The evolving role of familial history for prostate cancer.

BACKGROUND: family history of prostate cancer is a risk factor for prostate cancer occurrence. Differently from other neoplasms no major predisposing gene has been identified. MATERIAL AND METHODS: this review article presents the controversial results of studies about the prognostic and predictive role of family history in prostate cancer, reports the discovered predisposing genes, and biologic and pathologic findings. RESULTS: mortality from PC remains a significant health care problem, but no trial investigated if it changed in presence of positive family history. The largest family study

Unique finding of prostatic tissue in a benign cystic ovarian teratoma.

Benign cystic ovarian teratomas are neoplasms with a 46,XX karyotype originating from a single abnormal ovarian germ cell after the first meiotic division. Since these tumors lack the Y chromosome (required for production of the H-Y antigen that is presumably obligatory for embryonic male sexual differentiation), identifiable male sex organ structures should not be found in the tumor. We report a case in which prostatic tissue was identified in a benign cystic ovarian teratoma.

Giant multilocular prostatic cystadenoma.

BACKGROUND: The giant multilocular prostatic cystadenoma is a very rare benign tumor of the prostate gland. It is composed of predominantly cystic enlarged prostatic glands in a fibrous stroma and spreads extensively into the pelvis. Because of the large size at the time of diagnosis, it is not always possible to determine the exact point of origin for these multilocular cystic neoplasms. Thus, diagnosis before histological examination of a surgical specimen is often difficult. Here, we present a case involving one of the largest giant multilocular prostatic cystadenomas reported in the litera

Inverted papillomas of the prostatic urethra.

Inverted papillomas of the genitourinary tract are uncommon benign neoplasms usually occurring in the urinary bladder and less frequently in the upper urinary tract. To date, there are scant data and no comprehensive studies of inverted papilloma originating in the prostatic urethra. We identified 21 cases and evaluated their demographic, clinical, and histopathologic features. Patients had a mean age of 65.1 years (range: 30 to 89 y), with 10/21 (47.6%) presenting with gross hematuria (n = 8) or irritative symptoms (n = 2) related to the inverted papilloma and 11/21 (52.4%) detected incidenta

68Ga-Prostate-Specific Membrane Antigen PET/CT and 18F-FDG PET/CT of Primary Signet Ring Cell Breast Adenocarcinoma.

Primary signet ring cell adenocarcinoma of the breast is a rare neoplasm. Similar to other mucinous neoplasms, it has low FDG avidity and poor prognosis compared with the traditional ductal/lobular carcinomas. We present a case of a 40-year-old woman with signet ring cell adenocarcinoma and skeletal fluorosis showing higher uptake of Ga-prostate-specific membrane antigen in the primary lesion and the metastatic sites compared with F-FDG on PET/CT imaging. Additionally, diffuse increased prostate-specific membrane antigen uptake was noticed in the entire skeleton, likely due to skeletal fluoros

Polyarteritis nodosa mimicking prostatic cancer.

We describe a 72-year-old man with prostate enlargement, prostate-specific antigen level of 35 ng/dl, mild polyarthritis, and constitutional symptoms. Prostatic ultrasonography suggested neoplasm; however, transrectal biopsy revealed findings consistent with polyarteritis nodosa (PAN). The patient went on to develop leg paresthesia and dysesthesia, increased serum creatinine, and systemic hypertension. Steroids and intravenous cyclophosphamide were administered, followed by improvement. Our case emphasizes the protean onset of PAN, and provides a new differential diagnosis of prostatic disease

Urinary cytologic findings in patients with benign and malignant adenomatous polyps of the prostatic urethra.

CONTEXT: Urethral adenomatous polyps with prostatic epithelium (also known as benign prostatic epithelial polyps [BPEPs]) are a documented cause of hematuria, dysuria, and hematospermia, conditions that may prompt cytologic evaluation of urine. DESIGN: The urine cytologic test findings in 5 cases of biopsy-proven BPEPs and in 1 case of prostatic ductal adenocarcinoma (PDA) that presented as a urethral polyp were retrospectively evaluated. Immunocytochemical stain for prostate-specific antigen (PSA), prostatic acid phosphatase (PAP), and high-molecular-weight cytokeratin (34betaE12) were used

Extragonadal seminoma involving urinary bladder and arising in the prostate.

The authors report the first instance of a patient with seminoma probably arising from the prostate but also involving the bladder. A 58-year-old man presented with symptoms and signs of prostatic enlargement. Cystoscopy revealed a fungating neoplasm, probably arising from the prostate and surrounding the bladder neck. The biopsy was consistent with the diagnosis of seminoma. The patient experienced complete remission after chemotherapy with bleomycin, cisplatin, and etoposide.

Impact of diagnostic labels and management benefit-to-harms information for low-risk prostate pathology.

OBJECTIVES: To evaluate the effects of diagnostic labels and management benefit-to-harms information for low-risk prostate pathology. METHODS: Two factorial (3x2) online hypothetical experiments randomised participants 11:1 to three labels: "low-risk prostate cancer, Gleason Group 1", "low-risk prostate neoplasm", or "low-risk prostate lesion", with second randomisation 1:1 to high or low information on management options. Participants were Australians aged ≥50 years with a prostate (males) or with a male partner aged ≥50 years (females). The primary outcome was preference for management with

Benign phyllodes tumour of the prostate: An extremely rare entity.

Benign phyllodes tumour (BPT) of the prostate is a very rare neoplasm. It is composed of hyperplastic and neoplastic glandular stromal proliferation. Patients with BPT of the prostate generally present with lower urinary tract symptoms and hematuria. BPT of the prostate can potentially cause recurrent obstructive symptoms. Complete transurethral resection (TUR) and close postoperative follow-up is recommended. A 59-year-old man presented with dysuria and obstructive urinary symptoms. Flexible cystoscopy revealed prostatic hyperplasia and a polypoidal lesion originating from the right lateral l

Malignant jejunal gastrointestinal stromal tumor with history of prostate cancer: A case report.

RATIONALE: The problem of the coexistence of gastrointestinal stromal tumor (GIST) with other neoplasms is complex, and carcinomas of prostate is one of the common types of GIST-associated cancers. Doubling time of GIST is about 3.9 months for high-risk GIST, and the treatment paradigm for GIST has required the integration of surgery and molecular therapy. PATIENT CONCERNS: A 70-year-old man with postoperative history of prostate cancer experienced fast-growing malignant jejunal GIST with multiple peritoneal metastases within 1 year. DIAGNOSES: Enhanced computed tomography (CT) detected a ne

A profile of metastatic carcinoma of the spine.

Metastatic bone disease in 322 patients was analyzed to assess the frequency and behavior of disseminated carcinoma to the vertebral column. Breast, lung, and prostate neoplasms were the most frequent tumors of origin in the 55% of patients who had vertebral lesions. The lumbar spine was the site of the greatest number of metastases. Back pain did not occur in 36% of the 179 patients with spinal disease. Cord compression occurred in 20% of the patients with vertebral involvement, and prostate tumors were the most frequent neoplasm to cause epidural spinal cord impingement. Hypernephroma was th

Metastatic esophageal adenocarcinoma to the prostate presenting with bilateral ureteral obstruction.

Carcinoma metastatic to the prostate occurs rarely and is most commonly associated with malignant bladder neoplasms. We present the case of a 73-year-old male with a history of gastroesophageal adenocarcinoma and clinically symptomatic benign prostatic hyperplasia who underwent photoselective vaporization of the prostate and presented several months later with gross hematuria, intermittent urinary retention and bilateral ureteral obstruction causing acute renal failure. After relieving the ureteral obstruction, subsequent transurethral resection of the prostate revealed locally invasive metast

Primary Prostatic Diffuse Large B-Cell Lymphoma: A Case Report and Literature Review.

INTRODUCTION: Primary lymphomas of the prostate are globally rare, representing <0.1% of all prostatic neoplasms. Their rarity and non-specific symptomatology at presentation usually prompt a clinical diagnosis of benign prostatic hyperplasia or chronic prostatitis, leading to significant delay in diagnosis. CASE DESCRIPTION: A 32-year-old man presented to our clinic with complaints of difficult urination and perineal pain. An enlarged, hard and nodular prostate was palpable on digital rectal examination. Needle biopsy of the prostate was performed, which revealed diffuse large B-cell non-Hod

GATA-3 and FOXA1 expression is useful to differentiate breast carcinoma from other carcinomas.

GATA-3, a member of the GATA family of zinc-finger DNA binding proteins, and FOXA1, a member of the forkhead transcription factor family, are both associated with estrogen receptor expression. Both GATA-3 and FOXA1 are useful markers for breast carcinoma, but their expression in the different breast cancer subtypes and other neoplasms has not been thoroughly evaluated. We examined the expression of GATA-3 and FOXA1 in estrogen receptor-positive, Her2/neu-positive, and triple-negative breast carcinomas as well as in 10 other common carcinomas, including hepatocellular, colonic, pancreatic, gast

Tumour pathology of the neuron-paraneuron system and its evolutionary background, with special attention to neuro-endocrine differentiation in prostate and mammary carcinomas.

From results of phylogenetical investigations of the neuron-paraneuron or neuroendocrine system it is known that cells producing hormonal peptides appear first in the nervous system, then also as disseminated cells of an open or closed type in the mucosa of the alimentary tract, and, lastly, as the parenchyma of the classical endocrine glands. In all these three parts of the neuroendocrine system the parenchymal cells can undergo neoplastic transformation, expressing "eutopic" and/or "ectopic" peptide hormones and/or biogenic amines. The neuro-paraneuronal tumours arising in the central or per

Potential Pitfall: Positive Follicular Lymphoma on [ 68 Ga]-PSMA-11 PET/CT for Prostate Cancer Staging.

Prostate-specific membrane antigen (PSMA)-targeted PET agents are a mainstay in staging high-risk prostate cancer. Nevertheless, PSMA expression is not exclusive to prostate cancer and is present in various neoplasms and inflammatory conditions. We report a case of biopsy-proven follicular lymphoma positive on PSMA PET/CT originally performed for staging of prostate cancer in a 66-year-old patient. This case highlights the potential pitfall of lymphoma mimicking prostate cancer nodal metastasis on PSMA PET/CT and the importance of careful evaluation of distribution and uptake of lymph nodes wh

Metastatic carcinoma to the testis: a clinicopathologic analysis of 26 nonincidental cases with emphasis on deceptive features.

Metastatic carcinomas to the testis may simulate primary testicular neoplasms, even in patients with known extratesticular primaries, but information on this topic is limited. We therefore reviewed our experience with 26 cases from consultation (N=23) or hospital (N=3) files; none of the cases were from autopsy material or incidentally discovered in therapeutic orchiectomies from patients with prostate cancer. The tumors occurred in men 29 to 90 years old, with the prostate the most common primary site (N=11), followed by the renal parenchyma (N=4), colon (N=4), urinary tract (N=3), lung (N=2)

Well-differentiated neuroendocrine tumor of the urinary bladder expressing GATA 3.

Neuroendocrine neoplasms of the urinary bladder are uncommon tumors represented by small-cell neuroendocrine carcinoma and by fewer cases of large-cell neuroendocrine carcinomas and well-differentiated neuroendocrine tumors. Less than 30 examples of this latter entity have been published so far and consisted of clinically indolent lesions mainly located in the bladder neck arranged in a pseudo-glandular architecture often associated with reactive urothelial changes like cystitis cystica/glandularis. Due to their infrequency, pathologists may face difficulty to recognize this proliferation cons

Morphological and clinical observations of patients with early bladder cancer treated with total cystectomy.

In 21 cases, early bladder cancer was detected by urine cytology, although not by cystoscopy, and was treated by total cystectomy. The neoplasms, all transitional cell carcinomas of moderate to high degrees of anaplasia, were entirely in situ in 17 of the 21 patients; in 4, although mainly in situ, the tumors showed additional minimal microinvasion. Widespread mucosal involvement was demonstrated in every case by step-sectioning, and extension into the prostatic ducts occurred in 7 of the 19 male patients and into the mucosa of one or both distal ureters in 12 patients.
