. We analyzed the frequency, manifestation, course, and biology of these tumors before, during, and after puberty in 136 children younger than 20 years old at surgery, identified by review of 2230 patients treated from 1969 to 1993. Tumors were classified by clinical phenotype. Adrenocorticotropic hormone-releasing adenomas were most common before puberty, and prolactinomas were most common during and after. The frequencies of adrenocorticotropic hormone-releasing adenomas, prolactinomas, and
