Machine Learning for Improved Dengue Diagnosis in Puerto Rico.

OBJECTIVES: Diagnosing dengue accurately, especially in resource-limited settings, remains challenging due to overlapping symptoms with other febrile illnesses and limitations of current diagnostic methods. This study aimed to develop machine learning models that leverage readily available clinical data to improve diagnostic accuracy for dengue, potentially offering a more accessible and rapid diagnostic tool for healthcare providers. METHODS: We used data from the Sentinel Enhanced Dengue Surveillance System in Puerto Rico (May 2012-June 2024). The Sentinel Enhanced Dengue Surveillance Syste

Oculopharyngeal Muscular Dystrophy: A Case Report From Puerto Rico.

Oculopharyngeal muscular dystrophy (OPMD) is a late-onset inherited skeletal myopathy. The diagnosis is based on a clinical presentation of blepharoptosis, dysphagia, and a positive family history of the disease in patients past 40 years of age. A 57-year-old male patient presented with ptosis without lid crease, adult-onset dysphagia, and bilateral pseudophakia. The patient underwent ptosis repair of upper eyelids via frontalis slings with silicone rods. His mother was subsequently found to have ptosis, dry eyes, and anorexia due to dysphagia, thus suggesting a probable family history. Based

Fatal Leptospira spp./Zika Virus Coinfection-Puerto Rico, 2016.

Coinfection with pathogens that cause acute febrile illness (AFI) can complicate patient diagnosis and management. This report describes a fatal case of Leptospira spp./Zika virus (ZIKV) coinfection in Puerto Rico. The patient presented with a 5-day history of AFI; reported behavioral risk factors for leptospirosis; was diagnosed with possible leptospirosis, dengue, chikungunya, or ZIKV disease; and received appropriate treatment for leptospirosis and dengue. Following a 3-day hospitalization, the patient died due to acute gastrointestinal hemorrhage, and kidney and liver failure. Serologic di

Mohs micrographic surgery: 10 year experience in Puerto Rico.

OBJECTIVE: The incidence of nonmelanoma skin cancer (NMSC) is increasing rapidly worldwide. As NMSC incidence increases, the modalities to treat this condition have become diverse. However, Mohs surgery remains the standard treatment for skin cancer in several particular locations such as the face. The objective of this study is to compare the changes, occurring over a 10-year period, in the characteristics of those cancers that were treated with Mohs micrographic surgery (MMS) at the dermatology clinics of the University of Puerto Rico as well as the modifications in the repair patterns used

Avian hosts of West Nile virus in Puerto Rico.

West Nile virus (WNV) ecology in neotropical ecosystems is poorly understood, and vertebrate hosts responsible for infecting mosquitoes remain unidentified throughout the Caribbean Basin. After a period of intense WNV transmission among sentinel chickens near Ceiba, Puerto Rico, we measured abundance of resident birds and species-specific prevalence of WNV infection. Taking the product of these measures indicates the relative number of WNV infections by species. Greater Antillean grackle (Quiscalus niger) accounted for the most WNV infections among birds in our 100-km(2) study site. In urban h

Incidence and diagnoses of HIV infection - Puerto Rico, 2006.

In 2006, 33 U.S. states and five territories had confidential, name-based, human immunodeficiency virus (HIV) infection reporting; among territories, Puerto Rico had the second highest rate of HIV infection. To characterize the HIV epidemic in Puerto Rico in 2006 (the year with the most recent available data), the Puerto Rico Department of Health and CDC analyzed data on diagnoses of HIV infection (including infections that occurred in 2006 and in previous years) and used a stratified extrapolation approach developed by CDC to generate HIV incidence estimates (the number of persons newly infec

Basic epidemiology of inflammatory bowel disease in Puerto Rico.

BACKGROUND: Previous epidemiological studies have suggested that the incidence of inflammatory bowel disease (IBD) is lower in Latin American populations. The aim of this study was to estimate the incidence of IBD in Puerto Rico, a predominantly Hispanic population. METHODS: A nonconcurrent prospective study was conducted in collaboration with private gastroenterologists in southwest Puerto Rico. Basic medical history and demographics were extracted from the medical records of patients for which a new diagnosis of IBD (Crohn's disease, CD; ulcerative colitis, UC; or nonspecified IBD) was made

[Abnormalities of sexual development in Puerto Rico: status report].

Puerto Rico presents the highest incidence in the world of anomalous sexual development. The authors have collected over 3100 cases in the past 19 years. Clinical and laboratory studies suggest possible estrogenic contamination of meats and poultry products. Variation in diet provides protection to a significant number of patients. The possibility of mycotic contamination of food employed in animal husbandry by Fusarium sp., and mycotoxins capable of estrogenic effects have been suggested in a preliminary study. Private and government investigators are active in the study of the condition, but

Results of living-related kidney transplantation in Puerto Rico.

A total of 103 kidney transplantations from living-related donors were performed in the Puerto Rico Kidney Transplant Program between January 1977 and June 1984. The majority of these patients were male, 76% were between 21 and 50 years of age, 33% were indigent, and 38% were either black or of mixed race. Rigid exclusion criteria were established in the selection of kidney donors. All donor operations were at least 2-antigen-compatible. Surgical technique and posttransplant management were standard, except that in 1980 the immunosuppressive policy was changed as follows: posttransplant predni

Association between Increased Perceived Stress and Decreased Self-Rated Health among Adults in Puerto Rico during COVID-19: A Puerto Rico-CEAL Study.

OBJECTIVE: To examine how perceived changes in stress among adults during the COVID-19 pandemic are associated with changes in self-rated health (SRH) in Puerto Rico. METHODS: This cross-sectional analysis used data from the Puerto Rico Community Engagement Alliance (Puerto Rico-CEAL). Adults aged ≥ 18 years (n = 654) completed an online survey from December 2021 through February 2022. One item assessed perceived changes in psychological stress throughout the pandemic. Two separate items assessed SRH (rated from poor to excellent) before and during the pandemic; changes in SRH ratings were ca

Hurricanes Irma and Maria and Diabetes incidence in Puerto Rico.

OBJECTIVE: To evaluate the impact of Hurricanes Irma/Maria on diabetes incidence in Puerto Rico. Mortality increased substantially after the hurricanes, but morbidity was not assessed. METHODS: We recruited 364 participants from the San Juan Overweight Adults Longitudinal Study (SOALS) aged 40-65 years who completed a three-year follow-up and were free of diabetes. We conducted additional questionnaires 1.7-2.5 years after hurricanes. Glycosylated hemoglobin (HbA1c), fasting glucose and insulin were assessed at all three visits. We compared diabetes incidence between pre-hurricane visits and

COXPD9 in an individual from Puerto Rico and literature review.

Defects of mitoribosome assembly with destabilization of mitochondrial ribosomal proteins and subsequent aberrant mitochondrial translation machinery are one of the emerging categories of human mitochondrial disease. Mitochondrial translation deficiency constitutes a growing cause of combined oxidative phosphorylation deficiency and overall causes a set of clinically heterogeneous multi-systemic diseases. We present here the sixth individual with combined oxidative phosphorylation deficiency-9 (COXPD9) secondary to a likely pathogenic homozygous MRPL3 variant c.571A > C; p.(Thr191Pro). MRPL3 e

A patient with the Weaver syndrome in Puerto Rico: a case report.

We report a 4 year-old boy with clinical features consistent with the Weaver syndrome. In addition to the typical findings, our patient had tarsal epicanthus, intermittent exotropia, hyperopia, and astigmatism. The deletion of the subtelomeric region of 18q has not been previously described in patients with the syndrome. Comparing clinical findings between patients with the de Grouchy and the Weaver syndrome suggest that both entities may be a spectrum of the same disease.
