Concomitant trimodality therapy of re-irradiation, chemotherapy and regional hyperthermia for a pretreated inoperable sarcoma recurrence.

We hereby present a case of pre-treated unresectable sarcoma recurrence of the trunk which showed an excellent response to concomitant tri-modal therapy, consisting of re-irradiation, chemotherapy and regional hyperthermia even with a strong compromised re-irradiation dose. No significant toxicity of the combined therapy and fast achievement of the pain and neurological symptoms relief are reported. The case shows that concurrent tri-modality treatment can be considered as a therapeutic option for the management of pre-treated unresectable recurrence even in there-irradiation setting.

Case Report: Simultaneous stereotactic re-irradiation using HyperArc for 44 brain metastases after whole-brain radiotherapy.

BACKGROUND: Treatment options for disseminated intracranial recurrence after whole-brain radiotherapy (WBRT) are limited. Repeat WBRT offers modest efficacy and may worsen neurocognitive function. Modern single-isocenter techniques such as HyperArc may enable efficient stereotactic radiotherapy (SRT) of numerous brain metastases. CASE PRESENTATION: A 62-year-old man with extensive-stage small-cell lung cancer and more than 40 brain metastases received systemic therapy followed by WBRT with 30 Gy in 10 fractions, resulting in near-complete intracranial regression. Approximately ten months late

Re-irradiation versus systemic therapy for the management of local-regionally recurrent head and neck cancer.

PURPOSE: The optimal management of local-regionally recurrent head and neck cancer that is not amenable to surgical resection is uncertain. We sought to compare outcomes among patients treated with and without re-irradiation in this setting. METHODS AND MATERIALS: A review of institutional registries identified 65 patients with local-regionally recurrent squamous cell carcinoma of the head and neck who were ineligible for surgery. Forty patients (62 %) opted for re-irradiation with the remaining 25 patients (38 %) undergoing initial systemic therapy alone. All patients had measurable disease.

Stereotactic Re-Irradiation for Local Recurrence after Radical Prostatectomy and Radiation Therapy: A Retrospective Multicenter Study.

Prostate cancer recurrence in patients previously treated with radical prostatectomy and radiation therapy is challenging. Re-irradiation could be an option, but data regarding efficacy and safety are lacking. We retrospectively evaluated salvage re-irradiation for local recurrence after prostatectomy and external beam radiation therapy. We collected data from 48 patients who underwent salvage reirradiation with stereotactic radiation therapy for local prostate cancer recurrence in the prostatic bed at four French centers. Fifteen patients (31%) were on androgen deprivation therapy during ster

Brachytherapy is an effective and safe salvage option for re-irradiation in recurrent glioblastoma (rGBM): A systematic review.

PURPOSE: To evaluate the clinical efficacy and toxicity of brachytherapy as a salvage therapy for patients with recurrent glioblastoma (rGBM). METHODS AND MATERIALS: We searched the PubMed, Embase, and Cochrane libraries from its inception to June 2023, for eligible studies in which patients underwent brachytherapy for rGBM. Outcomes of interest were mOS, mPFS, OS, PFS, and adverse events (AEs). For individual clinical survival outcomes and common AEs, weighted-mean descriptive statistics were calculated as a summary measure using study sample size as the weight. The calculation formula is as

Eribulin combined with radiation therapy in a young patient re-irradiated for a new lesion of breast cancer.

Eribulin is widely used in the treatment of metastatic breast cancer, with a manageable toxicity profile. This aggressive disease often requires systemic and local treatments, comprising surgery or radiotherapy. However, eribulin is usually discontinued during radiation therapy due to the lack of data concerning the safety of this combination, especially in the setting of repeat locoregional radiation therapy. Our patient was diagnosed with ER positive invasive ductal carcinoma of the left breast initially treated by surgery, radiation therapy, chemotherapy, and hormone therapy. She then recei

Re-irradiation with protons or heavy ions with focus on head and neck, skull base and brain malignancies.

Re-irradiation can offer a potentially curative solution in case of progression after initial therapy; however, a second course of radiotherapy can be associated with an increased risk of severe side-effects. Particle therapy with protons and especially carbon ions spares surrounding tissue better than most photon techniques, thus it is of high potential for re-irradiation. Irradiation of tumors of the brain, head and neck and skull base involves several delicate risk organs, e.g. optic system, brainstem, salivary gland or swallowing muscles. Adequate local control rates with tolerable side-ef

Longitudinal Swallowing and Salivary Changes With CLR 131 and Re-Irradiation in Recurrent Head and Neck Cancer.

BACKGROUND: Patients with recurrent head and neck cancer (HNC) often present with severe, persistent dysphagia and xerostomia following prior chemoradiation. Although swallowing impairments and salivary dysfunction have been reported in this population, prior longitudinal studies have not examined changes in salivary composition or how these changes relate to swallowing physiology. PURPOSE: This study characterized longitudinal changes in swallowing and salivary function-including composition-following treatment with CLR 131, a novel tumor-selective radiotherapeutic, combined with external be

Evaluating the Role of Re-Irradiation in the Management of DIPG: A Systematic Review and Meta-Analysis of Current Evidence.

OBJECTIVE: Overall survival (OS) of patients with diffuse intrinsic pontine glioma (DIPG) is poor. Re-irradiation (re-RT) represents an emerging strategy aimed at improving outcomes for patients who experience recurrence or progression after initial radiation therapy (RT). While re-RT is increasingly used at progression, its survival benefit lacks robust quantification. This systematic review and meta-analysis aims to evaluate the impact of re-RT timing on OS and toxicity in pediatric DIPG. METHODS: This review was conducted in the PubMed/MEDLINE, Europe PMC, and Scopus (2014-2024), following

Re-irradiation with cetuximab or cisplatin-based chemotherapy for recurrent squamous cell carcinoma of the head and neck.

PURPOSE: Locoregional recurrence remains the main pattern of failure after primary combined modality treatment of squamous cell carcinoma of the head and neck (SCCHN). We compared the efficacy and toxicity of either cisplatin or cetuximab in combination with re-irradiation (ReRT) for recurrent unresectable SCCHN. Various clinicopathological factors were investigated to establish a prognostic score. PATIENTS AND METHODS: Between 2007 and 2014, 66 patients with recurrent SCCHN originating in a previously irradiated area received cetuximab (n = 33) or cisplatin-based chemotherapy (n = 33) concom

Cerebellar re-irradiation after whole brain radiotherapy significant symptom relief with minimal toxicity in metastatic brain patients.

Whole-brain radiotherapy (WBRT) remains a standard treatment for extensive brain metastases, providing symptom relief and improved progression-free survival (PFS). Re-irradiation is often necessary for recurrent disease, particularly in the cerebellum, which accounts for 10-20% of cases. Cerebellar metastases are associated with distinct symptoms and poorer prognoses compared to supratentorial lesions. This study evaluates the outcomes of cerebellar-only re-irradiation for brain metastases, with or without stereotactic radiosurgery (SRS) for supratentorial lesions. A retrospective analysis of

Pharyngeal mucositis and esophagitis after re-irradiation spine stereotactic body radiotherapy with 24 Gy in two fractions.

BACKGROUND: Pharyngeal mucositis and esophagitis incidence following re-irradiation stereotactic body radiotherapy (SBRT) for spinal metastases remains unclear. We retrospectively examined their incidence and risk factors after re-irradiation SBRT to clarify organ tolerance. METHODS AND MATERIALS: Patients who underwent spine SBRT following prior conventional radiotherapy were included if the SBRT delivered more than 10 Gy to the pharynx or esophagus. SBRT dose was limited to 24 Gy in two fractions. Dose constraints were pharyngeal D1 cc < 20, esophageal D2.5 cc < 19, and D0.035 cc < 24 Gy. T

Initial feasibility cohort of temporally modulated pulsed proton re-irradiation (TMPPR) for recurrent high-grade intracranial malignancies.

Recurrent high-grade intracranial malignancies have a grim prognosis and uniform management guidelines are lacking. Re-irradiation is underused due to concerns about irreversible side effects. Pulsed-reduced dose rate radiotherapy (PRDR) aims to reduce toxicity while improving tumor control by exploiting dose-rate effects. We share our initial experience with temporally modulated pulsed proton re-irradiation (TMPPR), focusing on workflow, safety, feasibility, and outcomes for the first patient cohort. TMPPR was administered to patients with recurrent or progressive central nervous system malig

Prognosis Forecast of Re-Irradiation for Recurrent Nasopharyngeal Carcinoma Based on Deep Learning Multi-Modal Information Fusion.

Radiation therapy is the primary treatment for recurrent nasopharyngeal carcinoma. However, it may induce necrosis of the nasopharynx, leading to severe complications such as bleeding and headache. Therefore, forecasting necrosis of the nasopharynx and initiating timely clinical intervention has important implications for reducing complications caused by re-irradiation. This research informs clinical decision-making by making predictions on re-irradiation of recurrent nasopharyngeal carcinoma using deep learning multi-modal information fusion between multi-sequence nuclear magnetic resonance i

Validation of nomogram-based prediction of survival probability after salvage re-irradiation of head and neck cancer.

OBJECTIVE: Treatment outcomes after salvage re-irradiation in patients with recurrent head and neck cancer vary widely due to heterogeneous patient characteristics, and it is difficult to evaluate optimal re-irradiation schedules. This study aimed to validate a nomogram, originally developed by Tanvetyanon et al., used to predict the survival probability of patients with recurrent head and neck cancer after re-irradiation. METHODS: Twenty-eight patients with recurrent head and neck cancer who underwent salvage re-irradiation between June 2007 and November 2011 were evaluated. The median total
