Propionic acidemia and methylmalonic aciduria: A portrait of the first 3 years-Admissions and complications.

BACKGROUND: Propionic acidemia (PA) and methylmalonic acidemia (MMA) are organic acidurias that can present with metabolic acidosis and hyperammonemia, often leading to frequent hospital admission. While recent studies have provided guidelines for management and diagnosis of these conditions, there is a lack of research detailing the frequency and reasons for hospital admission in the first three years of life. METHODS: As part of a quality improvement project, data were extracted from medical records of all patients with PA and MMA (born between 2000 and 2024) who were seen at Children's Nat

Ventricular Fibrillation and Cardiomyopathy in Propionic Acidemia: A Rare Metabolic Red Flag in Young Adults.

BACKGROUND: Propionic acidemia (PA) is a rare autosomal recessive metabolic disorder, typically presenting in infancy. Cardiac involvement in adults is uncommon and underrecognized. CASE SUMMARY: A previously healthy 20-year-old man suffered an out-of-hospital cardiac arrest caused by ventricular fibrillation. Initial evaluation revealed dilated cardiomyopathy, elevated lactate and ammonia levels, and increased propionyl-L-carnitine. Genetic testing identified biallelic pathogenic variants in the PCCB gene, confirming PA. He was managed with a subcutaneous implantable cardioverter-defibrillat

Metabolic perturbations mediated by propionyl-CoA accumulation in organs of mouse model of propionic acidemia.

Propionic acidemia (PA) is an autosomal recessive metabolic disorder after gene encoding propionyl-CoA carboxylase, Pcca or Pccb, is mutated. This genetic disorder could develop various complications which are ascribed to dysregulated propionyl-CoA metabolism in organs. However, the effect of attenuated PCC on propionyl-CoA metabolism in different organs remains to be fully understood. We investigated metabolic perturbations in organs of Pcca-/-(A138T) mice (a mouse model of PA) under chow diet and acute administration of [13C3]propionate to gain insight into pathological mechanisms of PA. Anesthetic Management for Pediatric Liver Transplantation in a Patient With Propionic Acidemia: A Case Report.

Propionic acidemia is an inborn error of metabolism characterized by accumulation of propionic acid due to deficiency of propionyl-CoA carboxylase. Main stay of treatment focuses on reducing dietary protein. However, orthotropic liver transplantation decreases the frequency of metabolic decompensations and improves life expectancy. We report a case of a 4-year-old boy undergoing orthotropic liver transplantation to treat propionic acidemia. This case highlights the use of intraoperative monitoring of metabolic markers like urine ketones, arterial ammonia, and lactate levels as these patients a

Incidence of maple syrup urine disease, propionic acidemia, and methylmalonic aciduria from newborn screening data.

Incidence for the branched-chain intoxication-type disorders, maple syrup urine disease, propionic acidemia and methlymalonic aciduria is dependent on the population screened. Here newborn screening results from three world regions, state screening laboratories in the United States, a region in Germany and Kuwait provides new incidence numbers. Maple syrup urine disease incidence in the United States was calculated to be 1: 220219, in South-West Germany 1: 119573 (Germany nationwide 1:177978), and in Kuwait 1: 59426. Incidence of propionic acidemia alone is calculated to be 1: 242741 in the Un

Progressive Dilated Cardiomyopathy in a 24-Year-Old Man: A Rare Manifestation of Propionic Acidemia.

BACKGROUND: Adult onset propionic acidemia (PA) is exceedingly rare, particularly when manifesting as dilated cardiomyopathy. CASE SUMMARY: A previously healthy 24-year-old man presented with cardiac dilatation and acute progressive heart failure. His condition was refractory to guideline-directed medical therapy, deteriorating to cardiogenic shock within 6 weeks. Plasma acylcarnitine profile revealed elevated propionylcarnitine and an increased propionylcarnitine-to-acetylcarnitine ratio. Urinary organic acid analysis showed elevated methylcitric acid. Whole-exome sequencing identified a hom

Breath biopsy in inborn errors of metabolism: A proof-of-principle study in propionic acidemia.

BACKGROUND: Impaired oxidation of branched chain amino acids may give rise to volatile organic compounds (VOCs). We hypothesized that VOCs will be present in exhaled breath of participants with propionic acidemia (PA), and their relative abundance would correlate with clinical and biochemical characteristics of the disease. METHODS: We enrolled 5 affected participants from a natural history study of PA (ClinicalTrials.gov ID NCT02890342) plus five age- and sex-matched unaffected controls. We collected exhaled breath using a non-invasive breath sampling platform paired with thermal desorption-

Carglumic Acid Contributes to a Favorable Clinical Course in a Case of Severe Propionic Acidemia.

Propionic acidemia (PA) is manifested as an abnormal accumulation of propionic acid and its metabolites, including methylcitrate, 3-hydroxypropionic acid, and propionylglycine, and is caused by a defect of propionyl-CoA carboxylase. PA is complicated by acute life-threatening metabolic crises, which are precipitated by a catabolic state and result in multiple organ failure or even death, if untreated. A neonate with PA recovered from the first metabolic crisis 3 days after birth but developed a second metabolic crisis during the recovery phase. This patient was considered to have severe PA and

Dual mRNA therapy restores metabolic function in long-term studies in mice with propionic acidemia.

Propionic acidemia/aciduria (PA) is an ultra-rare, life-threatening, inherited metabolic disorder caused by deficiency of the mitochondrial enzyme, propionyl-CoA carboxylase (PCC) composed of six alpha (PCCA) and six beta (PCCB) subunits. We herein report an enzyme replacement approach to treat PA using a combination of two messenger RNAs (mRNAs) (dual mRNAs) encoding both human PCCA (hPCCA) and PCCB (hPCCB) encapsulated in biodegradable lipid nanoparticles (LNPs) to produce functional PCC enzyme in liver. In patient fibroblasts, dual mRNAs encoded proteins localize in mitochondria and produce

Propionic acidemia as a framework for understanding the impact of disturbed propionyl-CoA on histone modifications.

Histone propionylation-the transfer of a three-carbon propionyl group from propionyl-CoA to lysine residues-is an emerging post-translational modification (PTM) with potential effects on gene expression. Interest in this PTM has grown because propionyl-CoA is at the interface between valine, isoleucine, methionine, and threonine catabolism, odd-chain fatty acid oxidation, gut-derived propionate assimilation, and anaplerotic entry into the tricarboxylic acid cycle. An informative context for studying the influence of disturbed propionyl-CoA metabolism on chromatin state and phenotype is propion

Characterizing the frequency of clinical events and assessing biomarkers in propionic acidemia: a natural history study.

BACKGROUND: Propionic acidemia (PA) is a rare disease resulting in toxic accumulation of metabolites that cause metabolic decompensation events (MDEs). Natural history characterization of PA is limited partly due to its rarity and complexity. This study examined clinical events in participants with PA and their correlation with biomarkers to better understand the disease. METHODS: Participants diagnosed with PA (early- versus late-onset) were evaluated every 6 months from baseline to month 36. Outcome measures included annualized MDEs and changes in 3-hydroxypropionic acid (3-HP), 2-methylcitr

Therapeutic potential of living donor liver transplantation from heterozygous carrier donors in children with propionic acidemia.

BACKGROUND: Current world experience regarding living donor liver transplantation (LDLT) in the treatment of propionic acidemia (PA) is limited, especially in terms of using obligate heterozygous carriers as donors. This study aimed to evaluate the clinical outcomes of LDLT in children with PA. METHODS: From November 2017 to January 2020, 7 of the 192 children who underwent LDLT at our institution had been diagnosed with PA (median age, 2.1 years; range, 1.1-5.8 years). The primary indication for transplantation was frequent metabolic decompensations in 6 patients and preventative treatment i
