Bradycardia and the trigeminal nerve.

A review of the literature suggests that the oculocardiac reflex is part of a wider phenomenon involving any structure supplied by the trigeminal nerve. This can lead to potentially fatal complications in maxillofacial surgery. A review of the literature is discussed and two further cases are reported.

Fulminant acute inflammatory demyelinating polyradiculoneuropathy: case report and literature review.

BACKGROUND: The inflammatory polyradiculoneuropathies (Guillain-Barré Syndrome) represent a diverse spectrum of diseases, with variable pathophysiological mechanisms (demyelinating versus axonal), clinical manifestations (motor, sensory, and autonomic), presentation patterns, and degrees of severity. Fulminant cases of total body paralysis with loss of all cranial nerve reflexes have been described, whereby patients appear to have lost all central nervous system (CNS) function. METHODS: The case of a patient with a fulminant polyradiculoneuropathy in whom consideration was given to discontinu

Paediatric orbital trapdoor fracture misdiagnosed as a head injury: a cautionary tale!

Trapdoor fractures, otherwise known as 'white-eyed blowout' fractures, occur predominantly in the paediatric cohort and have a male predilection. Patients commonly present with acute fractures to the emergency department, and delayed diagnosis can result in significant morbidity. A lack of external signs, such as oedema or ecchymosis, often misleads physicians into underestimating the seriousness of the injury. It can be initially misdiagnosed as a head injury due to the oculocardiac reflex, nausea, vomiting, poor patient compliance and a failure to examine the eye appropriately. The incarcera

An asystolic event after eyelid skin bupivicaine injection during chalazion surgery.

We report an asystolic event in an 8-year-old girl under general anesthesia that occurred after bupivicaine was injected into the eyelid during routine chalazion incision and drainage surgery. Because the bupivacaine was injected directly into the subcuticular space and not intravascularly, we concluded that the patient's asystolic event was induced by an oculocardiac reflex.

The gastroauricular phenomenon and related vagus reflexes.

Eight lesser known reflexes were grouped together because of their anatomical and physiological relationship. In all of them a branch of the vagus nerve forms a bridge between a circumscribed area of the skin, mostly the external auditory meatus, and an internal organ, namely the stomach, esophagus, lungs, heart, uterus, and some male and female sex organs. The eight reflexes are: (1) Gastroauricular phenomenon (Gaph) (Engel, 1922) in man; (2) Auricular phenomenon (Malherbe, 1958) in man; (3) Pulmonoauricular phenomenon (Deutsch, 1919) in man; (4) Auriculogenital reflex (Bradford, 1937) in cat

Sudden arrhythmia followed by bradycardia during sagittal split ramus osteotomy: trigeminocardiac reflex.

Trigeminocardiac reflex (TCR) is a brainstem reflex triggered by the stimulation of any branch of the fifth cranial nerve along its course, presenting as a reduction in heart rate and blood pressure. Oculocardiac reflex is a well-known subtype of TCR. In the case reported here, remarkable arrhythmia followed by bradycardia occurred suddenly in a healthy patient undergoing orthognathic surgery. The heart rhythm recovered when the surgical manipulation ceased, but bradycardia was reproduced when the surgery resumed. This case of TCR is unique in that remarkable arrhythmia first appeared and led

Does Topical Proparacaine Improve Postoperative Comfort After Strabismus Surgery?

BACKGROUND: Strabismus surgery is often performed on children and adults as a quick-turnover, outpatient procedure under general anesthesia. Ideal methods to reduce post-operative pain and nausea are not yet perfected. We postulated that a simple topical anesthetic drop after surgery might help. METHODS: In a prospective study of oculocardiac reflex (OCR) and strabismus surgery, a cohort of ongoing patients either received proparacaine immediately post-op, or none. Co-variables were Intraoperative opioid and OCR, patient age, type of surgery. Several post-operative recovery outcome variables

The possible role of nystagmus in motion sickness: a hypothesis.

An alternative hypothesis to that provided by conflict theory is formulated to account for motion sickness. The new approach is predicted on the oculocardiac reflex (i.e., bradycardia produced by extra-ocular muscle traction) (18) and empirical evidence that retrobulbar anesthesia significantly lowers the incidence of emesis after strabismus surgery (14). Eye muscle traction is presumed to elicit afferent signals that ultimately stimulate the vagus nerve (15). The same neuromotor sequence is presumed to occur during reflexive eye movements under vestibular control and during more complicated c

Forensic aspects of ocular injury.

A case of homicidal stabbing resulting in bilateral penetrating ocular injuries is described. The case is noteworthy in that it highlights an unusual mechanism of death in homicidal stabbing. Disturbances in heart rhythm including asystole can be ascribed to the so-called oculocardiac or trigeminocardiac reflex. Although this phenomenon is well known to ophthalmologists, neurosurgeons, and anesthetists, it is much less familiar to forensic pathologists. This is a potential mechanism of death worthy of consideration in cases of sudden unexpected death occurring in the context of facial injury.

Auditory brain-stem responses in comatose patients: relationship with brain-stem reflexes and levels of coma.

Auditory brain-stem responses (BSR) were recorded in 20 comatose patients in whom the level of brain-stem dysfunction was defined by clinical assessment of brain-stem reflexes and posture. No BSR abnormalities were found in the 10 cases with cortico-subcortical or diencephalic levels. The other 10 patients showed a clear relationship between alteration of the different components of the BSR and the clinical levels of brain-stem dysfunction caused by the rostro-caudal evolution. Alteration of wave P5 seems related to a midbrain dysfunction, of P3 to a pontine dysfunction and of P1 or P2 to a lo

Anesthetic management of ophthalmic surgery.

Ophthalmic surgery presents challenges for the anesthesiologists, including control of intraocular pressure, prevention and management of the oculocardiac reflex. In addition to understanding ocular anatomy and physiology, the anesthesiologists must have possess technical expertise and knowledge of ophthalmic drug's systemic effects. Patients undergoing ophthalmic surgery have extremes of age and several medical diseases, like as hypertension, diabetes mellitus, coronary heart disease, chronic renal failure, and chronic obstructive lung disease. Anesthesiologist should be knowledgeable about t

Systemic complications associated with retinal cryoablation for retinopathy of prematurity.

Eighty infants with proliferative retinopathy of prematurity (ROP) were treated with peripheral retinal cryoablation. Among the serious systemic complications encountered were three instances of respiratory arrest and one of cardiorespiratory arrest. Recommendations that may help prevent these adverse systemic effects in premature infants include: (1) avoidance of excess subconjunctival anesthetic doses, (2) preoperative administration of systemic atropine to minimize the oculocardiac reflex, (3) consideration of an analgesic agent to decrease the pain and exhaustion, and (4) cardiorespiratory

The Anesthetic Challenges of Caring for a Pediatric Patient With Incontinentia Pigmenti: A Case Report.

Incontinentia pigmenti (IP) is a rare X-linked dominant disorder. We present a case of an infant with IP who was brought to the operating room for panretinal diode photocoagulation under general anesthesia. The anesthesia team was unable to obtain intravenous access even with instruments such as a vein finder and ultrasound. Anesthesia for IP patients also poses challenges such as prevention of the oculocardiac reflex, obesity and airway management, and preemptive measures for intravenous access due to skin manifestations. Patients with IP may present with many challenges for the anesthesiolog

[Syndrome of vagal hypertonia in a newborn infant manifested during the surgical repair of aortic coarctation associated with banding of the pulmonary artery].

We report a case of vagal hypertonia syndrome in a newborn infant, developed after surgical repair of an aortic coarctation combined with banding of the pulmonary artery trunk. The parasympathetic activity had adverse repercussions on haemodynamics. The diagnosis was confirmed by prolonged asystole on the oculocardiac reflex and by concomitant arrhythmia and disorders of conduction demonstrated by Holter recordings. To our knowledge, no other case of vagal hypertonia associated with a congenital cardiopathy has yet been reported. Infants with this syndrome are at a high risk of sudden death. T

Overview of pediatric orbital fractures.

Orbital fractures in children are uncommon. The pattern of orbital fractures changes as children age. Although the management of pediatric orbital fractures is evolving, a thorough clinical assessment with computed tomographic scan imaging is essential. Urgent surgical intervention is indicated in cases of entrapment or acute enophthalmos. Entrapment with oculocardiac reflex is common in the white-eyed blow-out or trapdoor fractures. Otherwise, pediatric fractures may be treated conservatively with surveillance. A variety of autogenous and allogenic materials may be used to repair the fracture
