Assessing tumor volumetric reduction with consideration for setup errors based on mathematical tumor model and microdosimetric kinetic model in single-isocenter VMAT for brain metastases.

The volumetric reduction rate (VRR) was evaluated with consideration for six degrees-of-freedom (6DoF) patient setup errors based on a mathematical tumor model in single-isocenter volumetric modulated arc therapy (SI-VMAT) for brain metastases. Simulated gross tumor volumes (GTV) of 1.0 cm and dose distribution were created (27 Gy/3 fractions). The distance between the GTV center and isocenter (d) was set at 0-10 cm. The GTV was translated within 0-1.0 mm (Trans) and rotated within 0-1.0° (Rot) in the three axis directions using affine transformation. The tumor growth volume was calculated usi

Characterization of a novel large-area electronic portal imaging device system for a ring-shaped HalosTx linear accelerator in radiotherapy quality assurance.

BACKGROUND: Electronic portal imaging device (EPID)-based in vivo dosimetry (IVD) has attracted considerable interest in recent years. Nevertheless, the limited detection area of conventional EPID systems has hindered their broad clinical adoption. Therefore, this study evaluated a novel large-area EPID (65 cm × 61 cm) integrated into the HalosTx linear accelerator (linac), focusing on stability, imaging quality, field coverage, and error sensitivity for radiotherapy quality assurance (QA). METHODS: Basic physical characteristics, including short-term repeatability, dose-response linearity, t

Comparative Evaluation of Setup Errors Using Planar Imaging and Cone Beam Computed Tomography: Feasibility of Asymmetric Planning Margins.

Background Accurate patient positioning in radiotherapy is critical to ensure adequate dose delivery to tumors and minimize radiation dose to normal tissues. Two image-guided radiotherapy (IGRT) techniques, planar and cone beam computed tomography (CBCT), are commonly used to measure patient setup deviations. Objective This study aims to compare the mean setup errors between planar and CBCT procedures in various irradiated areas, across the three orthogonal axes and at different times of day (morning-afternoon), and to evaluate whether the use of asymmetric planning margins between CTV and PTV

Time is NTCP: Should we maximize patient throughput or perform online adaptation on proton therapy systems?

BACKGROUND: Compared to conventional radiotherapy (XT), proton therapy (PT) may improve normal tissue complication probabilities (NTCP). However, PT typically requires higher adaptation rates due to an increased sensitivity to anatomical changes. Systematic online adaptation may address this issue, but it requires additional replanning time, decreasing patient throughput. Therefore, less patients would benefit in such case from PT for a given machine capacity, with results in worse NTCP. AIM: To investigate the trade-off between PT patient throughput and NTCP gain as a function of the time ne

Feasibility of transitioning to marker-less immobilisation using optical surface guidance in head and neck cancer treatment: a study at territory state cancer centre in a developing country.

PURPOSE/OBJECTIVES: In countries such as India, where a large share of cancer patients undergoes head and neck radiotherapy, setups still rely primarily on laser alignment with surface markers on the mask. This study explores transitioning from conventional marker-based setup to markerless setups using optical surface guidance (SGRT) to determine if treatment accuracy can be maintained particularly in a territory state cancer centre setting where resources are limited. MATERIALS/METHODS: A prospective study of 80 head-and-neck cancer patients, randomized into two groups was performed: open-fa
