Symptom Reduction Without Remoralization: A Randomized, Waiting-List Controlled Study Aimed at Separating Two Beneficial Psychotherapy Outcome Effects.

OBJECTIVE: Treatment effects in psychotherapy outcome research are generally based on the reduction of symptoms. Standard inclusion of other beneficial treatment effects such as remoralization (increase of hope, self-efficacy, well-being) might lead to more elaborate findings in the field of psychotherapy. On the other hand, it is also possible that symptom reduction and remoralization always go hand in hand in the experience of patients. The present study sought to experimentally test this assumption. METHOD: A total of 78 patients suffering from panic disorder were randomly assigned to brie

A randomized controlled trial testing the effects of sequential psychotherapy in depression: Changing therapist, or both therapist and method?

BACKGROUND: This study examined the effectiveness of sequential psychotherapy strategies for adults with major depressive disorder who did not respond to an initial course of cognitive behavioural therapy (CBT) or short-term psychodynamic supportive psychotherapy (SPSP). METHODS: In this randomized controlled trial, conducted in specialized mental health care in the Netherlands, from 185 non-responders from an initial 16-session treatment phase, 163 were randomly assigned 1:1:1 to one of three conditions: continuing treatment with the same therapist, switching to a different therapist, or swi

Residential Intensive Short-Term Dynamic Psychotherapy, Part 1: Accelerating the Treatment of Personality Disorders.

Intensive short-term dynamic psychotherapy (ISTDP) delivered in an individual therapy (vs. group) format has proved to be an effective treatment option for a broad range of patients with treatment-resistant mental health conditions. A novel residential program based on ISTDP was designed for patients whose personality disorders were treatment resistant (N=155), meaning that patients did not benefit from outpatient psychotherapy. The authors proposed that a specific treatment approach-residential ISTDP-can be a treatment option for patients with personality disorders. This article describes the

A brief treatment for fear of heights.

Objective To assess the effectiveness of a novel imaginal intervention for people with acrophobia. Methods The design was a randomized controlled trial with concealed randomization and blinded to other participants' intervention. The intervention was a single novel imaginal intervention session or a 15-min meditation. The setting was in Auckland, New Zealand. The participants were a convenience sample of the public with a score >29 on the Heights Interpretation Questionnaire (HIQ), a questionnaire validated against actual height exposure. The primary outcomes were the proportion of participant

Psychotherapeutic treatments in late life.

PURPOSE OF REVIEW: To assess recent research in psychotherapies for the elderly. RECENT FINDINGS: There is preliminary evidence for the effectiveness of cognitive-behaviour therapy in panic disorder, and problem-solving therapy in depression with cognitive impairment. Significant advances have been made showing the benefits of cognitive-behaviour therapy in the medically ill. Examples of public and private psychotherapy services are also described. SUMMARY: There remain many virgin territories awaiting exploration. Psychotherapists and researchers working with the elderly are encouraged to r

A methodologic approach to crisis therapy.

Resistance to using limited goal, brief psychotherapy often rests upon the mythology that a patient's problems must require a great deal of time for resolution. This paper offers a suitable rationale and methodology for psychotherapists to exercise their enormous capacity to assist patients effecting meaningful change in a brief time period.

Effects of panic-specific cognitive behavioural and psychodynamic psychotherapies on work ability in a doubly randomised clinical trial.

Objective: The effects of panic-specific psychotherapy on occupational functioning remain under-researched. This study tests whether two brief psychotherapies for Panic Disorder with or without Agoraphobia (PD/A) may generate improvement in work ability. Methods: Adults (N = 221) with a primary diagnosis of PD/A were randomised to wait-list, panic-focused psychodynamic psychotherapy (PFPP), panic control treatment (PCT), or to the choice between the two treatments. Participants completed the Work Ability Inventory (WAI) at baseline, post-treatment, and during 24-month follow-ups. Change in WAI

Effectiveness of intensive short-term dynamic psychotherapy for Tension-Type Headache (TTH): A randomized controlled trial of effects on emotion regulation, anger, anxiety, and TTH symptom severity.

Research has demonstrated the potential efficacy of intensive short-term dynamic psychotherapy (ISTDP) in addressing medically unexplained symptoms (MUS). However, the specific application of ISTDP for tension-type headache (TTH), a prevalent and common MUS, has not been extensively studied. In light of this gap in the literature, the current study investigated the effectiveness of intensive short-term dynamic psychotherapy (ISTDP) in treating TTH. The study assessed ISTDP's impact on emotion regulation (ER) capacities, levels of anxiety and anger, and TTH symptoms. 30 patients from the neurol

Prognostic indices with brief and standard CBT for panic disorder: II. Moderators of outcome.

BACKGROUND: Despite the growth of reduced therapist-contact cognitive behavioural therapy (CBT) programmes, there have been few systematic attempts to determine prescriptive indicators for such programmes vis-à-vis more standard forms of CBT delivery. The present study aimed to address this in relation to brief (6-week) and standard (12-week) therapist-directed CBT for panic disorder (PD) with and without agoraphobia. Higher baseline levels of severity and associated disability/co-morbidity were hypothesized to moderate treatment effects, in favour of the 12-week programme. METHOD: Analyses w

Personality Disorders as a Possible Moderator of the Effects of Relational Interventions in Short-Term Psychoanalytic Psychotherapy with Depressed Adolescents.

UNLABELLED: A significant proportion of adolescents suffering from major depressive disorder (MDD) are likely to have a co-morbid personality disorder (PD). Short-term psychoanalytic psychotherapy (STPP) was found to be one treatment of choice for adolescents suffering from MDD. BACKGROUND: The first experimental study of transference work-in teenagers (FEST-IT) demonstrated the efficaciousness of transference work in STPP with adolescents suffering from MDD. The usefulness of STPP may be enhanced by exploring possible moderators. METHODS: Depressed adolescents (N = 69), aged 16-18 years, we

Using brief therapeutic interventions to create change in self-efficacy and personal control of chemically dependent women.

This experimental study examined effectiveness of two group therapy models on chemically dependent women's perceptions of self-efficacy and personal control. Participants were randomly assigned to two therapeutic approaches (experiential or cognitive) or the control group. Experimental and control groups were pre- and posttested on self-efficacy and personal control. Significant differences were found for social self-efficacy, with the cognitive and experiential groups having higher scores than the control group. Changes from pre- to posttest in social self-efficacy were greatest for the cogni

Treatment of requests for sex-change surgery with psychotherapy.

The authors describe two patients whose requests for sex-change surgery represented crises in sexual identity and anxiety-masking symptoms. Brief psychotherapy enabled these patients to relinquish their belief in a surgical "cure". In evaluating such request, the psychiatrist should consider the patient's total personality rather than focusing on the genuineness of the perceived gender disorder. Whatever the final decision, the opportunity for continued psychotherapy should be provided.

Trauma, Epigenetic Alterations, and Psychotherapy.

Psychotherapy was until recently described from a biological standpoint as causing structural changes in the brain and physiological alterations of neurotransmission pathways. Current research recognizes that psychotherapy also causes changes at the level of the DNA, with alterations in epigenetic mechanisms that correlate with symptom reduction and treatment response. The authors provide a brief overview of the evolving research in epigenetics, highlighting the association between trauma, DNA methylation patterns of specific gene regions, and psychiatric disorders. They also review several st

Gut feelings 1. Mind, mood and gut in irritable bowel syndrome: approaches to psychiatric care.

OBJECTIVE: Psychiatrists are likely to encounter patients with irritable bowel syndrome (IBS). We aim to provide a clinically-focused summary of psychiatric comorbidities and management. CONCLUSIONS: IBS affects up to 15% of the population. Antidepressants and brief psychotherapy can reduce symptom severity and improve coping. These treatments are effective for patients without comorbid mental illness, as well as those with increased somatisation, health-care seeking and sexual abuse histories.

Treatment of eating disorders in primary care: a systematic review.

This review evaluated psychological treatments for Anorexia Nervosa, Bulimia Nervosa, and Binge Eating Disorder conducted in primary care. Five studies met inclusion criteria. Guided self-help cognitive-behavioral therapy via a self-help book may be a beneficial, first-line treatment for reducing binging and purging symptoms. Outcomes combining self-help with antidepressants remain unclear, although antidepressants alone may provide reduction of symptoms. High attrition and non-compliance rates among studies reviewed indicate the importance of a strong therapeutic alliance between provider and
