Xanthogranulomatous pyelonephritis (XGPN) represents an unusual suppurative granulomatous reaction to chronic infection, often in the presence of chronic obstruction from a calculus. We present a case of XGPN in a horse shoe kidney in an adult. Hemi-nephrectomy of the involved side was followed by clinical improvement. The case highlights the importance of early hemi-nephrectomy in XPGN with horse shoe kidney.

Xanthogranulomatous pyelonephritis is an uncommon chronic inflammatory renal disorder. Most cases have been described in middle aged women and it is extremely rare in children. We report a case of a 4 year old girl who suffered from recurrent urinary tract infection and antibiotic therapy resistance. She had low growth-rate and palpable left renal mass on examination and was confirmed by radiological findings. We performed left kidney partial resection and then histological examination showed focal xanthogranulomatous pyelonephritis. The focal form of the disease may respond to antibiotic trea

Xanthogranulomatous pyelonephritis is a form of chronic infection of the kidney and surrounding tissues characterized by destruction and replacement of renal parenchyma by lipid-laden macrophages. Gross pathologic features include massive renal enlargement, lithiasis, peripelvic fibrosis, hydronephrosis, and lobulated yellow masses replacing renal parenchyma. Typically, the disease is diffuse and has characteristic imaging features. Less commonly, the process is focal and is difficult to differentiate from malignant disease on radiologic studies. Ultrasound demonstrates renal enlargement with

INTRODUCTION: Xanthogranulomatous pyelonephritis (XGPN) is a rare form of chronic renal inflammation, caused by long-term obstruction of the urinary tract. Pyonephrosis is a severe suppurative complication of acute obstructive pyelonephritis. Although minimally invasive approaches have many advantages, the safe dissection of the kidney may not be always achievable.
MATERIALS AND METHODS: We reviewed 27 cases diagnosed with either XGPN or pyonephrosis, who underwent laparoscopic total nephrectomy between October 2016 and March 2022 in our department. All interventions were performed using the

Xanthogranulomatous pyelonephritis (XGP) is a rare variant of chronic pyelonephritis, occurring in the setting of obstructive uropathy and recurrent urinary tract infections (UTIs). It is difficult to diagnose as it can be asymptomatic until late-stage disease. Localized symptoms such as flank pain and dysuria may be attributed to nephrolithiasis or UTIs without prompting need for further workup. Extrarenal manifestations, most notably fistula formation, may present distal to the kidney and not be readily attributed to a renal pathology. The only known definitive therapy is nephrectomy. A dela

BACKGROUND: To retrospectively determine key demographic and clinical features of 35 patients with Xanthogranulomatous pyelonephritis (XGP) in a New Zealand setting and to compare it with the existing literature.
METHODS: A retrospective review of patients having a confirmed diagnosis of XGP on histopathology examination was performed. Key clinical and demographic features were analysed and compared with the published literature.
RESULTS: XGP was diagnosed in 35 patients over a 12-year (2001-2013) period in Auckland Public Hospital. Ninety-one percent of the patients were female. The populat

Xanthogranulomatous pyelonephritis (XGP) is a rare chronic inflammatory disorder of the kidney characterised by an infectious phlegmon arising in the renal parenchyma. It is seen in patients who have urolithiasis, urinary tract infection and immunocompromised status. The clinical presentation is variable and renal neoplasm is considered as a differential due to its characteristic extrarenal visceral invasion. The treatment is almost universally extirpative and can pose a formidable challenge to the treating physician and surgeon. The authors report a rare case of XGP in a postpartal woman who

INTRODUCTION: Xanthogranulomatous pyelonephritis is a morphologically and clinically unique manifestation of chronic pyelonephritis with the formation of pus or granulomas. The most frequent predisposing factors for the development of xanthogranulomatous pyelonephritis are urinary obstruction (e. g., stones, tumors, congenital anomalies and functional impairment) and infection of the collecting system.
CASE REPORT: We describe a 2-year-old female patient with unclear abdominal complaints, diarrhea, malaise, loss of appetite, weight loss, pale skin color, and recurrent and undulating fever in

Ultrasonographic and computed tomographic findings of 21 cases of pathologically proven xanthogranulomatous pyelonephritis (XGP) in 20 patients were retrospectively evaluated. Seventeen (81%) were diffuse, and extrarenal extension occurred in 13 cases (62%). The kidney was enlarged diffusely in 12 cases (57%), and focally in 3 (14%). Urinary calculi were present in 16 cases (76%), with staghorn calculi in 4 of these, and hydronephrosis occurred in 17 (81%). In addition to typical features of XGP, the condition may also show variable imaging findings.

Four patients with xanthogranulomatous pyelonephritis were found to have renocolic fistulae. Coincidentally, the left kidney was involved in all four cases. All patients presented with renal mass. Two cases have had coexistent renal stones, one of them presented with massive upper gastrointestinal bleeding as a result of portal hypertension. Another patient had a history of Schistosomiasis. In none of the patients was the renal condition confidently diagnosed preoperatively, nor was the colonic fistula suspected. In all four patients, nephrectomy was performed together with resection of the in

The coincidence of systemic amyloidosis and xanthogranulomatous pyelonephritis has been reported previously only once. Clinical findings, such as the nephrotic syndrome, cardiac and autonomic nervous system dysfunction, and adrenal insufficiency, are suggestive and a thorough investigation to rule out other causes of secondary amyloidosis is warranted. We report a case of xanthogranulomatous pyelonephritis associated with secondary systemic amyloidosis and the nephrotic syndrome. Treatment consisted of nephrectomy and intensive supportive care. The unique clinical, radiographic and pathologica

Xanthogranulomatous pyelonephritis is associated with obstruction, stones and infection. CT is the mainstay of diagnosis, but appearances can mimic other conditions, including renal cell carcinoma. Nephrectomy is commonly recommended, but conservative treatment with antibiotics has been described after tissue diagnosis. We present a case of xanthogranulomatous pyelonephritis with concomitant renal cell carcinoma, which was an association that was suggested in 1988 and supported by subsequently reported cases. Conservative management of biopsy or cytology proven xanthogranulomatous pyelonephrit

Xanthogranulomatous pyelonephritis (XGP) is a chronic suppurative infectious process that only rarely affects pediatric patients, and most commonly occurs in the setting of a large obstructing calculus. Histologically, XGP is characterized by the presence of chronic inflammation and lipid-laden macrophages. This case report illustrates the radiological, surgical, and pathologic findings in a young patient who presented to our institution for treatment of this uncommon condition. Although rare, xanthogranulomatous pyelonephritis is a clinically important entity that can affect pediatric patient

INTRODUCTION AND OBJECTIVE: Xanthogranulomatous pyelonephritis (XGP) is a chronic inflammatory condition of the kidneys. Nevertheless, disparities between the pre-operative and pathological diagnoses are frequently encountered. We reviewed all patients with XGP over a 17-year period to identify and characterize the pre-operative and pathological characteristics of the disease in our patients.
METHODS: A comprehensive review of all nephrectomy patients with a pre-operative diagnosis of pyelonephritis at National Taiwan University Hospital from 1991 to 2008 with the pathological diagnosis of XG

A 73-year-old woman with leukemia presented with urinary tract infection, splenic abscess, and a renal mass. Both urine culture and pus culture of the splenic abscess yielded Klebsiella pneumoniae. Percutaneous biopsy of the renal mass confirmed the diagnosis of xanthogranulomatous pyelonephritis. Because of high risk for surgery, the patient received treatment with antibiotic therapy for 2 months. With antibiotic therapy, not only was the splenic abscess cured but follow-up ultrasonography also showed progressive resolution of the renal mass. Xanthogranulomatous pyelonephritis is frequently a

OBJECTIVES: Xanthogranulomatous pyelonephritis is an atypical chronic infection which simulates a malignant renal neoplasia, definitive diagnosis of which is obtained by pathologic study after surgical removal. In this paper we analyze our experience and compare it to that reported on the literature.
METHODS: We performed a retrospective study of cases of xanthogranulomatous pyelonephritis diagnosed in the period from January 1945 to December 2000 at the Institute of Neoplastic Diseases, analyzing clinical, radiological, and surgical features documented in the medical records.
RESULTS: There

Clinicopathologic features in a series of 11 cases of xanthogranulomatous pyelonephritis are reviewed. There were seven males and four females ranging in age from 6-56 years with an average age of 34 years. The disease was unilateral in ten cases and bilateral in one. There was associated urinary tract obstruction in eight cases due to renal or ureteric stones (5), ureteric stricture (2), and carcinoma of the urinary bladder (1). Two of the patients were renal transplant recipients in which native kidneys were involved by xanthogranulomatous pyelonephritis. In one of these cases, a small renal
