Distal radius nonunion after volar locking plate fixation of a distal radius fracture: a case report.

We describe a 59-year-old man who had nonunion of a right distal radius fracture after volar locking plate fixation. He underwent open reduction and internal fixation with a volar locking plate system for a dorsally displaced, unstable distal radius fracture at a previous hospital 5 months ago. Radiographs of the injured wrist showed nonunion of the distal radius with 1.5-mm ulnar minus variance. Radiographs of the unaffected wrist showed 3.5-mm ulnar plus variance. Intraoperative findings of surgical revision showed an unstable nonunion; thus, debridement of the nonunion, autogenous inlay bon

Distal Radius Fractures in the Elderly.

Distal radius fractures are common in elderly patients, and the incidence continues to increase as the population ages. The goal of treatment is to provide a painless extremity with good function. In surgical decision making, special attention should be given to the patient's bone quality and functional activity level. Most of these fractures can be treated nonsurgically, and careful closed reduction should aim for maintenance of anatomic alignment with a focus on protecting fragile soft tissues. Locked plating is typically used for fracture management when surgical fixation is appropriate. Su

Exposure of the forearm and distal radius.

Approaches to the forearm use internervous planes to allow adequate bone exposure and prevent muscle denervation. The Henry approach utilizes the plane between muscles supplied by the median and radial nerves. The Thompson approach utilizes the plane between muscles supplied by the radial and posterior interosseous nerves. The distal radius may be approached volarly. The extended flexor carpi radialis approach is useful for intraarticular fractures, subacute fractures, and malunions. The distal radius can be approached dorsally by releasing the third dorsal compartment and continuing the disse

Management of malunions of the distal radius.

Despite encouraging results from small case series, correction of distal radius malunion remains a challenging procedure with uncertain outcomes. The most appropriate treatment for a distal radius malunion is prevention. If a symptomatic malunion is discovered, correction should be undertaken as early as possible. It is recommended that action be taken within six months of the primary injury to decrease the negative impact of soft-tissue contracture on the eventual reconstruction. Although some patients complain about residual problems after malunion surgery, corrective surgery has been shown

Surgical exposures of the radius and ulna.

The forearm contains many muscles, nerves, and vascular structures that change position on forearm rotation. Exposure of the radial shaft is best achieved with the Henry (volar) or Thompson (dorsal) approach. The volar flexor carpi radialis approaches are used increasingly for exposure of the distal radius. Although the dorsal approach is a safe utilitarian option with many applications, its use for managing fracture of the distal radius has waned. Potential complications associated with radial exposure include injury to the superficial branch of the radial nerve, the lateral antebrachial cuta

[Pseudarthrosis of the radius in a child with neurofibromatosis].

In children, pseudarthrosis of the forearm is generally associated to neurofibromatosis. It results from a pathological fracture because it appears in a bone that is considered abnormal for the presence of deformity or cyst. The authors present the case of a child who developed pseudartrosis of the radio after fracture in a bone without structural alteration. Only four similar cases can be found in the literature. The authors discuss its treatment.

Open fractures of the distal radius.

A review of 18 patients treated at our institution for open distal radius fractures was performed. The patients were followed up for an average of 32 months (range, 12 months to 5.3 years). Wounds were classified according to Gustilo and Anderson with 9 type I (50%), 3 type II (17%), and 6 type III (34%) (3 IIIa, 2 IIIb, 1 IIIc) injuries. Seven patients underwent a total of 2 to 4 procedures to treat their injuries, and 3 patients required 5 or more procedures. Eleven (61%) patients had postoperative complications. Patients were evaluated with the Disabilities of the Arm, Shoulder, and Hand qu

African histoplasmosis: osteomyelitis of the radius.

Bone lesions in African histoplasmosis caused by Histoplasma capsulatum var, duboisii, without other signs of the infection, are often misdiagnosed as cancer. A case of isolated osteomyelitis due to this fungus in a 30-year-old woman is described. Diagnostic and therapeutic problems of isolated bone lesions in African histoplasmosis are discussed. A high diagnostic index of African histoplasmosis is advocated in isolated bone lesions in the endemic region of this disease.

Fractures of the distal radius and scaphoid.

Simultaneous fractures of the distal radius and scaphoid is an uncommon wrist injury. During the period 1980 to 1993, 23 patients with a median age of 39 years were treated for this injury. The median follow-up period was 7 years. All scaphoid fractures were undisplaced and located in the waist or distal third. An extraarticular distal radial fracture was observed in 15 wrists. A below elbow cast including the thumb was applied in 18 wrists for a mean duration of 9 weeks, and operative treatment was used in only 5 patients. All scaphoid fractures healed without complications. During conservati

Vascularisation of the head of the radius in the adult.

Although rare, ischemic necrosis of the head of the radius after fracture is a theoretic possibility. The epiphyseal vascularisation of the proximal end of the radius was studied by dissection and diaphanisation techniques. The main extra-osseous supply is derived from epiphyseo-metaphyseal arteries given off by the recurrent radial a. and the first collateral of the ulnar a. These, to the number of three or four, anastomose together in a peri-cervical arterial circle continuous with the periosteal vessels of the shaft. From the proximal part of this network there arise three main intra-epiphy

Treatment of malunion of the distal radius.

Malunions of the distal radius are common. Although the best treatment is prevention, symptomatic malunions can be salvaged through more careful fracture management, by corrective osteotomy or, in the presence of post-traumatic arthritis, radiocarpal arthrodesis. Dynamic midcarpal instability following radius malunion does not respond as well to ligament repair or intercarpal arthrodesis as it does to osteotomy of the radius and correction of the malunion deformity.

ESTES recommendations regarding distal radius fractures.

Distal radius fractures (DRFs) account for 15% of all fractures in adults. Optimal treatment of DRFs depend on patient and fracture characteristics. Non- or minimally displaced DRFs can be treated nonoperatively with a cast. Although surgical treatment is becoming more popular, the majority of patients with a DRF are still treated nonoperatively. In addition, these considerations may be different for elderly patients. Conflicting evidence exists on the different aspects of treatment of DRFs. These recommendations regarding distal radius fractures describe considerations regarding diagnostic mo

Distal Radius Fracture with Dorsal Angulation.

Distal radial fractures (DRFs) are prevalent injuries and represent one of the most common fractures seen in the Emergency Department (ED). DRFs often result from falls on an outstretched hand (FOOSH). This case study details a 64-year-old female who sustained a distal radial fracture with dorsal angulations (DRFDA) from ice skating, confirmed via radiographic evaluation. Initial treatment involved immobilization, pain management, and follow-up with orthopedic specialists. Proper diagnosis and management are essential to prevent long-term functional impairments, with treatment ranging from cas

[Conservative management of distal radius fractures].

Distal radius fractures are the third most frequent fractures in Germany. The decision for conservative treatment and the weighing up of surgical treatment need an exact consideration of the indications based on instability criteria and the extent of possible articular involvement. Indications for an emergency operation must be excluded. In cases of stable fractures or multimorbid patients in a poor general condition conservative treatment is indicated. The principles for a successful treatment are the precise reduction and stable retention in a plaster splint. In the further course, fractures

OBJECTIVE: The aim of surgical treatment is fracture healing with restored alignment, rotation, and joint surface. Stable fixation allows for functional postoperative aftercare. INDICATIONS: Displaced intra- and extra-articular fractures which either could not be adequately reduced or in which a secondary displacement is to expected due to instability criteria. The following factors are considered instability criteria: age > 60 years, female, initial dorsal displacement > 20°, dorsal comminution, radial shortening > 5 mm, palmar displacement. CONTRAINDICATIONS: The only absolute contraindica
