The clinical practice of Cognitive Behavioural Therapy for children and young people with a diagnosis of Asperger's Syndrome.

Children and young people diagnosed with Asperger's Syndrome (AS) have significant social-communication difficulties and impaired empathy and theory of mind skills. These difficulties place them at risk of developing mental health problems, particularly anxiety, depression and obsessive compulsive disorder. Although Cognitive Behavioural Therapy (CBT) is recognised as an effective intervention for these problems in both child and adult populations, little research has specifically looked at the use of CBT with children and young people with an AS diagnosis. However, limited evidence suggests t

Trauma-induced dissociative amnesia in World War I combat soldiers. II. Treatment dimensions.

OBJECTIVE: This is the second part of a study of posttraumatic amnesia in World War I (WW I) soldiers. It moves beyond diagnostic validation of posttraumatic amnesia (PTA), to examine treatment findings, and relates these to contemporary treatment of dissociative amnesia, including treatment of victims of civilian trauma (e.g. childhood sexual abuse). METHOD: Key WW I studies are surveyed which focus on the treatment of PTA and traumatic memories. The dissociation-integration and repression-abreaction models are contrasted. RESULTS: Descriptive evidence is cited in support of preferring Myer

Overwhelming Uncertainty: Acceptance and Commitment Therapy Informed Communication for Serious Illness Care.

Acceptance and commitment therapy (ACT) is a psychotherapeutic model that focuses on developing psychological flexibility by encouraging patients to attune to their present moment experience, cultivate openness to those experiences, and respond to those experiences with committed action that is rooted in their values. By combining the six core processes of ACT (attention to the present moment, self-as-context, defusion, acceptance, values, and committed action) with serious illness communication skills, palliative care clinicians can nurture psychological flexibility for patients facing uncert

Intolerance of uncertainty in youth with obsessive-compulsive disorder and generalized anxiety disorder: A transdiagnostic construct with implications for phenomenology and treatment.

Obsessive-compulsive disorder (OCD) and generalized anxiety disorder (GAD) are early-onset disorders with significant overlapping phenomenology, especially in young patients who, due to developmental stage, may have difficulty recognizing obsessions and worries as unrealistic or excessive. Shared phenomenology and high rates of comorbidity between OCD and GAD raise the possibility of common underlying processes, and recent work has focused on intolerance of uncertainty (IU) as a reasonable candidate. With an emphasis on the youth literature, we review the phenomenological overlap between OCD a

Failure to mourn a stillbirth: an overlooked catastrophe.

The reasons for the difficulty of mourning a stillbirth and the therapeutic management of this mourning process are outlined. A case is described of failed mourning of a stillbirth which gave rise to a profound disturbance in mothering of a subsequent live baby. A psychotherapeutic technique is discussed which facilitates the failed mourning of a stillbirth.

The evolution of psychotherapy: from Freud to prescription digital therapeutics.

The evolution of psychotherapeutic treatments from Freud to digitally administered evidence-based treatments reflects a history of progressive advance. This history is characterized by identification of problems with the current state of the art, followed by solutions inspired and supported by advances in basic science and technology leading to subsequent recognition of other limitations revealed by the new advance. The common thread running through this process is (a) increasing specificity of the psychotherapeutic interventions, (b) increasing evidence of efficacy and safety, (c) increasing

[Ambulatory psychotherapy of sex offenses].

We present the theoretical basis and results of a comprehensive research project concerning the psychotherapeutic treatment of sexual offenders in an ambulant setting. The therapeutic sample (n = 86) consists of patients with the following symptoms: exhibitionism, child molestation and sexual aggressive acts towards women. Data concerning the sexual deviant symptomatology, the general and sexual socialization and the actual psycho-social situation are demonstrated. The sexually deviant symptoms is considered in regard to basic personality deficits and general life-coping strategies of the pati

The use of effective treatments: the case of an adolescent girl with anorexia nervosa in the context of a conversion disorder.

The use of empirically supported treatment (EST) has been shown to enhance treatment outcome. The purpose of this case study was to suggest that ESTs further encourage effective reconceptualisation and the ongoing delivery of effective treatment, especially in the case of complex or atypical presentations or response to treatment. This report describes the case of an adolescent girl who underwent Maudsley family-based treatment for anorexia nervosa (AN) for a period of 12 months. Atypical response lead to an understanding of her presentation as representing a primary conversion disorder, withi

Causal effects in psychotherapy: counterfactuals counteract overgeneralization.

Causal inference of psychotherapy effects is usually based on the theory of internal and external validity. The authors argue that as an inductive strategy it often leads to overgeneralization because it promotes neglect of specific clinical boundary conditions (such as practically relevant combinations of treatments, settings, patients, and therapists). Adding the counterfactual conceptualization of causal effects counteracts overgeneralization by considering individuals at a fixed time under two possible treatment conditions as basic units of a causal effect. Consequently, causal effects are

Cross-examining dissociative identity disorder: neuroimaging and etiology on trial.

Dissociative identity disorder (DID) is probably the most disputed of psychiatric diagnoses and of psychological forensic evaluations in the legal arena. The iatrogenic proponents assert that DID phenomena originate from psychotherapeutic treatment while traumagenic proponents state that DID develops after severe and chronic childhood trauma. In addition, DID that is simulated with malingering intentions, but not stimulated by psychotherapeutic treatment, may be called pseudogenic. With DID gaining more interest among the general public it can be expected that the number of pseudogenic cases w

Therapist's Assessment of Their Patient's Session-Level Emotional Processes: Validation of the In-Session Patient Affective Reactions Questionnaire-Clinician Form.

BACKGROUND: The current study aimed to evaluate a therapist version of the in-Session Patient Affective Reactions Questionnaire (SPARQ). The SPARQ was developed to assess a pattern of emotions, thoughts, and behaviors experienced by a patient toward their therapist during a session. The SPARQ has existed only as a patient self-report measure and has demonstrated promise as a psychotherapy process measure. This study intended to validate a complementary clinician-report version of the questionnaire: the SPARQ-C. METHODS: A sample of licensed mental health clinicians (N = 151) completed the SPA

A journey through chaos and calmness: experiences of mindfulness training in patients with depressive symptoms after a recent coronary event - a qualitative diary content analysis.

BACKGROUND: Psychological distress with symptoms of depression and anxiety is common and unrecognized in patients with coronary artery disease (CAD). Efforts have been made to treat psychological distress in CAD with both conventional methods, such as antidepressant drugs and psychotherapy, and non-conventional methods, such as stress management courses. However, studies focusing on the experiences of mindfulness training in this population are still scarce. Therefore, the aim of this study was to explore immediate experiences of mindfulness practice among CAD patients with depressive symptoms

Developing the nuclear idea: concept, technique, and process.

I introduce an approach to group that has remained undeveloped in the literature, but represents an essence of relationally oriented group psychotherapy. Evolving from the verbalizations and enactments through which the group symbolizes and becomes known-a nuclear idea takes shape. It emerges from the nucleus of the group process: co-created from intersubjective forces and locations that cannot be fully specified, yet may be possible to observe, name, and utilize clinically. Groups organize themselves by developing nuclear ideas, with the therapist's active participation. They are vehicles thr

Psychotherapy--an active agent: assessing the effectiveness of psychotherapy and its curative factors.

In the last two decades there has been a remarkable expansion in research on psychotherapy in two areas: (a) assessing the effectiveness of psychotherapy compared to wait-list, no-treatment patient groups and groups treated with pharmacotherapy; (b) investigating curative factors in the therapeutic process: the extent of the psychotherapist's experience and kinds of interventions and errors in handling techniques. The first section of the review deals with the first of these areas and the second with the other. The following are the main research findings: 1. The effectiveness of psychotherapy

TMS in combination with a pain directed intervention for the treatment of fibromyalgia - A randomized, double-blind, sham-controlled trial.

BACKGROUND: Fibromyalgia Syndrome (FMS) is a highly prevalent condition, that causes chronic pain and severe reduction in quality of life and productivity, as well as social isolation. Despite the significant morbidity and economic burden of FMS, current treatments are scarce. OBJECTIVE: To investigate whether stimulation of ACC -mPFC activity by dTMS enhances a pain-directed psychotherapeutic intervention. METHODS: 19 FMS patients were randomised to receive either 20 sessions of dTMS or sham stimulation, each followed by a pain-directed psychotherapeutic intervention. With the H7 HAC coil o

Psychosomatic - psychotherapeutic treatment of stress-related disorders impacts the sphingolipid metabolism towards increased sphingosine and sphingosine-1-phosphate levels.

OBJECTIVE: Chronic stress is a risk factor for developing stress-induced mental disorders like major depression and post-traumatic stress disorder. Low-grade inflammatory processes seem to mediate this association. The sphingolipid metabolism with its most important lipid messengers ceramide and sphingosine-1-phosphate (S1P) was shown to play an important role in the pathophysiology of affective disorders and inflammation. METHOD: We conducted an exploratory trial to investigate the effect of intensive psychosomatic - psychotherapeutic treatment of stress-induced disorders on the biological l

Psychiatry during National Socialism: Contacts with relatives of the victims of NS-Euthanasia as part of a consequent Memorial Culture.

Crimes against humanity took place in many institutions as well as psychiatric departments during the Nazi dictatorship. The cruel and inhumane history of psychiatry during National Socialism is already widely known and broadly published. Victims of the Nazi regime, of the Holocaust, of racism, racial hygiene, and hereditary biology have often been the subjects of scientific inquiry; the relatives and descendants of the killed inpatients have yet to be part of a scientific or psychotherapeutic response. Analogous to the psychiatric-psychotherapeutic approach with traumatized individuals, the p

Early change in defence mechanisms and coping in short-term dynamic psychotherapy: relations with symptoms and alliance.

Several patient-related variables have already been investigated as predictors of change in psychodynamic psychotherapy. Defensive functioning is one of them. However, few studies have investigated adaptational processes, encompassing defence mechanisms and coping, from an integrative or comparative viewpoint. This study includes 32 patients, mainly diagnosed with adjustment disorder and undergoing time-limited psychodynamic psychotherapy lasting up to 40 sessions, and will focus on early change in defence and coping. Observer-rater methodology was applied to the transcripts of two sessions of
