Barriers to the best care of the dying in Queensland, Australia.

The process of dying for many Australians is not ideal. To improve the care of the dying in our community, the barriers preventing optimal care must be identified. Forty-two important barriers were identified by focus groups. Health care professionals (HCPs) working in palliative care (PC) throughout Queensland were asked to rate the importance of each of the barriers. Inadequate funding for PC, lack of after-hours care, insufficient medical support and the lack of HCPs in PC across several different settings were highlighted as the most important barriers. Uncertainty regarding death certific

Death of the first white resident of North Queensland.

The first white resident of North Queensland's death certificate gives the final illness as 'arthritis'. This examination of contemporary records and more recent reports, together with the results of discussion with colleagues interested in medicine and history, attempts to suggest the reasons for his various symptoms and his final demise. This life story is reminiscent of a 'Boy's own' adventure with shipwrecks, survival at sea, coexistence with Aboriginal tribesmen before returning to 'white society', marriage and the start of a family. Are there lessons here for the twenty-first century phy

A survey of the current surgical treatment of gallstones in Queensland.

BACKGROUND: To survey the current practice of general surgeons in Queensland in their management of patients with gallstones. METHODS: A postal survey of 123 surgeons practising in Queensland. RESULTS: There were 114 responses to the questionnaire (92.7%) and seven were excluded from the analysis. Laparoscopic cholecystectomy (LC) was preferred by 97.2% and operative cholangiography was almost always attempted by 82.3%. One-third of surgeons used drains routinely. In cases of acute cholecystitis, 56 surgeons (52.3%) favoured laparoscopic cholecystectomy on the same admission. Operative chola

Fulminant hepatitis A in indigenous children in north Queensland.

Since 1993, three Indigenous children in north Queensland have died of fulminant hepatitis A. Even if the children had been able to undergo liver transplantation, prolonged immunosuppressant therapy and the likelihood of opportunistic infections would inevitably have jeopardised any chance of long-term survival. As hepatitis A has become a leading infectious cause of death in young Indigenous children in north Queensland, hepatitis A vaccine has recently been introduced into the vaccination schedule for these children.

Review of leptospirosis notifications in Queensland 1985 to 1996.

To provide an overview of leptospirosis in Queensland, the World Health Organization/Food and Agriculture Organization Collaborating Centre for Reference and Research on Leptospirosis undertook a study of notifications of the disease from 1985 to 1996. The review encompassed information drawn from notifications to Queensland Health and questionnaires sent to doctors. Notifications were highest between February and July and the highest population rates were identified in the Central West and Peninsula Health Regions. Ninety-one per cent of notifications were for males. At risk population groups

Equine onchocerciasis in Queensland and the Northern Territory of Australia.

Investigations were conducted on the taxonomy, distribution in the carcase, pathology and transmission of Onchocerca spp. in equids from Queensland and the Northern Territory. Examination of small groups of horses and ponies revealed high infection rates with O. cervicalis, while lesser numbers were infected with O. gutturosa. O. reticulata was not found. Neither of the Australian species is likely to be of economic importance to the horsemeat industry. The findings support the belief that O. cervicalis is a pre-disposing factor in the aetiology of equine nuchal disease, most commonly seen cli

Epidemiology of notified chlamydia infections, Queensland, 2000-2019.

BACKGROUND: Chlamydia infection is a frequently reported notifiable condition in Queensland, with the highest prevalence observed among young people. Infection can occur in several sites including the anus, eye, pharynx, cervix/vagina, and urethra. Currently, historical trends in chlamydia by site of infection in Queensland are unknown. METHODS: A descriptive analysis of all chlamydia notifications in Queensland, 2000-2019, was performed. Queensland Health statewide notification data were classified as either anorectal, eye, genitourinary, upper respiratory tract, or other/unspecified. The nu

Establishing the Queensland Pelvic Mesh Service: Preliminary experience.

BACKGROUND: Transvaginal mesh (TVM) has been used for treatment of pelvic organ prolapse (POP) and stress urinary incontinence (SUI). Mesh-related complications are reported in 3% of women receiving mid-urethral sling surgery for SUI and in up to 20% of women who receive TVM for POP. The Australian Senate Enquiry report in March 2018 recommended that each Australian state establish specialist multidisciplinary units for management of TVM complications. AIMS: The aims of this study are to report on the setting up of the Queensland Pelvic Mesh Service (QPMS) and summarise its first 24 months to

Major lower limb amputations in Far North Queensland.

BACKGROUND: Major lower limb amputation is a devastating operation most commonly performed for complications of peripheral artery disease or diabetes mellitus. Data suggest that there is a widespread variation in major amputation rates within and between countries. This study aimed to identify key characteristics of patients undergoing this procedure in our region, and to compare our population to the rest of Australia. Secondary analysis was performed to assess differences seen in the Indigenous population. METHODS: Cases were identified from a prospectively maintained database and medical r

Queensland tick typhus in rural New South Wales.

We report five cases of Rickettsia australis infection from southern coastal New South Wales, Australia. All patients presented with a cutaneous eruption of erythematous papules and pustules and systemic features of malaise, headache, lymphadenopathy and myalgia. Acute kidney injury (AKI) was present in two of five cases and one of five cases had acute delirium. Improvement was only seen after treatment with doxycycline 100 mg b.i.d. Positive serology for R. australis was present in four of five cases and a positive polymerase chain reaction (PCR) was seen in one of five cases. Histology showe

Plasmodium vivax malaria acquired in far north Queensland.

In February 1996, vivax malaria was diagnosed in a man from a remote community in far north Queensland who had not visited a malarious area for the past 19 years. Microscopy and DNA studies of blood from other residents of the community did not identify a source of infection. It was suspected the infection was transmitted by mosquitoes from a neighbour who had been infected in Papua New Guinea, but whose blood was not available for DNA tests.

Leptospiral infection in domestic mares in North Queensland.

Leptospira species are found worldwide, favouring tropical regions, and infect a wide range of animal species. Although renal persistence in infected individuals and excretion in urine is thought to be the primary mechanism of disease transmission, recent reports have suggested that persistence in the reproductive tract may be a feature in certain species, including the horse. The aim of this study was to investigate leptospiral infection, particularly within the reproductive tract, in healthy, non-breeding mares. Serum and endometrial swab samples were collected from 50 mares from the James C

Expanding the Geographic Boundaries of Melioidosis in Queensland, Australia.

Melioidosis is an infectious disease caused by the bacterium Burkholderia pseudomallei. Although this environmental organism is endemic in certain regions of Australia, it is not considered endemic in Southern Queensland, where the last case was reported 21 years ago. We report a climate change-associated outbreak of melioidosis occurring during two La Niña events in a region previously considered nonendemic for B. pseudomallei. During a 15-month period, 14 cases of locally acquired melioidosis were identified. Twelve patients were adults (> 50 years), with diabetes mellitus the most common ri
