We describe two cases of venous reconstruction after radial artery (RA) harvesting for coronary artery bypass grafting (CABG). Patient 1, a 70-year-old man who underwent CABG 16 years earlier, presented with acute posterolateral myocardial infarction caused by total occlusion of the saphenous vein graft (SVG). RA grafting was considered ideal, but he had bilateral hypoplastic ulnar arteries. Venous reconstruction with RA harvesting using SVG was performed, and surgery was successful. Patient 2, a 57-year-old-man with chronic renal failure caused by polycystic kidney disease, presented with ang

A 26 year old women with a history of drug intoxication was taken to intensive care unit and had an ischemic, hand injury due to insufficient collateral low after placement of a radial artery catheter. Ischemic hand injury 1 occurred 36 hours after the cannulation. The patient had fingers gangrene in spite of the removal of the catheter, elevation and heating of the hand, systemic anticoagulation therapy, stellate ganglion blockade, and peripheral circulatory regulator therapy. Any evidence of hand ischemia after radial artery cannulation should be aggressively investigated and promptly treate

Percutaneous radial artery cannulation is commonly used for continuous monitoring of blood pressure and estimation of arterial blood gases. The purpose of this study is to define the incidence of radial artery thrombosis and associated hand ischaemia after cannulation. A prospective study of 40 patients who underwent radial artery cannulation was carried out with patients examined before and after cannulation for radial and ulnar pulses. Doppler waveforms, and finger/brachial and finger/wrist pressure indices were obtained. After cannulation 27.5% of patients developed abnormal radial artery f

INTERVENTIONS: Directed history and hand examination (palpation of wrist pulses, Doppler Allen test, and Doppler interrogation of palmar arch and digital vessels).
MAIN OUTCOME MEASURES: History of hand claudication, weakness, or cold sensitivity or paresthesias and patency of wrist

BACKGROUND: Radiation exposure is one of the most adverse occupational hazards faced by interventional cardiologists. Various arterial access sites have shown to yield different operator radiation exposure during diagnostic cardiac catheterization.
METHODS: This single-center randomized controlled trial assessed the cumulative radiation exposure and normalized radiation exposure at 4 different anatomic locations (thorax, abdomen, left eye, and right eye) of the primary operator when using the left radial artery (LRA) approach compared with a uniform hyper-adducted right radial artery (HARRA)

Radial artery catheterization is commonly performed using the pulse palpation method. However, anatomical variations, such as the persistent median artery-an embryonic structure that may persist into adulthood-can complicate this procedure. A 47-year-old female scheduled for laparoscopic surgery required radial artery catheterization for hemodynamic monitoring. Multiple attempts to place a 22-gauge catheter in the left radial artery were unsuccessful. During the procedure, a pulsating vessel was identified on the ulnar side of the radial artery. Vascular visualization devices revealed the pres

BACKGROUND: The aim of this study was to evaluate the incidence of complications in the upper limbs as a new event after radial artery (RA) harvesting for coronary artery bypass grafting (CABG).
METHODS: From June 1997 to August 2001, the RA graft was used in 271 patients who underwent cardiac surgery at our department. These patients were prospectively reviewed. All patients were preoperatively examined to determine the presence of normal sensation and circulation in the upper limbs; then we evaluated the incidence of complications at discharge, 8 weeks, and 6 months.
RESULTS: No donor arms

A case of iatrogenic intracranial artery dissection is reported. A 52-year-old female developed severe headache and nausea. Brain CT showed diffuse subarachnoid hemorrhage. On admission, carotid angiography revealed an aneurysm in the right middle cerebral artery and the intact right internal carotid artery. The aneurysm was clipped successfully. Carotid angiography on day 7 revealed dissection in the right internal carotid artery. Repeated angiograms at 10 and 31 days showed progression of the carotid artery dissection. Findings of ECD-SPECT on day 31 (Balloon occlusion test) suggested low pe

Coronary angioplasty with 6F guiding catheters via the radial artery is associated with a minimal risk for major entry site-related complications. Although the incidence of radial artery occlusion (RAO) in the literature is approximately 30% after prolonged cannulations, little is known about the incidence and its clinical consequences of RAO following transradial percutaneous coronary angioplasty. In a prospective study, 563 patients with a normal Allen test were evaluated on patency and function of the radial artery after transradial angioplasty, by physical and ultrasound examination at dis

PURPOSE: To compare the patency outcomes and safety profile between the modified TR Band and the PreludeSYNC DISTAL (PSD) for hemostasis following distal transradial access (dTRA) for transarterial chemoembolization (TACE).
METHODS: This prospective randomized trial enrolled 104 participants undergoing TACE via dTRA (143 procedures, all performed using a 4-Fr catheter. Participants achieved hemostasis with either the modified TR Band (n = 74) or PSD (n = 69). The primary endpoint was the incidence of radial artery occlusion (RAO) assessed by Doppler ultrasonography at 4 h, 24 h, and > 1 week.

Protocols for hemostasis after trans-radial approach (TRA) vary depending on the institute as there is no established evidence-based protocol. This study aimed to investigate the clinical implications of radial compression protocols. Consecutive patients who underwent outpatient invasive catheter angiography before and after April 2018 were treated with traditional and new protocols, respectively. Using the same hemostasis band, in the conventional protocol, fixed amount of air was removed soon after the procedure, 2 h later, and 3 h later, whereas the air was removed as much as possible every

BACKGROUND: The purpose of the present paper was to determine the radial nerve morbidity associated with harvest of different venous conduits in the radial artery free flap.
METHODS: Retrospective study comparing subjective and objective symptoms of radial nerve morbidity in 51 consecutive free flaps over a 3-year period.
RESULTS: Damage to the radial nerve occurred in 9% (1/11) of patients in the cephalic vein group versus 18% (4/28) in the venae comitante group.
CONCLUSIONS: Raising the cephalic vein for venous drainage in the radial artery free flap doubles the incidence of radial nerve

Although the radial arterial system is fairly consistent in structure, rare anomalies have been encountered. During the elevation of a radial forearm free flap for floor of mouth reconstruction, the authors have encountered an accessory branch of the radial artery arising from the main radial artery at the level of the mid forearm, extending laterally over the brachioradialis muscle, and descending into the dorsal wrist in the subcutaneous plane. This artery, named as the superficial radial artery, is a very rare vascular anomaly of the radial arterial system in the forearm. The reconstructive

BACKGROUND: Radial artery occlusion complicates transradial access, limiting future access and precluding bypass grafting and dialysis. Distal radial access offers a promising solution by allowing retrograde recanalization of occluded radial arteries.
OBJECTIVES: The authors sought to evaluate multicenter outcomes of radial artery recanalization though distal radial access (DRA), focusing on its safety, efficacy, and reproducibility, while identifying procedural strategies to optimize patency.
METHODS: This international registry included 110 patients with radial artery occlusion from 7 cent

AIMS: This study aims to verify the feasibility and safety of percutaneous coronary intervention (PCI) after a distal transradial approach (dTRA) with radial artery occlusion (RAO) recanalization.
METHODS: Between July 2018 and January 2022, 30 patients underwent PCI following attempted RAO recanalization via dTRA. Among these cases, the target radial arteries could not be recanalized in five patients, necessitating alternative vascular access. The remaining 25 patients with successful RAO recanalization were divided into a standard group (n = 19) and tough group (n = 6), the latter requiring
