Impact of eFOV CT distortion on surface-guided radiotherapy registration: Phantom-based analysis.

PURPOSE: This study evaluated the impact of extended field-of-view (eFOV) CT distortion on the alignment accuracy of surface-guided radiation therapy (SGRT) using CT-based reference surfaces, and examined the influence of beam center location. METHODS: An anthropomorphic phantom, segmented into chest, abdomen, and pelvis, was scanned at the center position (sFOV) and at four off-center positions. The center corresponds to the CT bore isocenter, while the off-center positions were obtained by placing the phantom boundary 315 mm or 390 mm to the left or right of the CT isocenter to simulate eFO

Direct aperture deformation: an interfraction image guidance strategy.

A new scheme, called direct aperture deformation (DAD), for online correction of interfraction geometric uncertainties under volumetric imaging guidance is presented. Using deformable image registration, the three-dimensional geometric transformation matrix can be derived that associates the planning image set and the images acquired on the day of treatment. Rather than replanning or moving the patient, we use the deformation matrix to morph the treatment apertures as a potential online correction method. Measurement-based range evaluation for quality assurance of CBCT-based dose calculations in adaptive proton therapy.

PURPOSE: The implementation of volumetric in-room imaging for online adaptive radiotherapy makes extensive testing of this image data for treatment planning necessary. Especially for proton beams the higher sensitivity to stopping power properties of the tissue results in more stringent requirements. Current approaches mainly focus on recalculation of the plans on the new image data, lacking experimental verification, and ignoring the impact on the plan re-optimization process. The aim of this study was to use gel and film dosimetry coupled with a three-dimensional (3D) printed head phantom (b

Evaluation of OPTOS wide-field fundus image projections for radiotherapy planning of uveal melanoma.

PURPOSE: To investigate the relationship between the stereographic and azimuthal equidistant projection (AEP) of the human retina for radiotherapy planning with OPTOS optomap wide-field fundus images (Optos, UK). Further, the geometric accuracy of an OPTOS fundus image is quantified. METHODS: The fundamental relationship between both projection modes was applied to transform images acquired with an OPTOS Silverstone camera to the azimuthal equidistant projection using MATLAB. Fundus images of four patients were used to quantitatively demonstrate the impact for neglecting the proper projection

Measurement of the volume CT dose index on spiral CT scanning with a real-time ionization chamber.

BACKGROUND: The measurement of computed tomography dose index 100 ( C T D I 100 $CTD{{I}_{100}}$ ), which is feasible only through axial scanning, requires that the clinical spiral protocols be replaced with those for axial scanning. The real-time ionization chamber detects the integral of radiation dose rate profile, enabling the direct verification of the volume CTDI on spiral CT scanning ( C T D I v o l S p i r a l $CTDI_{vol}^{Spiral}$ ). PURPOSE: This study aimed to develop a direct measurement technique for C T D I v o l S p i r a l $CTDI_{vol}^{Spiral}$ and compare its accur

The effects of mega-voltage CT scan parameters on offline adaptive radiation therapy.

TomoTherapy involves image-guided radiation therapy (IGRT) using Mega-voltage CT (MVCT) for each treatment session. The acquired MVCT images can be utilized for the retrospective assessment of dose distribution. The TomoTherapy provides 18 distinct imaging conditions that can be selected based on a combination of algorithms, acquisition pitch, and slice interval. We investigated the accuracy of dose calculation and deformable image registration (DIR) depending on MVCT scan parameters and their effects on adaptive radiation therapy (ART). We acquired image values for density calibration tables

BACKGROUND: Conventional cone-beam computed tomography (CBCT) systems are limited by suboptimal image quality, inaccurate Hounsfield unit (HU) calibration, and reduced reliability for dose calculation. HyperSight CBCT on the Halcyon platform offers improved HU accuracy, expanded field-of-view (FOV), and enhanced image quality. PURPOSE: This study aimed to assess the dosimetric accuracy of treatment planning using HyperSight CBCT through phantom-based dose verification. METHODS: This study included three institutions equipped with the HyperSight imaging system on the Halcyon platform, with al

Physics-informed patient-specific calibration for monocular visual-based dose reconstruction in breast cancer radiotherapy.

BACKGROUND: Interfractional anatomical variations during breast cancer radiotherapy can significantly deviate the delivered dose from the treatment plan. Standard image guidance via Cone-Beam CT (CBCT) is typically sparse due to radiation and workflow constraints, leaving daily geometric changes unmonitored. PURPOSE: To bridge this gap, we propose a physics-informed framework that utilizes monocular surface vision for continuous, non-ionizing 3D dose reconstruction. METHODS: The system utilizes the weekly CBCT scans from the first 2 weeks of treatment (Week 1 and Week 2) for patient-specific

Computed tomography for four-dimensional dose calculation: Effect of detector array length.

PURPOSE: Four-dimensional computed tomography (4DCT) is susceptible to a geometrical error when respiration changes upon patient shift. 4DCT using 16-cm detector arrays, not needing the shift, has been shown to improve geometrical accuracy, compared with a conventional 4DCT (4-cm array), albeit with some Hounsfield unit discrepancies. The 4DCTs were validated for 4D planning. MATERIALS AND METHODS: A lung-mimicking phantom containing a spherical target with ten respiratory traces and three tumor-shaped targets with their traces was respectively imaged when, to each of the ten-positions/phases

Robust automated method of spatial resolution measurement in radiotherapy CT simulation images.

BACKGROUND: Variation in imaging protocol, patient positioning, and the presence of artifacts can vary image quality in CT images used for radiotherapy planning. Automated methods for spatial resolution (SR) estimation exist but require further investigation and validation for wider adoption. PURPOSE: To validated previously existing algorithm for SR estimation and introduce improvements that make it robust to patient positioning, CT protocol, site, and artifacts. METHOD: A reference algorithm based on the previous gold standard was recreated and modified to improve robustness. The algorithm

Automatic adaptive radiotherapy triggering based on CBCT using deep learning for esophageal cancer underwent volumetric modulated arc therapy.

BACKGROUND: Esophageal cancer (EC) is a highly fatal malignancy for which radiotherapy plays a critical role in treatment. However, anatomical changes during radiotherapy can lead to increased doses to surrounding organs at risk (OARs). Adaptive radiotherapy (ART) is a strategy that incorporates patient-specific anatomic changes into treatment plan modification to minimize overdose to surrounding healthy tissues. Identifying appropriate triggers with in-room verification imaging is critical to maximize the benefits of ART and prevent additional imaging dose to a certain extent for patients wit

Uncertainty quantification of U-Net based segmentation tool using conformal prediction.

BACKGROUND: The radiation therapy treatment process is very labour intensive, and artificial intelligence (AI) based auto contouring tools are increasingly being adopted to improve efficiency. However, current acceptance testing of AI auto contouring algorithms relies primarily on area- and distance-based metrics, with limited assessment of model uncertainty. PURPOSE: To demonstrate conformal prediction as a complementary error analysis technique for AI auto contouring algorithms, providing spatially localized uncertainty information that traditional metrics do not capture. METHODS: A U-Net

Effect of dose constraint on the thyroid gland during locoregional intensity-modulated radiotherapy in breast cancer patients.

The aim of the present study was to compare radiation dose received by thyroid gland using different radiotherapy (RT) techniques with or without thyroid dose constraint (DC) for breast cancer patients. Computerized tomography (CT) image sets for 10 patients with breast cancer were selected. All patients were treated originally with opposite tangential field-in field (FinF) for the chest wall and anteroposterior fields for the ipsilateral supraclavicular field. The thyroid gland was not contoured on the CT images at the time of the original scheduled treatment. Four new treatment plans were cr

Dosimetric impact of cylinder size in high-dose rate vaginal cuff brachytherapy (VCBT) for primary endometrial cancer.

The purpose of this study was to evaluate the dosimetric impact of cylinder size in high-dose-rate (HDR) vaginal cuff brachytherapy (VCBT). Sample plans of HDR VCBT in a list of cylinders ranging from 2.5 to 4 cm in diameter at 0.5 cm incre-ment were created and analyzed. The doses were prescribed either at the 0.5cm depth with 5.5 Gy for 4 fractions or at the cylinder surface with 8.8 Gy for 4 frac-tions, in various treatment lengths. A 0.5 cm shell volume called PTV_Eval was contoured for each plan and served as the target volume for dosimetric evaluation. The cumulative and differential dos

Evaluation of registration accuracy using short-arc cone-beam computed tomography for image-guided radiotherapy in breast cancer.

BACKGROUND: Image-guided radiotherapy (IGRT) using cone-beam computed tomography (CBCT) enables highly accurate imaging registration; however, increased radiation exposure remains a concern. Recently, short-arc CBCT (SA-CBCT), which reduces projection data and reconstructs images, has become available. Although previous studies have shown that image quality deteriorates when projection data are reduced, no study has directly evaluated the clinical usefulness of SA-CBCT or its effect on registration accuracy. PURPOSE: We verified the registration accuracy of SA-CBCT in breast tangential irradi
