[Intravesical radioimmunotherapy of carcinoma in situ of the urinary bladder after BCG failure].

BACKGROUND: In failure to respond to bacillus Calmette-Guérin (BCG) in patients with carcinoma in situ (CIS) of the urinary bladder, radical cystectomy remains the mainstay after BCG failure. OBJECTIVES: The aim of this pilot study was to evaluate tolerability and safety of the α‑emitter radioimmunoconjugate instillation in patients after BCG failure. MATERIALS AND METHODS: Nine patients were included. After emptying the bladder via a transurethral catheter, Bi-213-anti-EGFR-mAb was instilled. Treatment was terminated by emptying of the radioimmunoconjugate from the bladder 120 min after ins

Synthesis and application of a novel cysteine-based DTPA-NCS for targeted radioimmunotherapy.

INTRODUCTION: For the development of safe and effective protein-based radiolabeled complexes such as radioimmunotherapy (RIT), the selection of the radionuclides and the chelating agents used for the radiolabeling of tumor-targeting molecules is a critical factor. We aim to synthesize a novel bifunctional chelating agent containing the isothiocyanate group for easy conjugation with antibodies having the characteristics of high stable chelation with therapeutic radionuclides. METHODS: We have synthesized the DTPA analogue retaining L-cysteine as a core ligand of the thiol group. The chelating

Radioimmunotherapy for the treatment of non-Hodgkin lymphoma: current status and future applications.

Radioimmunotherapy (RIT) has proved to be a safe and effective treatment for patients with relapsed or refractory indolent non-Hodgkin lymphoma (NHL) including rituximab-refractory follicular lymphoma. Further, FDA approval was recently granted for use in newly diagnosed follicular lymphoma as consolidative therapy immediately following induction chemotherapy. We detail herein the scope of clinical studies performed in relapsed/refractory and newly diagnosed indolent lymphoma and summarize the associated safety data. In addition, we discuss new applications of RIT that have been investigated i

Development and evaluation of a pharmacokinetic model for prediction of radioimmunotherapy based on pretherapy data.

The aim of this work was to develop a pharmacokinetic model for the analysis of the pharmacokinetics of (111)Inlabeled monoclonal antibodies (mAbs) in B-cell lymphoma patients and to evaluate the model's ability to predict a subsequent radioimmunotherapy by (90)Y-labeled mAbs. Data from quantified scintillation camera images and blood samples were used to fit a compartment model. The modeling included two steps: 1) a two-compartment model describing the total-body kinetics for the estimation of a set of global parameters and 2) a multicompartment model for estimating the model parameters for o

Targeted monoclonal antibody therapy and radioimmunotherapy for lymphoproliferative disorders of the ocular adnexa.

PURPOSE OF REVIEW: The goal of this review is to update ophthalmologists and orbital and adnexal specialists with the emerging role of targeted monoclonal antibody therapy and radioimmunotherapy for orbital and adnexal non-Hodgkin's lymphoma and other lymphoproliferative disorders of the orbit. RECENT FINDINGS: Rituximab as monotherapy for indolent lymphoma of the orbit and conjunctiva may be associated with good initial response, but the risk of distant relapse is about 50%. Rituximab may be used in combination with conventional chemotherapy, and such combination therapy may be associated wi

Current results and future applications of radioimmunotherapy management of non-Hodgkin's lymphoma.

Monoclonal antibodies labeled with radionuclides have become an important therapeutic tool in the treatment of patients with non-Hodgkin's lymphomas (NHL). At the present time, their use in the US is approved for patients with rituximab-resistant, low-grade, follicular or transformed NHL. Encouraging responses seen in the relapsed and refractory patients have prompted their evaluation in earlier disease or in other histologic sub-types either alone or in combination with conventional chemotherapy. Additionally, they have been included as preparative regimens for stem cell transplant protocols

Radiation-Triggered Chemokine-Secreting Probiotics for Boron Neutron Capture Therapy Based Radioimmunotherapy.

An ideal radioimmunotherapy eradicates primary tumors and drives immune clearance of residual or metastatic tumor cells, but its clinical translation is severely limited by systemic immune toxicity and the immunosuppressive tumor microenvironment (TME). Here, we constructed engineered Escherichia coli Nissle 1917 with a tannic acid-mediated boronophenylalanine coating and a radiation-responsive genetic circuit, enabling TME-responsive boron delivery for boron neutron capture therapy (BNCT). BNCT-mediated selective tumor killing spared intratumoral probiotics, whose integrated genetic circuit w

Size Matters: Superior Efficacy of α Particle Radioimmunotherapy in Micrometastases versus Macrometastases.

Radiopharmaceuticals, which emit either α or β particles, have changed the landscape of prostate cancer imaging and treatment. Each particle type has unique physical properties that can be exploited to optimize treatment effect. Using murine models of disseminated prostate cancer and a CD46-targeted α particle-emitting radioimmunoconjugate, a recent study shows superior efficacy in mice bearing micrometastastic versus macrometastatic disease. See related article by Bidkar et al., p. 2963.

Preclinical study of 212Pb alpha-radioimmunotherapy targeting CD20 in non-Hodgkin lymphoma.

BACKGROUND: Despite therapeutic advances, Non-Hodgkin lymphoma (NHL) relapses can occur. The development of radioimmunotherapy (RIT) with α-emitters is an attractive alternative. In this study, we investigated the potential of α-RIT in conjunction with 212Pb-rituximab for the treatment of NHL. METHODS: EL4-hCD20-Luc cells (mouse lymphoma cell line) were used for in vitro and in vivo studies. Biodistribution and efficacy studies were performed on C57BL/6 mice injected intravenously with 25 × 103 cells. RESULTS: 212Pb-rituximab (0.925-7.4 kBq/mL) inhibit proliferation of EL4-hCD20-Luc cells in

Tumor microenvironment modulation by SERPINE1 increases radioimmunotherapy in murine model of gastric cancer.

An elevated extracellular matrix (ECM) and interstitial fluid pressure (IFP) in gastric cancer limits the targeting of HER2-expressing cells when radioimmunotherapy (RIT) with 64Cu-trastuzumab (64Cu-TRZ) is utilized. Here, we used Losartan (LOS) to downregulate ECM and IFP in gastric cancer mice model. In our study we treated the gastric cancer mice model with a dose of 40 mg/kg of LOS. We found that the LOS treatment increases a twofold higher Alexa-647-TRZ accumulation which significantly enhanced 64Cu-TRZ. We determined that the LOS-treated samples exhibited reduced mRNA and protein express

CD38-bispecific antibody pretargeted radioimmunotherapy for multiple myeloma and other B-cell malignancies.

Pretargeted radioimmunotherapy (PRIT) has demonstrated remarkable efficacy targeting tumor antigens, but immunogenicity and endogenous biotin blocking may limit clinical translation. We describe a new PRIT approach for the treatment of multiple myeloma (MM) and other B-cell malignancies, for which we developed an anti-CD38-bispecific fusion protein that eliminates endogenous biotin interference and immunogenic elements. In murine xenograft models of MM and non-Hodgkin lymphoma (NHL), the CD38-bispecific construct demonstrated excellent blood clearance and tumor targeting. Dosimetry calculation

Three-step radioimmunotherapy with yttrium-90 biotin: dosimetry and pharmacokinetics in cancer patients.

A three-step avidin-biotin approach has been applied as a pretargeting system in radioimmunotherapy (RIT) as an alternative to conventional RIT with directly labelled monoclonal antibodies (MoAbs). Although dosimetric and toxicity studies following conventional RIT have been reported, these aspects have not previously been evaluated in a three-step RIT protocol. This report presents the results of pharmacokinetic and dosimetric studies performed in 24 patients with different tumours. Special consideration was given to the dose delivered to the red marrow and to the haematological toxicity. The

Combined modality radioimmunotherapy with Taxol and 90Y-Lym-1 for Raji lymphoma xenografts.

Despite effective therapies for non-Hodgkin's lymphoma (NHL), the majority of patients are not cured. Radioimmunotherapy (RIT) has shown good results in preclinical and clinical trials even in patients that are non-responsive to standard chemotherapy. To make RIT more effective, agents such as paclitaxel (Taxol), that can enhance radiation effects, are being tested. Nude mice bearing human Burkitt's lymphoma (Raji) xenografts were treated with: 1) 150 or 200 microCi (5.5 or 7.3 MBq) of 90Y-2IT-BAD-Lym-1 alone, 2) 600 micrograms of Taxol alone, 3) 150 or 200 microCi of 90Y-2IT-BAD-Lym-1 plus 60

Engineering TLR7/8 Agonist-Loaded and Tumor-Anchored Gold Nanosensitizers for Enhanced Radioimmunotherapy.

Immunomodulators that can amplify the in situ vaccine effect by regulating radiation-induced immunosuppression have been proven to play a pivotal role in improving the radioimmunotherapeutic efficacy of tumors. Nevertheless, they still face challenges to be addressed for effective tumor radioimmunotherapy, such as poor tumor specificity, rapid clearance from lesions, and potential side effects. Herein, a folate receptors (FRs)-targeting and protein sulfenic acid (PSA)-reactive gold nanosystem FA-dAu-CDR is rationally designed and fabricated in which the toll-like receptor 7/8 immunoagonists ar

Nanosensitizer for Cancer Radioimmunotherapy via Anti-IL-35 Blockade Boosted Innate Immunity Activation.

The cGAS-STING signaling pathway has emerged as a promising target for cancer immunotherapy. However, STING agonists have a dual-edged nature. Although they enhance antitumor T cell activity, STING agonists also elicit protumorigenic effects by promoting IL-35-producing B regulatory (Breg) cells, suppressing natural killer (NK) cell density, and fostering immune suppression. To address these challenges, this work develops a tumor microenvironment-responsive hollow mesoporous nanosystem that degrades under high glutathione conditions, thereby releasing the STING agonist MSA-2 and Mn ions. Selective Personalized RadioImmunotherapy for Locally Advanced Non-Small-Cell Lung Cancer Trial (SPRINT).

PURPOSE: Standard therapy for locally advanced non-small-cell lung cancer (LA-NSCLC) is concurrent chemoradiotherapy followed by adjuvant durvalumab. For biomarker-selected patients with LA-NSCLC, we hypothesized that sequential pembrolizumab and risk-adapted radiotherapy, without chemotherapy, would be well-tolerated and effective. METHODS: Patients with stage III NSCLC or unresectable stage II NSCLC and an Eastern Cooperative Oncology Group performance status of 0-1 were eligible for this trial. Anti-CD25 radioimmunotherapy with BEAM autologous hematopoietic cell transplantation conditioning in Hodgkin lymphoma.

High-risk relapsed or refractory (R/R) classical Hodgkin lymphoma (HL) is associated with poor outcomes after conventional salvage therapy and autologous hematopoietic cell transplantation (AHCT). Post-AHCT consolidation with brentuximab vedotin (BV) improves progression-free survival (PFS), but with increasing use of BV early in the treatment course, the utility of consolidation is unclear. CD25 is often expressed on Reed-Sternberg cells and in the tumor microenvironment in HL, and we hypothesized that the addition of 90Y-antiCD25 (aTac) to carmustine, etoposide, cytarabine, melphalan (BEAM)
